The contributions of the right and left hemispheres of the brain to the polymorphism and heterogeneity of pyramidal syndrome.

This study was conducted to address the clinical and neurophysiological features of pyramidal syndrome in patients with lateralized hemispheric lesions. A total of 52 patients were studied, all with hemiparesis following acute cerebrovascular accidents of the ischemic type in the right (24 patients) or left (28 patients) hemispheres of the brain. Clinical neurological analysis was performed, along with transcranial magnetic stimulation, evoked abdominal reflexes, movement-associated motor potentials, and somatosensory evoked potentials. Lesions of the right hemisphere resulted in significant weakening of activatory influences at both the hemisphere level and on descending motor structures, with marked impairment of the afferent flow from both sides of the body and excessive disorganization of activity in the left, "presumptively healthy," side. Left-sided pathology impaired efferent control not only of the contralateral, but also of the ipsilateral side of the body. These data identify different neuroanatomical and neurophysiological features of the cerebral hemispheres not only in healthy people, but also in pathological conditions, this probably accounting for the polymorphism and heterogeneity of pyramidal syndrome in patients with lateralized cerebral lesions.

[Contribution of right and left brain hemispheres to pyramidal syndrome polymorphism and heterogeneity].

The study aimed at investigation of clinical and neurophysiologic peculiarities of pyramidal syndrome in lateralized hemispheric lesion. Fifty-two patients with hemi-paresis caused by acute ischemic attack in the right (24 patients) and left (28 patients) brain hemispheres have been studied, using clinico-neurological analysis, transcranial magnetic stimulation, evoked abdominal reflexes, movement-related method of motor potential and somatosensory evoked potentials. In right hemispheric lesion, significant reduction of activating influences both on hemispheric and descending movement structures, pronounced damage of afferent flow from both body sides and excessive disorganizing activity in the left "relatively healthy" hemisphere were observed. Left hemispheric pathological process disturbs efferent control not only in the counter lateral body side, but in the ipsilateral one as well. The results obtained allow to discuss different neuroanatomical and neurophysiologic peculiarities of brain hemispheres, both in normal and pathological cases, as a factor, possibly determining polymorphism and heterogeneity of pyramidal syndrome in lateralized cerebral lesion.

[Headache caused by sexual activity].

Headache caused by sexual activity, or coital headache, is included in International headache classification (1988). The paper presents 19 cases (15 males, 4 females, mean age 34 years) examined by the authors. In 58% of the cases, headache developed before orgasm, in 26%--during orgasm and in 16%--after it. Duration of severe headache was from several to 15 min and of moderate one--up to 7 h. In total, disease lasted from 1 week to 8 years. Focal neurological symptoms were absent; magnetic resonance tomography did not reveal any changes in 68% of the patients, while the others had mild hydrocephalia. However, there was a tension of jugular, trapeziform and, less often, of temporal muscles, and, in some of the patients, an increase of arterial pressure. Among psychological features, hypochondria in men and emotional lability in women were detected. According to the clinico-pathophysiological results obtained, different pathogenetic mechanisms of coital headache were suggested: (1) disturbance of venous outflow and dysfunction of antinociceptive systems; (2) increase of arterial pressure combined with tonic tension of pericranial muscles on the background of vertebrogenic disturbances.

[Clinical and neurophysiological analysis of pyramidal syndrome in right and left hemisphere stroke].

The study aimed at investigation of clinical and neurophysiological features of pyramidal syndrome in patients with vascular damage of right- and left hemisphere in stroke. Forty patients with consequences of acute brain circulation damage were studied: 21 of them with right hemi-paresis (RHP) and 19--with left hemi-paresis (LHP). Cliniconeurological analysis, transcranial magnetic stimulation (TMS), evoked abdominal reflexes, movement motor potential method and somatosensory evoked potentials were used for the examination. Pronounced paresis, tone disturbance, Babinsky symptom, other extensor pathological feet signs, Zhukovsky hand reflex, pseudobulbar syndrome, gait disorder were determined more frequently in patients with LHP. Patients with RHP were characterized by more expressed distal hand paresis and pathological bending feet signs. Compared to RHP patients, those with LHP displayed significantly longer central conduction in TMS, increase of latent periods of components N20, P25, N30 of somatosensory evoked potentials and motor potential area reduction. Functional features of brain hemispheres may to play a certain role in pyramid syndrome formation, right hemisphere damage being characterized by more expressed pyramid disturbances. The data obtained allow to differentiate a great brain hemispheres role in descending motor control as to inhibiting and activating pyramid influences and hemisphere interactions.
