[Results of clinical studies on drug modification of progressive lens opacities].

This paper begins with a discussion and explanation of the special features and requirements of a controlled clinical trial of the efficacy of anticataract preparations. So far, such trials have been carried out with Phakan and two types of eye drops (preparations "A" and "B") in accordance with the conditions for controlled therapy studies. The procedure is explained taking the Phakan study as an example. Lens findings were evaluated by densitometric image analysis of Scheimpflug photographs of the anterior eye segment, made at different times during the trial. The validity of the data for statistical calculations was at first overrated. Mean values obtained with the method, however, correspond to the values expected: they show the required progression over longer periods. The reliability of the method was checked by taking into account the clinical observations and visual acuity tests on 35 patients (62 eyes) which had been under observation for a considerable period of time (up to 5.5 years). The results indicate clearly that Phakan can favorably influence opacities located in the lens cortex and that after a 9-month treatment even a prolonged effect (2-3 years) may be observed. With preparation "A" some indication - although not completely satisfactory - of efficacy in cases of cortical opacities was found. A one-year treatment with preparation "B" produced no differences as compared to untreated controls. However, it appears advisable to extend observation periods in future trials.

[Biometry of the lens with respect to age and cataract morphology. Evaluation of Scheimpflug photos of the anterior segment].

Thickness of the lens, cortex and nucleus or nucleus with supranuclear layer, respectively, were determined for 58 controls aged 11-69 years (115 lenses), and for 175 patients aged 30-86 years with different types of cataract (247 lenses). Measurements of the densitograms of Scheimpflug photos (taken along the optical axis, magnified 10-fold) were performed, accurate to 0.5 mm (corresponding to 0.05 mm original size). The cataractous lenses were grouped in 5 morphological classes and 3 degrees of opacification (density according to the Scheimpflug photos). As regards normal lenses, the present authors' findings correspond to earlier findings of Niesel et al. (1976). Deviations from the normal pattern are found in typical cortical cataract (water-cleft and spokes cataract, wedge-shaped cataract) where nucleus and nucleus with supranuclear layer are thinner than the normal lenses, while with nuclear cataracts and mixed types the opposite is the case. In eyes with posterior cortical opacities no increase in the thickness of the anterior cortex is found with aging. With the true nuclear cataracts the increase in the thickness of the anterior cortex is negligible. The degree of opacification is closely related to an increase in the thickness of the nucleus and supranuclear layer, which implies a decrease in the thickness of the anterior cortex. Water-clefts and spokes of larger size cause the anterior cortex to become thinner. In eyes with wedge-shaped cataracts the different degrees of opacification did not affect the parameters measured.

Biometry of the anterior eye segment by Scheimpflug photography.

Scheimpflug photos of 262 cataract patients, classified according to various forms of opacifications characteristic for senile cataracts were evaluated by biometric methods. The measured values on cornea thickness, depth of anterior chamber, lens thickness, and radii of curvatures of cornea and anterior lens surface show that a correlation between some of the parameters and certain forms of opacities must be assumed.

Functional ophthalmodynamometry. Comparison between brachial and ophthalmic blood pressure in sitting and supine position.

Ophthalmodynamometry was done by 96 normotensive volunteers and 62 hypertensive patients in sitting and supine position. A high correlation between the brachial and ophthalmic blood pressure was found in both groups, and in both positions. The brachial blood pressure of normotensive subjects did not show any difference in both positions. Although the systolic brachial blood pressure did not show significant changes in hypertension, the diastolic--and the calculated mean brachial blood pressure has been slightly but significantly decreased in the supine position. In both groups, normotension and hypertension, the ophthalmic blood pressure was found to be dependent from the level of the systemic blood pressure and the position of patient. It is significantly higher in the supine position than in the sitting position. The higher the systemic blood pressure in the sitting position, the less was the increase of the ophthalmic blood pressure in the supine position.

We recently had the opportunity of evaluating the technique of computer analysis of fundus photography and fluorescein angiograms. Two angiograms were obtained over a period of three months from a patient with senile macular degeneration. At this time we are applying two techniques: black and white photographs with differential contrast enhancement, as well as photographs made with false colours, with and without contrast enhancement. Both of these techniques enables us to confirm the presence of chorio-retinal anastomoses at the site of the macular degeneration. We assume that this was caused by two occlusions of a choroidal venule.

Clinical and statistical aspects on standardization of tonometers.

Using the applanation tonometry of Goldmann (mechanical-optical applanation principle with constant diameter of the applanation circle) as a reference system, a test plan inclusive of the necessary conditions for the clinical control measurements of new tonometer types and guiding values for judging the usefulness of these new developments are described. For calculation of the statistical values a computer program in BASIC system is specified.

[First experiences with an instrument for TV display of digitally encoded text information for the visually handicapped (author's transl)].

An instrument was developed which allows the display, in enlarged form--especially for visually handicapped persons--of text information as used in modern data-acquisition systems such as writing automats, screen editing devices, and television text display (in England, Ceefax). With the aid of electronic character generators, the text is displayed continually, moving from right to left in one line on the screen of a commercial TV set. Rate of movement, size, brightness, and color of the characters (and/or the background) can be selected arbitrarily. We performed a preliminary reading test with visually handicapped subjects from a rehabilitation center for blind persons. The resulting optimal parameters for text display and the achieved reading speed with the system are discussed.
