[Morphologic changes in spinal cord of patients with spinal injuries who died in hospital].

Morphologic changes in the spine have been investigated in patients with spinal injuries who died in hospital. The incidence of injuries has been evaluated as 9.3% of mechanical injuries. Purposeful search for possible injuries increased the rate of detection of injuries of the spine and the cord. Spinal cord can be impaired without involving the osseous formations and spinal ligaments. Edema of the cord tissue develops during the early period of injury and prevents the detection of injuries, persisting for a long time after the injury. Edemas are detected even in subjects who died at the site of accident.

[Morphological changes in lungs of patients with spinal injuries died in hospital].

Morphological characteristics of changes in the spine and lungs in patients with spinal injuries dead in hospital are described. Spinal edemas were detected in all cases. Edemas were found even in subjects dead at the place where the injury was inflicted. Morphological changes in the lungs were characterized by a phase-wise process, depended on the volume of injury, duration of hospitalization, medical care rendered, and manifested by disorders of blood content of the organ, development of tissue edema, and pneumonia. These data do not rule out the development of pneumonia in patients with spinal injuries.

[Hypophyseal damage in persons with cerebrocranial injuries and who died in the hospital].

The pituitary was examined in patients with craniocerebral injuries who died in hospital in various periods after treatment. Control group consisted of victims died at the site of accident. The results indicate the significance of examining the pituitary in craniocerebral injuries for the diagnosis of thanatogenesis, particularly in patients died in hospital. Causes of traumatic changes in the organ were determined, highly incident in the practice of forensic medical experts: directly during injury, as a result of skull bone fractures; during development of dislocation syndrome and in disorders of blood and lymph circulation in the brain matter; resultant from augmenting traumatic edema of the brain.

[Opiate poisoning (an analysis of section material)].

Section specimens from subjects dead from narcotic poisoning in 1998-1999 are analyzed. The results indicate that narcotic poisonings were primarily due to opiates or their combinations with other drugs (primarily ethanol). The ratio of male to female mortality was 3:1. Death after narcotic poisoning occurs at home in the majority of cases. The concentration of morphine/heroin in the blood and urine of victims varied within a very wide range. Acute renal failure, myoglobulinuremic nephrosis, and compression syndrome are not typical of acute narcotic poisoning. Use of immunotest for urinary morphine-heroin deserves further investigation.

[Prevalence and postmortem diagnosis of hemocontact infections in intravenous drug abusers based on forensic medical research data].

The spread of viral hepatitis C and B and of HIV was studied among the intravenous drug consumers (IDC) on the basis of 42 forensic-medical autopsies. The enzyme immune-assay (EIA) of blood serum showed, in 95.2% of cases, antibodies to hepatitis C virus, antibodies to HbsAg and HIV were registered in 11.9% in both cases. The possibilities of using the autopsy-blood serum for EIA-diagnosis of viral hepatitis and HIV were demonstrated. Morphological examinations of IDC showed, in them, chronic hepatitis (CH) of minimal, weakly-pronounced and moderate activities. Viral CH in HIV infected IDCs is characterized by a minimal and small-pronounced activity of the process and it is not different, according to its activity, from viral CH in IDCs without HIV.

[The forensic-medical expertise in craniofacial trauma].

Archive materials of the recent 6 years, including 14720 death cases of victims, were analyzed for the purpose of detecting the general regularities of injuries within the craniofacial trauma (CFT). The diversity of CFT with injuries to skull bones and medullary substance was found not to interfere with establishing an actual type and mechanism of trauma provided all injuries are thoroughly and comprehensively examined. The trauma specificity must be evaluated with respect to a location and direction of traumatic force in order to define an actual impact that caused death. CFT is special variation of craniocerebral trauma, whose onset mechanism needs more research. The efficiency of forensic-medical examination of CFT cadavers directly depends on the coordination and logic of measure undertaken by expert as well as on his technical outfit and knowledge of such trauma. The diversity of variations of damage to anatomic head structures requires an interdisciplinary approach with the need to isolate a predominant chain that affects the course and outcome of trauma.

