Mediators of inflammation in blister fluids from patients with pemphigus vulgaris and bullous pemphigoid.

Blister fluids from 39 previously untreated patients with pemphigus vulgaris (PV) and 28 patients with bullous pemphigoid (BP) were tested for the presence of inflammatory mediators. Blister fluids from patients with PV and BP showed interleukin 1 (IL-1)- and interleukin 2 (IL-2)-like activity and contained prostaglandin (PG) E2, F1 alpha, and F2 alpha, thromboxane B2 (TB2), leukotriene (LT) B4 and C4, serine esterases, and proteolysis inhibitors. Pemphigus blister fluid was distinguished by high IL-1-like activity, proteolytic activity, and high concentrations of TB2 and LTB4, while pemphigoid blister fluid was characterized by higher IL-2-like activity, antiprotease activity, and high PGE2 concentration. We also determined that the content of mediators of inflammation varied depending on the duration of blister development. In both PV and BP, the initially high IL-1-like activity, amount of serine proteases, and concentrations of PGF2 alpha, TB2, LTB4, and LTC4 decreased by the fifth day of blister existence, whereas antiprotease activity as well as PGE2 and PGF1 alpha concentrations gradually increased as blisters developed. These findings suggest certain differences in the character of the bullous inflammatory process in pemphigus and pemphigoid.

Somatotrophic, thyrotrophic and adrenocorticotrophic functions of the anterior pituitary in obesity.

The basal plasma level of GH, TSH and ACTH and the reserve capacities of the adenohypophysis in the secretion of these hormones was investigated radioimmunologically in 80 patients with excessive weight (from 15.3% to 180.0%) and a group of healthy subjects. It is shown that obesity is accompanied by diverse changes of the anterior pituitary GH, TSH and ACTH functions, which may be explained by their varied participation in the fatty metabolism. The pituitary somatotrophic function is weakened in obese subjects and its disorders advance with increase of the grade of obesity. Reduced pituitary thyrotrophic function in obesity causes weakening of thyroid function; disorders of the thyroid regulation occur at the hypothalamic level. The increased blood plasma concentration of ACTH in obese subjects has a compensatory character. The reserve capacities of the adrenocorticotrophic function of the pituitary are diminished in obestiy. Disturbances of the anterior pituitary GH, TSH and ACTH functions show a single-direction character both in alimentary-constitutional and in hypothalamic obesity.

Glucagon secretion in subjects with prediabetes, diabetes mellitus and obesity.

The blood glucagon concentration (fasting and in insulin hypoglycemia) was determined by radioimmunoassay in diabetic patients, relatives of diabetic patients with a normal glucose tolerance test, patients with obesity and a group of normal weight subjects. The index of glucagon rise above the fasting level and glucagon release rate were estimated. In relatives of diabetic and obese patients the initial blood glucagon concentration did not differ from that of healthy subjects. However, during insulin hypoglycemia, glucagon secretion was significantly reduced, and in relatives of diabetic patients it also proved to be delayed. A comparison of glucagon and somatostatin changes in the above mentioned patients allows to suggest participation of the somatostatin mechanism in disorders of glucagon secretion.
