[Tactics in using antibiotics in diffuse forms of chronic nonspecific pneumonia in children].

The authors provide the results obtained during many years of studying different aspects of antibacterial treatment of purulent endobronchitis in children with prevalent forms of chronic pneumonia. The study was attempted at the clinic for disease of childhood, I.M. Sechenov Moscow Medical Academy. Base the complex program of such treatment, which rests on etiotropic , correspondence of the antibiotic administration to the activity of the infectious inflammatory process in the bronchi, and immunomodulating therapy carried out according to indications if any. Introduction of the antibiotic therapy program into clinical practice allowed the attainment of good direct results in 87.6% of patients and of stabilization of endobronchitis in 47% of patients within 1.5 to 3 years.

[The morphometric and immunohistochemical characteristics of the gastric and duodenal mucosa in systemic lupus erythematosus].

Histological, morphometric, and immunohistochemical studies of biopsy specimens from the mucosa of the body and antral portion of the stomach, the bulb and descending portion of the duodenum were performed in 27 children with systemic lupus erythematosis (SLE) and in 12 with chronic gastroduodenitis. The inflammatory infiltrate from SLE patients was found to contain higher levels of young and mature fibroblasts than that from patients with chronic gastroduodenitis, mature fibroblasts being prevalent. The inflammatory changes in the gastric and duodenal mucosa were ascertained to be associated with the progression of SLE. In exacerbation of SLE, the walls of vessels (arterioles) exhibited immune complexes classified mainly as IgG and, to a lesser degree, as IgM. In remission, the luminescence of the vessels decreased. The serum level of immunoglobins did not correlate with their regional production in the gastric and duodenal mucosa.

[Phagocytic activity of alveolar macrophages in children and adolescents with different bronchopulmonary pathological conditions].

A total of 68 children and adolescents suffering from tuberculosis, chronic pneumonia in the phase of exacerbation, pulmonary cysts and hamartochondroma were examined for the phagocytic activity of alveolar macrophages. In pulmonary tuberculosis, the low absorption properties of alveolar macrophages were established in all the phases of a specific process. The patients afflicted with chronic pneumonia were characterized in the phase of exacerbation by discreteness of the parameters of the activity of alveolar macrophages in the bronchoalveolar lavage fluid withdrawn from the focus of inflammation: the absorption activity increased whereas bactericidal one reduced.

[Rapid diagnosis of exacerbations of chronic bronchitis of pneumococcal etiology in children].

The diagnostic possibilities of the microscopic examination of bronchial secretion smears, stained by Gram's method, from 56 children with chronic bronchitis at different stages of exacerbation have been studied. The following criteria have been proposed for confirming the etiological role of pneumococci in cases of endobronchitis in children: the number of paired diplococci in the visual field must exceed 10 with polynuclears covering the whole field. In 84% of cases the use of this method makes it possible to determine the pneumococcal etiology of the disease within two hours from the arrival of the pathological material to a bacteriological laboratory; this method can also be used for evaluating the effectiveness of antibacterial therapy at its different stages.

[Morphological and endoscopic characteristics of chronic gastroduodenitis in children with lesions of the hepatobiliary system and pancreas].

Morphological and endoscopic features of a chronic gastroduodenitis combined with dyskinesia of bile ducts and pathology of hepatobiliary system and pancreas (chronic cholecystocholangitis, cholecystopancreatitis, "reactive" pancreatitis) are studied in 72 children. The most severe changes in the duodenal mucosa (chronic atrophy or, less frequently, diffuse duodenitis) were found in patients with dyskinesia of bile ducts and with disturbances of hepatobiliary system and pancreas. A characteristic feature of chronic duodenitis in patients with diseases of hepatobiliary system and pancreas lymphangiectasia in the villi.
