A new method for locating anterior skeletal landmarks from soft tissue measurements.

Clinical examination of human profiles provides some idea of the localization of hard tissue landmarks, but is unreliable because of large variability in thickness of the soft tissues. In this investigation an instrument was developed to measure the thickness of the lips. Linear regression was applied to estimate cephalometric hard tissue landmarks from those in the soft tissues. By adding information from direct clinical measurements to the model for estimating hard tissue landmarks from soft tissue landmarks, the correlation between the digitized (actual) and estimated or calculated landmarks was significantly improved.

Polymerase chain reaction (PCR) in various stages of HIV infection. Relationship to disease progression.

In order to establish whether the Polymerase Chain Reaction (PCR) may constitute a new marker of evolution towards AIDS in symptomless HIV infected subjects, we used PCR with three primer pairs (in the gag, pol and LTR regions) in 223 seropositive individuals at different stages of HIV infection. Among 176 symptomless seropositive individuals, 174 (98.8%) were positive with at least one primer pair. The subjects negative with at least one primer pair had a CD4 lymphocyte count significantly higher (p less than 0.01), and serum immunoglobulin G, immunoglobulin A, neopterin and beta-2-microglobulin concentrations significantly lower (p less than 0.01) than the individuals positive with the three primer pairs. Among 73 seropositive individuals followed over a two year period, 59 presented the same PCR pattern over this time period, while PCR showed different results in 14. Forty-seven AIDS patients were positive with the three primer pairs. The number of PCR negative with at least one primer pair was significantly fewer (p less than 0.001) in symptomatic individuals than in symptomless individuals. We conclude that the percentage of positive PCR results in HIV infected individuals is linked to the clinical stage of infection and to the disease progression.

Pharmacokinetics of 5-fluorouracil assessed with a sensitive mass spectrometric method in patients on a dose escalation schedule.

The pharmacokinetics of 5-fluorouracil (5-FUra) was investigated in 21 patients with advanced cancer (mainly colorectal cancer). 5-FUra was administered as weekly i.v. bolus injections usually at a starting dose of 500 mg/m2. Every 4 weeks the dose was escalated by 20% until dose-limiting toxicity was observed. Whenever possible, pharmacokinetic studies were performed at dose escalation. 5-FUra plasma concentrations were measured by high pressure liquid chromatography and a sensitive gas chromatography-mass spectrometry assay with a sensitivity as low as 3 x 10(-9) M. According to the 42 plasma concentration versus time curves, in all but one of the patients total body clearance decreased with increasing 5-FUra doses, consistent with the nonlinear pharmacokinetics of 5-FUra. The ultrasensitive assay revealed an almost horizontal phase in the plasma concentration versus time curves at plasma concentrations of 10(-8) to 10(-9) M. This plateau did not show correlation with the area under those curves. The use of a logistic regression method showed that clinical toxicity was correlated with the area under the plasma concentration versus time curve of 5-FUra.

Timolol and aceclidine. A clinical study of interaction in the eye.

In the Netherlands timolol maleate, probably the drug of first choice in antiglaucoma therapy, is often combined with other intraocular pressure lowering drugs. The efficacy of most combinations have been discussed in the literature several times, except the one with aceclidine, a parasympathetic drug, that has a reasonable popularity in Western Europe. Unlike pilocarpine, aceclidine does not seem to give a clinically significant additional pressure lowering effect in timolol pre-treated eyes. The decrease in IOP in aceclidine pre-treated eyes after single dose timolol seems to be an effect of timolol alone. Our results suggest that co-administration of timolol and aceclidine is not a rational action in antiglaucomatous therapy.

Crown-rump length measurements revisited.

UNLABELLED: Clinical and ultrasound data of 403 first-trimester pregnancies were collected prospectively. For our CRL study, two groups of patients were selected: Group A with an optimal (menstrual) history (111 patients, 156 CRL measurements) and Group B with available BBT charts (17 patients, 27 CRL measurements). Compared to the Robinson CRL curve (SD +/- 4.7 days) the curve of group A (SD +/- 4.4 days) showed a small underestimation of gestational age of 2.5 days in the 8th week, gradually declining (p less than 0.001). The CRL curve of group B (SD 4.3 days) is parallel to the group A curve, displaced 2.9 days to the left, and did not differ significantly from the Robinson curve. IN CONCLUSION: in contrast to suggestions in literature there is no need to revise the original CRL curve of Robinson. For individual variations in CRL measurements not only measure errors but also variations in the time of implantation and/or start of growth are responsible.

Stochastic events in the amplification of HTLV-I integration sites by linker-mediated PCR.

Human T-cell leukaemia virus type I (HTLV-I) proviral integration sites from an asymptomatic carrier and from the MT4 cell line were analysed by linker-mediated PCR (LMPCR) and inverse PCR (IPCR). LMPCR was more sensitive, allowing detection of a greater number of integrated proviruses. Reconstruction experiments using a cloned integrated HTLV-1 provirus indicated that > 100 copies were necessary to be detected frequently by LMPCR. To circumvent this problem, the LMPCR analysis was performed approximately 20 times per sample. Thus, for the MT4 cell line, the seven major integration sites were accompanied by approximately 20 clones of lesser frequency. For an asymptomatic HTLV-I carrier, nine integration sites were identified in a single amplfication, while a further 9 followed from 14 additional reactions. These findings show that there is a stochastic element to sampling HTLV-I integration sites by LMPCR, which tends to underestimate the actual number of HTLV-I bearing clones. Accordingly, those detected in at least two reactions represent the most abundant clones.

[Assessment of P glycoprotein expression by immunocytochemistry and flow cytometry coupled with functional efflux analysis: application to acute myeloid leukemia].

UNLABELLED: P glycoprotein (Pgp) expression is associated with failure of anticancer chemotherapy in acute myeloid leukemia (AML). However, a consensus has been difficult to reach, due to the variable results obtained by different methods. Samples of 27 patients with AML were studied here according to international recommendations (Beck, et al. , Cancer Research 1996; 56: 3010-20). Pgp expression was performed by immunocytochemistry (ICC) using the avidin-biotin peroxidase technique with JSB1 and UIC2 monoclonal antibodies. Flow cytometry (FCM) analysis of Pgp was investigated using UIC2 in an indirect immunofluorescent assay. UIC2 staining was measured by the Kolmogorov-Smirnov statistical test and fluorescence intensity ratio. Finally, the rhodamine 123 test (Rh 123) with or without verapamil was performed to detect functional activity. RESULTS: by ICC, results of JSB1 and UIC2 were consistent in 94% of the cases. In 74% of the cases, concordant conclusions were observed by ICC and FCM. Overall, Pgp expression was detected in 67% of the cases (ICC/JSB1+ and ICC/UIC2+ or FCM/UIC2+). Functional activity of Pgp was shown in 59% of the patients. Rh 123 efflux was correlated with Pgp expression in 70% of the 27 studied cases but 3 cases were Pgp-/Rh 123+ and 5 Pgp+/Rh 123-. In conclusion, assessment of Pgp expression by ICC and FCM using two different monoclonal antibodies coupled with functional efflux test is required to identify discordant expression/function cases suggesting a non functional Pgp or another alteration of drug transport.
