Evidence that circadian variations of circulating melatonin levels in fetal and suckling rats are dependent on maternal melatonin transfer.

Although the circadian variation of melatonin content in the pineal gland appears during the 3rd week of extrauterine life, recent studies suggest that the fetus perceives the day length through maternal melatonin transfer. Accordingly, we determined serum melatonin concentrations in pregnant and lactating rats and in their offsprings during the day (D) and at night (N). As compared with nonpregnant adult female rats (D: 6.0 +/- 0.5 and N: 112.0 +/- 5.0 pg/ml), significant increases of serum melatonin concentrations, both during the day and at night, were observed in 21-day pregnant (D: 21.0 +/- 2.6 and N: 222.0 +/- 7.2 pg/ml) and in 10-day (D: 20.0 +/- 2.6 and N: 145.0 +/- 12.3 pg/ml) and 20-day (D: 19.0 +/- 1.2 and N: 140.0 +/- 8.0 pg/ml) lactating animals, while a profound decrease was found in 5-day (D: 13.0 +/- 1.0 and N: 50.0 +/- 5.6 pg/ml) lactating rats. In addition, melatonin levels were significantly higher during the night than during the day in all the experimental groups. High levels and circadian variation of serum melatonin were detected also in 21-day-old fetuses (D: 13.0 +/- 0.8 and N: 108.0 +/- 4.8 pg/ml) and in 5- and 10-day-old suckling (in 5-day-old, D: 13.0 +/- 0.6 and N: 71.0 +/- 3.1 pg/ml; in 10-day-old, D: 17.0 +/- 1.4 and N: 63.0 +/- 3.8 pg/ml) rats.(ABSTRACT TRUNCATED AT 250 WORDS)

Relation of lipid peroxides to macrovascular disease in type 2 diabetes.

Lipid peroxides are thought to be formed by free radicals and may play an important role in the development of atheromatous vascular disease. We have investigated the relationship between lipids, lipoproteins, coagulation factors, and lipid peroxides (measured as thiobarbituric acid reacting species (TBARS) in Type 2 diabetic patients with macrovascular disease. Eighteen diabetic and 20 non-diabetic subjects with clinical evidence of ischaemic heart disease and/or peripheral vascular disease were investigated, together with 28 healthy subjects without evidence of vascular disease. TBARS concentrations in non-diabetic (mean 5.0 (95% Cl 4.5-5.7) mumol l-1) and diabetic groups (5.6 (5.1-6.0) mumol l-1) with macrovascular disease were not significantly different although values were higher in both groups of patients with vascular disease by comparison with control subjects (2.7 (2.4-3.1) mumol l-1, p less than 0.001). Significant univariate correlations between TBARS concentrations and measures of blood glucose control (fructosamine, blood glucose and HbA1) were found for all 66 subjects (r = 0.35-0.42, p less than 0.01-p less than 0.001), although no independent association between these parameters and TBARS was demonstrated in multiple regression analysis.(ABSTRACT TRUNCATED AT 250 WORDS)

Insulin induces a similar reduction in the concentrations of its own receptor and of an insulin-sensitive glycosyl-phosphatidylinositol in isolated rat hepatocytes.

We have used isolated rat hepatocytes to study whether the insulin-induced reduction of its own receptors may modify the transduction of hormone signals by changes in the content of a glycosyl-phosphatidylinositol. Both subsequent insulin binding and glycosyl-phosphatidylinositol concentrations markedly decreased as a function of time and insulin concentration during preincubation of hepatocytes with insulin. The modifications observed in insulin binding were due to changes in receptor concentration. These results show that insulin regulates both the number of its own receptors and glycosyl-phosphatidylinositol concentrations in target cells, which may be of interest in many pathophysiological situations.

[Carcinoma of the colon in La Rioja. Study of incidence in the period 1978-87].

A study was made of the incidence of colonorectal tumors in the Autonomous Community of La Rioja (Spain) over 10-year period between 1978 and 1987, inclusive. An increased incidence of these tumors is confirmed, with no decline in the age of presentation or in the frequency of detection of early tumors, which suggests that this disease is on the rise. In view of these results, measures leading to the early diagnosis of colonic tumors are needed, such as screening of the general population, particularly groups at risk.

Differential precipitation of isolated human plasma lipoproteins with heparin and manganese chloride.

