[ProVisc--a new viscoelastic material for surgery of the anterior segment. A multicenter clinical study].

Within the framework of a multicentric clinical study which was implemented at nine selected departments a new viscoelastic material ProVisc of Alcon company was tested during extracapsular extraction of cataract with implantation of posterior chamber intraocular lenses. A total of 100 eyes of 100 patients were operated and all pre- and postoperative data were recorded in uniform protocols. The selection of suitable patients and the type of surgical procedure was within the responsibility of the surgeon. The type and frequency of surgical and postoperative complications was similar as when other viscoelastic material ProVisc can be recommended as safe and effective in modern surgery of cataract.

[Problems with secondary implantations of intraocular lenses].

The authors focus their attention on the problem of secondary implantations of posterior chamber lenses. In a group of 20 patients they compare the peroperative and postoperative complications of secondary implantation into the sulcus ciliaris with and without fixation of the lens.

[Implantation of retropupillary intraocular lenses in post-traumatic conditions of the eye].

The investigation comprises the results of post-traumatic artephakias. In the course of 15 months the authors treated 15 patients with traumatic cataract after previous perforating or blunt eye injuries. In 12 patients after extracapsular extraction a retropupillary IOL was implanted. In three patients implantation was not performed. Retropupillary pseudophakia is the optimal correction of aphakia even in patients with previous serious eye injuries. A higher incidence of postoperative complications may be expected.

[Accuracy of ultrasound measurements of the axial length of the eye].

Ultrasonic assessment of the axial length of the eye provides important baseline data for the calculation of the dioptric power of an artificial intraocular lens. The accuracy of ultrasonic biometry performed by an OPHTASCAN B apparatus is discussed based on evaluation of a group of repeated measurements of the axial length and depth of the anterior chamber in 592 bulbi.

[Injuries of the orbit caused by foreign body].

Four cases of injury of the orbit by a big foreign body without damage to the bulbus are described. The bodies were extracted and the healing was without complications. Two of these bodies penetrated into the brain and in one case surgical intervention was necessary. In both instances vision was lost because of damage to the optic nerve.

[Combined use of ultrasound and radiodiagnostic methods in the diagnosis of posterior dislocation of the lens].

In a case record the authors present an account on a patient with an opaque media of the left eye on account of seclusion and occlusion of the pupil. The cause of the unilateral chronic uveitis was elucidated by ultrasonic examination with visualization type A. The cause was a calcified lens dislocated into the vitreous body and fixed to the posterior wall of the eye. The finding was confirmed by a negative X-ray picture and positive finding of a small foreign body with high density on CT. Dislocation of the lens occurred after a blow on the left eye during boxing 15 years before the patient sought medical assistance on account of a painful practically blind eyeball.

[Binocular vision after intraocular lens implantation].

The authors investigated the binocular function in a group of 127 patients after operation of cataract to whom an artificial intraocular lens was implanted. From the results ensues that adequate, or at least binocular vision satisfactory for human life, was restored in 82.74% members of the investigated group.

[Methods and results of intraocular lens implantation at the Eye Clinic in Brno].

The authors describe the technique of operation and the achieved results in a group of 148 patients to whom an intraocular lens was implanted. They achieved with a minimal number of complications in 96.3% practically normal vision. The author considers extracapsular extractions and implantation of lenses into the posterior chamber the future of the surgical approach to cataract in this country.

[Comparison of the predicted and actual refraction of the eye after intraocular lens implantation].

After previous assessment of the dioptric potency (by means of a SRK formula) the authors implanted the appropriate intraocular lenses. After 4-18 months following implantation the calculated refraction calculated before the intervention was compared with the postoperative reality. In 85% the difference was not greater than +/- 1.5 Dsf. The calculation of aniseiconia was made only in case of preoperative ametropia greater than +/- 2 Dsf.

[Reconstruction of the facial skeleton using Pallacos R].

Defects of the facial skeleton cause frequently severe aesthetic facial changes with negative repercussions also on the patient's mental state. The skeleton may be affected by injury, a neoplastic disease and iatrogenically when it is necessary to remove a part of the skeleton during operation. One possible way how to replace and reconstruct the skeleton is to use bone cement Pallacos R. This method was used in 8 patients with skeletal affections. The use of Pallacos R has the advantage that it can be used quickly, that it is easy to work with directly at the site of the defect and that its tolerance is satisfactory. It permits also to preserve the physiological shape of the paranasal sinuses. The method is one approach to individually indicated reconstruction.

