Multicenter private practice comparison of tripamide and hydrochlorothiazide in the treatment of hypertension.
One hundred fifty-nine patients with essential hypertension were randomly assigned to take 10 mg of tripamide or 50 mg of hydrochlorothiazide once a day for 12 weeks. Both drugs were equally effective in lowering both systolic and diastolic blood pressure in the supine (6/5 vs. 5/5 mm Hg) and standing (8/5 vs 7/6 mm Hg) positions. Tripamide had less effect on serum potassium (0.2 vs. 0.6 mEq/L decrease) and serum uric acid (0.7 vs. 1.2 mg/dL increase) than hydrochlorothiazide. The addition of propranolol to either drug produced a further reduction in blood pressure. The overall incidence and spectrum of adverse reactions were the same for both drugs.

Cancer surveillance is an important part of the follow-up of the cancer patient. Guidelines are given for the follow-up of the eight most common cancers. Psychological support is an ongoing need that the primary physician should supply.

The suicidal patient. The primary care physician's role in evaluation and treatment.
Evaluation of the suicidal patient involves careful consideration of risk factors, including attention to subtle clues from the history, and mental status examination. The most urgent decision for the physician to make is whether or not to insist on hospitalization. If the need for inpatient care is uncertain, a "no suicide" contract can be a helpful adjunct to management. Although suicidally depressed or psychotic patients require psychiatric consultation, many "suicidal" patients are simply experiencing a crisis and respond well to supportive crisis intervention counseling.

The muscular dystrophies.
Duchenne muscular dystrophy, the most common childhood-onset muscular dystrophy, is X-linked and is associated with cardiac and mental abnormalities. Becker's muscular dystrophy is similar to but milder than Duchenne muscular dystrophy. The rare facioscapulohumeral muscular dystrophy has an autosomal dominant mode of transmission. Myotonic muscular dystrophy is the most common of the adult-onset muscular dystrophies. Treatable diseases that must be excluded include polymyositis, potassium disorders and endocrine abnormalities.

What do family members expect from the family physician of a deceased loved one?
BACKGROUND AND OBJECTIVES: This study examined what actions are expected from a family physician by the family members of a deceased patient immediately following the death of the patient. METHODS: Those listed as next of kin on the charts of deceased patients of a suburban family practice residency were asked to complete a questionnaire about their expectations of their family member's family physician after the death. RESULTS: Expectation of the surveyed actions was surprisingly low, with attendance at the funeral the least expected, followed closely by acknowledgment of the anniversary of the family member's death. Nearly half of the respondents expected a telephone call shortly after the death, while close to one third expected the physician to ask about their emotional well-being at their next visit. CONCLUSIONS: The results suggest that family members do not expect or rarely expect any action from their family member's family physician after the death. However, comments received with the survey indicate that any gesture of sympathy or show of concern from the family physician was greatly appreciated by the survivors.
