[Experimental-clinical grounds for the treatment of granulating soft tissue wounds by the wound edge approximating stapler].

Factors of risk of the appearance of wound complications were studied in experiment on a model of granulating wounds in 18 mongrel dogs. A traditional method (control group) and an apparatus method (main group) were used for suturing the wounds. In the main group of animals suturing the wounds was performed after bringing together the wound edges by specially designed devices. Morphological and microbiological investigations have shown that the leading role in the appearance of complications belongs to the morphological substrate of the wound with the main component including the degree of traumatizing the paravulnerable tissue with a suture thread when connecting the wound edges by a traditional method, ischemia of the paravulnerable tissues under conditions of microbial dissemination. The introduction of the developed method of closing the soft tissue granulating wounds into clinical practice resulted in a statistically significant reduction of pyo-inflammatory complications in the main group of patients. A conclusion is made on a more favorable course of the wound process when using the apparatus method of closing the wounds.

[Distribution of alleles of DRB, DQB and DQA loci of major histocompatibility complex in healthy donors of Saint Peterburg].

The distribution of alleles of DRB, DQB and DQA loci was investigated in 118 healthy donors from St. Petersburg. Said alleles were identified by the restriction fragment length polymorphism method using DRB, DQB and DQA probes after TaqI digestion and Southern blotting. The frequencies of said alleles were compared with those of a cohort of healthy donors living in Germany. A significantly higher frequency of DRB-17-2 was identified in the Russian donors, as compared with the German counterparts. Also, the Russian donors revealed a decrease in the frequency of DQB-1 (P = 0.01) and an increase in that of DQB-X allele (P = 0.05). An analysis of DRB, DQB and DQA locus a alleles adhesion in a sample of donors from St. Petersburg showed it to agree, in a large proportion of cases, with the literature data. A parallel study of immunological specificity of DR antigen was undertaken to compare the results of genetic and serologic typing; errors in DR specificity identification were found to have been made in 17.0%.

[Preparation of an erythrocytic diagnostic agent based on staphylococcal proteinase].

Various methods for sheep red cell sensitization for the preparation of erythrocytic diagnostic agent from staphylococcal proteinase were under study. Sensitization with chromium chloride yielded the best results as regarded the sensitivity of the agent, optimal sensitizing dose, and sensitin consumption. The resultant erythrocytic diagnostic agent retained about 10% of the initial enzymic solution activity. The authors have demonstrated a high specificity of the erythrocytic diagnostic agent from staphylococcal proteinase, that may be used for the serologic diagnosis of staphylococcal infection and for proteinase indication in various staphylococcal preparations.

[Tissue typing using a Soviet automated luminescent microscope-photometer].

Automated fluorescent microscope developed by the authors permits photometry of microquantities of cellular suspensions in scanning standard 60-well plates with a flat bottom. Automated and semiautomated modes of operation are possible. Fluorescent stains and schemes of staining the examined cells that yield stable results in the lymphocytotoxic test have been selected. Comparative analysis of the efficacies of histocompatibility antigens detection by the routine and the fluorescent technique has shown a number of advantages of the fluorescent method. Specific features of fluorescent stains used for staining live cells are described.

[The contour characteristics of the mucous membrane of the hard palate as dependent on the form of its osseous base].

Analyzes the relief of the hard palate mucosa, depending on the shape of osseous base. Presents data on the prevalence of various morphological shapes of lateral folds in the hard palate mucosa and the levels of their disposition in the ipsiloid, ellipsoid, and parabolic osseous base of the hard palate. Demonstrates the possibility of using the characteristics of the hard palatal mucosa relief as an extra test for personality identification by the oral status.

[The possibilities for simulation in the expert assessment of the circumstances of the causes of head trauma].

Some aspects of simulation experiments in forensic medicine are discussed, specifically those in studies of biomechanics and expert evaluation of the probable circumstances of a head injury caused by falling on a plane from a standing posture. Based on mathematical simulation and multifactorial analysis of physical parameters characterizing the process of falling (inertia, angular velocity, etc.), the authors prove the possibility of differentiating between spontaneous falling of a victim and falling caused by an outer force, and estimating the value of this force.

[Evaluation of medical care quality in cases with health damage infliction leading to unfavorable outcomes].

Discusses modern approaches to evaluation of the quality of medical care in cases when civil action is brought against medical people for inflicting damage to health leading to unfavorable outcomes. Emphasizes the need in developing a special organization model of medical care quality and efficacy control system, which should be based on universal principles, notions, and adequate tests and evaluations.

[Expert evaluation of damage to health in dental injuries].

Discusses problems in forensic medical expert evaluation of the severity of dental injuries. Analyzes the classification of dental injuries and criteria for their expert evaluation. Considers some disputable and contradictory problems in expert interpretation of dental injuries involving complete loss of teeth and evaluation of injuries in subjects with maxillodental diseases. Offers approaches to solution of these problems.

[Forensic medical expert evaluation in connection with civil actions of patients complaining of improper dental care].

The authors discuss questions facing forensic medical experts in connection with civil "medical" actions. The questions are classified as common and private. Expert evaluations of correspondence of the scope and quality of dental care are offered as the base for analysis of these problems within the framework of civil legislation. The data evidence efficiency of forensic medical expert evaluations in civil legal procedure and the significance of forensic medical service in the solution of problems of quality and efficiency of medical (including dental) care of the population.

[Effect of vernalization and red light illumination of seedlings of bread wheat (Triticum aestivum L.) on the temperature profile of the cAMP phosphodiesterase activity].

Phenotypic manifestations of Vrn (vernalization) and Ppd (photoperiodism) genes responsible for transition of bread wheat Triticum aestivum L. to generative growth (flowering) are mutually related. Since the mechanism of phytochrome-induced photoperiodism involves the enzymes of cyclic adenosine monophosphate metabolism and phosphodiesterase in particular, we tested involvement of phosphodiesterase in the process of winter wheat vernalization and formation of flowering competence in alternate wheat requiring a long day but no vernalization for the transition to flowering. We studied temperature dependence of phosphodiesterase activity in vernalized and unvernalized winter wheat on the one hand and in etiolated and red light illuminated seedlings of alternate wheat on the other hand. Short-term experiments demonstrated that vernalization and red light illumination are similar to long day by the effect on the long-day plants. Both influences induced a pronounced inversion of the temperature profile of phosphodiesterase activity in the 28-45 degrees C range. We propose that phosphodiesterase is involved in vernalization processes and can serve as a sensor of low temperature in winter wheat. Changed temperature profile is a radical control mechanism of phosphodiesterase activity in response to the influences (red light and vernalizing temperatures) responsible for competence of various bread wheat forms for generative growth.
