[Forum on tissue expansion. Value of cutaneous expansion for the covering of synthetic materials used in cranioplasty. Apropos of a case].

The authors present a case of cover of synthetic material by expanded skin. In a context of post-sepsis revision, this cover proved to be of excellent quality, superior to normal scalp. The specific characteristics of tissue expansion may possible explain, in this context, the superiority of the expanded skin. The authors intend to adopt a similar therapeutic approach is such a situation should arise in the future.

[Giant juvenile fibroadenoma in an adolescent. A case report].

Fibroadenomas occupy the first place, in terms of prevalence, among benign breast disorders in adolescent girls. Juvenile giant fibroadenoma is a special though rare form, which must be recognised in terms of differential diagnosis from virginal hyperplasia in its asymmetrical early form and phylloid tumours, the prognosis of which is different. The authors describe the clinical and pathological features necessary for diagnosis and report the results of breast reconstruction after excision via a lateral radial approach in a 14-year-old adolescent girl.

[Treatment of incisional hernia of the abdominal wall by the skin lacing technique. Apropos of 30 cases].

The treatment of large incisional hernias is often difficult, as reflected by the many surgical techniques proposed and the high recurrence rate. Since 1983, the authors have used a skin lacing procedure in these large hernias which frequently tend to recur. This report was based on the study of 30 consecutively patients operated by this method between 1983 and 1989. The objective results evaluated clinically in terms of the quality of the abdominal wall after more than two years of follow-up appear to argue in favour of this little known technique which combines simplicity, rapidity and reliability. Based on their experience, the authors believe that this method has a place in the therapeutic arsenal for abdominal wall repair as, in some cases, it can resolve the difficult problem of treatment of large, recurrent incisional hernias regardless of the patient's age.

[Iliac eventration after posterior iliac bone removal. An original technique of reconstruction].

A tricortical bone graft removal from the iliac crest causes a parietal defect that may be complicated by iliac or iliolumbar eventration, according to the extent of the bone loss. On the basis of their experience with a particularly characteristic clinical case, the authors describe an original reconstruction technique. The surgical techniques aimed at parietal restoration described in the literature include either the use of foreign materials or of those provided by the patient (skin, fascia lata, etc.), or the use of the surrounding anatomical elements to reconstruct a solid wall. Most of these procedures may be difficult to implement if the loss of substance is extensive, or in a posterior site. Thus, in order to prevent possible recurrence in case of major bone defects, we propose to combine the cure of the eventration with a restoration of bone continuity with a cold-conserved allograft and the reconstruction of the musculoaponeurotic curtain by the translation of the lumbosacral fascia. This procedure allowed obtaining a very satisfactory result, which was stable after 18 months both as regards parietal reconstruction and as regards pain, owing to the restoration of balance in the paraspinal muscular support.

[Therapeutic attitude in extensive cutaneous lesions in Recklinghausen's disease].

Based on our experience with von Recklinghausen's disease, we present two cases characterized by highly developed skin tumors. One of these cases is unique in the literature. After describing these two cases, we review the different sites of the disease and discuss the limitations of surgery. Particular attention is paid to surgical palliation for patients with end-stage tumors.

Polyamines in human breast cancer and its relations to classical prognostic features: clinical implications.

Experimental evidence suggest an important role of polyamines in breast cancer development. Polyamines have been determined in tissue and erythrocyte samples from 100 patients with primary invasive breast cancer and 30 patients with fibroadenomas. Statistical analysis was performed in order to determine the prognostic value of the polyamine patterns of tumor tissues and erythrocytes in comparison with clinical and histological prognostic factors. In malignant tissues, polyamine levels were significantly higher than in benign tissues. They correlated with markers of tumor aggressivity (axillary node involvement and especially with markers of high mitotic rate as Ki-67 staining, histological grade). No correlation was found between estrogen and progesterone status, tumor size and polyamine concentrations. Erythrocyte polyamines levels were identical between cancer patients and controls. The knowledge of the polyamine pattern in breast cancer could become useful in clinical practice particularly if polyamine metabolism is targeted as a therapeutic approach.