[Forensic-medical aspects in assessment of damage to health as the result of chest and low back spinal traumas].

By the law of the Russian Federation, forensic medical examination is obligatory to assess damage to health in chest and low back spinal traumas. In persons who survived spinal trauma the assessment of its severity is regulated by the rules of assessment of the severity of bodily damage and harm to health. Difficulties arising in this assessment are outlined. Damage to health is proposed to be evaluated not only by clinical data on the condition of the patients but also by macro- and microscopic results of forensic-medical examination of the dead body, by the presence and severity of edema of the spinal cord tissue.

[A complex evaluation of the lytic potential of cellular cytotoxicity displayed by natural killers and K cells].

The method of testing the lytic potential of cell cytotoxicity, shown by whole-blood natural killer (NK) cells and K-cells, including the determination of the responsiveness of killers to interferon, is proposed. The method makes it possible to carry out mass survey of the population and to work with contaminated material. In this study the influence of different gene engineering preparations of interferon on the activity NK cells of humans and hamadryas monkeys is determined. The individual reaction of killers to their stimulation with interferon was confirmed. The study revealed that in volunteers immunized with yellow fever vaccine a decrease in the sensitivity of NK cells was observed.

[The activity of NK and K cells in mice vaccinated against plague].

The activity of NK and K cells in mice immunized against plague has been studied. This activity has been shown to depend on the dose of the vaccine and the time elapsed after immunization. The booster immunization of mice leads to an increase in the specific sensitization of lymphocytes, the antibody level and to an increase in the activity of killer cells. Changes in the cytotoxicity of NK cells on days 21-28 after primary and booster immunization are considered to occur not due to the action of Yersinia pestis themselves, but as the result of the immunological transformation induced by these microorganisms in the animals.

[The activity of NK and K cells in people vaccinated and revaccinated against plague].

Changes in the activity of NK and K cells in persons immunized against plague have been studied. A decrease in the activity of natural killer cells has been shown to occur. Booster immunization leads to a greater increase in the specific sensitization of lymphocytes, as well as in the antibody level. The observed increase of the activity of killer cells is regarded not as the result of the direct action of live Yersinia pestis cells, but as the result of the immunological changes in the body which they produce.

[The activity of natural killers and K cells and their interferon sensitivity in subjects inoculated against diphtheria and meningococcal infection].

The work deals with the time course of changes in the activity of natural killers, K-cells, their sensitivity to interferon, which reflects, in our opinion, the reserve possibilities of this killer system simultaneously with the development of the specific sensitization of lymphocytes. Significant changes in the cytotoxicity of natural killers, K-cells, have been detected in persons immunized against meningococcal infection, especially in those immunized with meningococcal vaccine introduced in combination with diphtheria toxoid. In this latter group of volunteers even more pronounced sensitization of peripheral blood lymphocytes has been observed than in persons immunized with monopreparations. The results obtained in this investigation indicate that the determination of these cell reactions may be of importance in the evaluation of the effectiveness of immunization.

[The cellular mechanisms of antianthrax immunity].

The influence of the vaccination of mice and volunteers on the activity of natural killer cells (NKC) and K cells, the effectors of natural and antibody-dependent cytotoxicity of cells, was studied. Both types of killer cells were found to take an active part in the vaccinal process during the whole term of observation. The changes revealed in this study were characterized by two phases. At the first contact with the antigen a rise in the activity of killer cells was observed; this rise was then followed by their pronounced suppression, reflecting total structural changes which occurred in the infected macroorganism. Repeated injections of the antigen induced mainly the activation of K cells. The use of protective antibodies contained in the commercial preparation of antianthrax globulin exerted influence on the duration of changes in the activity of NKC and K cells of the infected animals.