We studied the precipitation of isolated lipoproteins with heparin and MnCl2. Lipoproteins were isolated from human plasma by preparative ultracentrifugation and their free cholesterol was labeled. Each lipoprotein fraction was then precipitated at various pHs, with or without bovine serum albumin (60 g/L) present. Under no set of conditions was one class of lipoproteins completely separated from the other two. Specifically, under standard conditions for precipitation of serum lipoproteins (pH 7.4 and protein 60 g/L), 12% of the very-low-density lipoprotein (VLDL) and 8% of the low-density lipoprotein (LDL) remained in the supernatant liquid, and 30% of the high-density lipoprotein (HDL) was precipitated. These results indicate that, under these conditions, so-called HDL cholesterol may be a mixture of VLDL, LDL, and HDL, although the sum of the amount of these three fractions remaining in the supernate is fortuitously very close to the value for HDL cholesterol isolated by ultracentrifugation.

How do elderly veterans who fail to keep outpatient clinic appointments differ from those who do not.

The study asked how do elderly veterans who fail to keep appointments in the General Medicine and Surgery Out-Patient Clinics in the San Juan, VA Hospital differ from those who do not. Three hypothesis were formulated: 1. Elderly patients who fail to keep appointments have other medical alternatives in non-VA facilities. 2. Elderly patients who fail to keep appointments have a longer waiting time between appointments. 3. Elderly patients who are given appointments in various clinics within a short period of time fail to keep appointments. A sample of fifty-three elderly veterans from the Medical Out-Patient Clinics was drawn. A face-to-face structured interview was used to collect the data. The interview was designed to obtain socio-demographic characteristics, use of clinics, and recommendations to improve services. A percentage analysis was used to point out differences and similarities. The findings supported the hypothesis that patients who failed to keep appointments not only used private non-VA facilities, but also had the financial resources to pay for them. Data confirmed that those who failed to keep appointments have to wait a longer time between appointments. Patients who failed to keep appointments considered them too infrequent which resulted in forgetting the appointments.

[Adrenal function in topic corticotherapy].

It has been shown that corticosteroids for dermatological use produce an inhibition of the adrenal function, and it is considered that in our milieu these products are used without any precise indication. In this study, 29 ambulatory children from 3 to 15 years of age were selected, who were suffering from dermatoses with different degrees of extension, on whom urinary 17-ketosteroids, 17-ketogenicsteroids and serum cortisol were measured before and after non-occlusive treatment of two weeks, using 0.01 percent fluorocorticoids and controls after 24 hours and 8 days. A significant drop of the 17-keratogenicsteroids was found in patients studied at the end of the treatment, with immediate recovery. Additionally it was observed, in patients who had over 30 percent of skin area involved, that there was a decrease in serum cortisol, which persisted even 24 hours after discontinuing treatment, with normal levels in a measurement taken eight days later. It is therefore concluded that glucocorticoids applied topically are capable of inhibiting adrenal function.

17-Ketosteroids

[Scleroderma in children].

Fifteen children, ten with localized forms of scleroderma and five with progressive systemic sclerosis (PSS), were reviewed. Age of onset was 2 to 11 years. Mean follow-up was 3.4 years. Localized forms were: "en coup de sabre" in two cases, lineal scleroderma of extremities in five, morphoea in one, and combined lesions in two patients. None of them had systemic symptoms, although two had transient arthritis. Affected skin became atrophic in three cases, "En coup de sabre" lesions were removed surgically in two cases, and there were not recurrences. The other patients continued the same or worse. All five patients with PSS showed extracutaneous symptoms. Three of them with acrosclerosis, Raynaud's phenomenon and dysphagia. One patient died because of pulmonary fibrosis in less than a year. Disease remained unchanged in the other patients. The course of disease does not seems to be affected by treatment with several drugs (asiaticoside, colchicine, diphenyldantoin).

[Epidemiology of skin diseases in 10,000 patients of pediatric age].

Ten thousand new patients were seen between January 1971 and January 1975 at the Department of Pediatric Dermatology, "Hospital del Niño IMAN" in Mexico City. The patients were between 0 and 18 years old. They represented 10.4% of the entire pediatric population attending the general out-patient clinic of the hospital. The results of our study showed no significant difference among the sexes of patients. The highest proportion of patients in the sample were under one year of age. The most frequently observed groups of dermatoses were: parasitic, cutaneous reactions, viral and bacterial. The ten diseases most frequently observed were: papular urticaria (16.3%); atopic dermatitis (12.9%); scabies (10.4%); viral warts (8.4%); impetigo (6.8%); pitiriasis alba (6.6%); vitiligo (2.6%); acne (2.5%); dermatophytosis (2.4%) and numular eczema (2.3%). The 20 skin diseases most frequently recorded accounted for 85.8% of the sample. The ten most frequent skin diseases in each group of age were also recorded. The monthly and annual frequency for the more frequent dermatoses was studied. The present study provides epidemiological information for a rational development of programs on care, education and investigation in pediatric dermatology in Mexico.
