[Endoscopic and clinical parameters in assessing th degree of portal hypertension: the value of the serum-ascitic fluid albumin gradient].

A prospective study in thirty one patients with ascites, who were hospitalized at Cayetano Heredia National Hospital (H.N.C.H.), to investigate the association between the high serum-to-ascites albumin concentration gradient (high GRAD-Alb) with the degree and development of oesophageal varices, studied by endoscopy, is here reported. It was also studied its relationship with the degree of impairment of liver function, determined by the Child-Pugh's score. In addition our series found that the degree of high GRAD-Alb could discriminate patients with oesophageal varices finding like a signal of its presence a value of high GRAD-Alb greater than 1.435 +/- 0.015 g/dl. It is here demonstrated that the degree of high GRAD-Alb does not have any relationship with the degree of impairment of liver function (Child-Pugh's score), prothrombine time, serum bilirrubin, degree of encephalopathy neither grade of ascites; however, we found a light association and correlation with serum albumin.

Exosurf versus Survanta surfactant preparations: proportional-hazards regression analysis of time to successful extubation and discontinuation of oxygen therapy.

Time to successful extubation (VENT-DC) and time to discontinuation of oxygen therapy (OX-DC) for infants treated with Exosurf and Survanta surfactant preparations were compared in an observational study. Proportional-hazards (Cox) regression was used to adjust for confounding by differences in baseline risk factors between treatment groups. The Exosurf/Survanta hazard ratios, representing the relative instantaneous probabilities of the next event (VENT-DC or OX-DC) occurring in the Exosurf versus Survanta groups, were close to the null value of 1.0 and were not statistically significant. For VENT-DC the ratio was 1.08 (95% confidence interval 0.73 to 1.61, p = 0.60); for OX-DC the ratio was 0.87 (0.58 to 1.30, p = 0.83). By contrast, the hazard ratios for female versus male sex and black versus white race, presented as internal reference values, were further from the null value and were statistically significant. Survival curves showing the estimated proportion of patients in whom VENT-DC or OX-DC was achieved over time were generated from the proportional hazards models. Our findings suggest that any difference between Exosurf and Survanta surfactant for VENT-DC or OX-DC is likely considerably smaller than the differences associated with gender or race. The rationales for (1) using proportional-hazards regression when duration of therapy is an outcome of interest and (2) generating internal reference risk ratios when multivariate analysis is used are discussed in relation to future studies of hyaline membrane disease or other problems of prematurity.

[Echographic parameters in the evaluation of the degree of portal hypertension].

We did a prospective study in thirty one patients with ascites who were hospitalized at Cayetano Heredia Hospital (H.N.C.H.). We investigated the association and correlation between ultrasonographic parameters of portal hypertension (HTP) and the presence and level of HTP (determinate by the serum-to-ascites albumin concentration gradient). In our study, we demonstrated that ultrasonographic splenomegaly studied by longitudinal diameter of the spleen discriminate patients with HTP with a high positive predictive value (94.4%), although it didn't happen with transverse diameter of the spleen. However patients with elevated measurements of high GRAD-Alb (> or = 2.35 +/- 0.05 g/dl) had alterations in the transversal diameter of the spleen with a high negative predictive value (91.7%), these findings were suggested its use like a parameter for reject high levels of HTP. In our study, we demonstrated that the existence of changes at diameters of portal vein and splenic vein could not discriminate patients with HTP. By the way, we didn't find association between degree of high GRAD-Alb and the different splenic measurements.