[Complications of intraocular lens implantation].

The authors analyze the causes of per- and postoperative complications of implantations of intraocular retropupillary lenses. In a group of 132 extracapsular extractions of cataract with implantation of an intraocular lens no marked decentering was found. The most frequent postoperative complication were transitory striae and fibrin exudates in the pupil. Only once the "toxic lens syndrome" was encountered. The fine cilia of the posterior capsule, pigment dispersion and changes in the width of the pupil did not affect markedly the visual functions of the organ. The authors did not observe regenerating cataracts, or fibroses of the posterior capsule (mainly because of the short follow up period).

[Malignant melanoma of the conjunctiva].

The authors analyze six cases with malignant melanoma of the conjunctiva treated at the Ophthalmological Clinic in Brno in 1967-1988. They evaluate contemporary therapeutic possibilities and results of treatment. The prognosis of the condition is in general more favourable than in malignant melanomas of other mucous membranes.

[Delayed extraction of an intraocular foreign body].

The authors describe two cases of siderosis of the bulbus which developed as a consequence of late diagnosis of intraocular foreign bodies. The character of siderosis differed, however, in the two patients. In the first patient the picture of generalized siderosis confirmed also by electroretinographic examination permitted the assumption of an adverse course of the disease already on admission, as was confirmed by the progression of the disease even after removal of the intraocular foreign body. In the second patient probable encapsulation of the foreign body led to focal siderosis and a favourable course of the disease.

[Is the activation of growth of a pigmented lesion of the iris always a sign of malignancy? Case report].

A pigmented lesion of the iris followed up for five years in a 31-year-old female patient with marked progress during the past two years which led to block excision of the formation. Clinical signs (growth, vascularization, pigmentation and blood in the angle) as well as results of auxiliary examinations (ultrasound, fluorescein angiography) suggested a malignant lesion. However, histological examination provided evidence of the benign nature of the formation. Contemporary diagnostic non-invasive methods cannot decide definitely on the nature of a progressing neoplasm of the iris. The authors recommend long-term patient observation of these patients and surgery only in fully indicated cases.

[Extraction magnets in ophthalmology].

Magnetic extraction of foreign bodies has been used in the ophthalmologic practice more than one hundred years. The magnetic field of the extractive magnet causes the magnetization of the material present in the foreign intraocular body. Magnetization could be induced only in the ferromagnetic bodies and enables surgeons to perform the extractions. At this time the hand-manipulated and the based magnets are used, without adequate adaptability for the needs of now prevailing ocular microsurgery. Authors explain the use of the new generation of the extraction performed by means of the permanent magnet ORMEX which is based on Czechoslovak invention of the magnetic system ORMAKON characterized by significantly increased effectivity.

Eye Foreign Bodies

[Permanent Ormex magnets in clinical practice].

Permanent magnets ORMEX have been successfully used in 14 extractions of the foreign intraocular nodies localized in the anterior and posterior segments of the eye. These magnets proved to be more advantageous than the usually used electromagnets, with respect to their size, weight, easier manipulation and usual mode of sterilization. The ORMEX magnets do not need source of the electric energy and their energetic performance is greater than of the routinely used magnets of the Hirschberg's type. Authors recommend to equip all ophthalmic stationaries with these permanent ORMEX magnets.

[Work capacity evaluation in patients after surgery for detached retina].

The authors investigated the enlistment into work in patients after operation of detached retina at the Ophthalmological Clinic in Brno in 1985. From the results of the work it is apparent that the risk of relapse in conjunction with unsuitable work after operation is practically nil. Socio-economic aspects of patients who before operation suffered from some predisposing eye disease are more affected. The authors recommend systematic screening and dispensary care, in particular during the period when these people decide on their future occupation, i. e. already in primary school.

[Central serous retinopathy as the first manifestation of malignant melanoma of the choroid].

The authors give an account of 12 patients where the original ophthalmological finding corresponded to central serous retinitis. In the course of check-up examinations the picture changed and finally the complex of applied examinations revealed that a malignant melanoma of the choroid was involved. The authors mention the time needed for the clinical decision on the final diagnosis of the disease which was confirmed histologically after enucleation of the eye. In the conclusion the authors draw attention to the necessity of frequent check-up examinations after the initial establishment of the diagnosis of central serous retinitis with regard to the differential diagnostic possibilities of the above disease.
