Cytokeratin 7 and cytokeratin 20 expression in epithelial neoplasms: a survey of 435 cases.

Cytokeratin 7 (CK 7) and cytokeratin 20 (CK 20) are low molecular weight cytokeratins. Their anatomic distribution is generally restricted to epithelia and their neoplasms. We surveyed 435 epithelial neoplasms from various organ systems by immunohistochemistry using CK 7 and CK 20 monoclonal antibodies. Expression of CK 7 was seen in the majority of cases of carcinoma, with the exception of those carcinomas arising from the colon, prostate, kidney, and thymus; carcinoid tumors of the lung and gastrointestinal tract origin; and Merkel cell tumor of the skin. The majority of cases of squamous cell carcinoma of various origins were negative for CK 7, except cervical squamous cell carcinoma, in which 87% of cases were positive. Approximately two thirds of cases of malignant mesothelioma were CK 7-positive. CK 20 positivity was seen in virtually all cases of colorectal carcinomas and Merkel cell tumors. CK 20-positive staining was also observed in cases of pancreatic carcinomas (62%), gastric carcinoma (50%), cholangiocarcinomas (43%), and transitional cell carcinomas (29%). The expression of CK 20 was virtually absent in carcinomas from other organ systems and in malignant mesothelioma. CK 7- and CK 20-negative epithelial neoplasms included adrenal cortical carcinoma, germ cell tumor, prostate carcinoma, renal cell carcinoma, and hepatocellular carcinoma.

Simultaneous quantitative determination of norgestrel and progesterone in human serum by high-performance liquid chromatography-tandem mass spectrometry with atmospheric pressure chemical ionization.

A selective, reliable and rapid method for the simultaneous determination of progesterone and norgestrel concentrations in human serum after taking oral contraceptive tablet has been developed using high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS-MS) with atmospheric pressure chemical ionization (APCI) interface. The norethisterone was used as the internal standard. Selected transitions of m/z 313/245 for norgestrel, m/z 315/97 for progesterone and m/z 299/109 for norethisterone were monitored using multiple reaction monitoring (MRM) mode for quantitation. The assay was linear over the concentration range of 0.2-50 ng mL-1 for norgestrel and progesterone. The lower level of quantitation in human serum was obtained at 0.2 ng mL-1 for both norgestrel and progesterone using optimum tuning parameters. The intra-assay precision and inter-assay precision do not exceed 10 and 9%, respectively. The method has been applied to the determination of norgestrel and progesterone in serum of female volunteers.

In vivo determination of body composition of rats using magnetic resonance imaging.

Magnetic resonance imaging (MRI) has potential as an instrument to measure body composition because it can discriminate various soft tissues in vivo. These soft tissues include adipose tissue, muscle, organs, and brain. We report on preliminary studies using a 4.2-tesla MRI for measuring body composition in the mouse and rat. We employed image segmentation methods that include an image correction method, a necessary requirement when the images are taken in the presence of nonuniform radio-frequency (RF) coil response. The software for 3-D data segmentation, quantification, correction, image manipulation, and visualization has been developed as a research tool. This method currently is being validated.

Primary noncutaneous melanoma.

Primary noncutaneous melanomas are rare neoplasms that affect patients in an older age group than primary cutaneous melanomas. The prognosis is worse than primary cutaneous melanomas because of the advanced stage at the time of diagnosis, the rich vascular and lymphatic supply of mucosal sites, and the lack of clinical suspicion of the tumor because of its rarity. The initial treatment is surgical resection but the location may make it technically difficult to obtain complete tumor removal. Unlike cutaneous melanoma, sun exposure is not a risk factor for noncutaneous melanomas. Darker-skinned individuals may have a higher incidence of some noncutaneous melanomas, such as anorectal melanomas. It is important for clinicians and pathologists to recognize primary noncutaneous melanomas to provide early detection and optimum management.

Role of synaptic vesicle proton gradient and protein phosphorylation on ATP-mediated activation of membrane-associated brain glutamate decarboxylase.

Previously, we have shown that the soluble form of brain glutamic acid decarboxylase (GAD) is inhibited by ATP through protein phosphorylation and is activated by calcineurin-mediated protein dephosphorylation (Bao, J., Cheung, W. Y., and Wu, J. Y. (1995) J. Biol. Chem. 270, 6464-6467). Here we report that the membrane-associated form of GAD (MGAD) is greatly activated by ATP, whereas adenosine 5'-[beta,gamma-imido]triphosphate (AMP-PNP), a non-hydrolyzable ATP analog, has no effect on MGAD activity. ATP activation of MGAD is abolished by conditions that disrupt the proton gradient of synaptic vesicles, e.g. the presence of vesicular proton pump inhibitor, bafilomycin A1, the protonophore carbonyl cyanide m-chorophenylhydrazone or the ionophore gramicidin, indicating that the synaptic vesicle proton gradient is essential in ATP activation of MGAD. Furthermore, direct incorporation of (32)P from [gamma-(32)P]ATP into MGAD has been demonstrated. In addition, MGAD (presumably GAD65, since it is recognized by specific monoclonal antibody, GAD6, as well as specific anti-GAD65) has been reported to be associated with synaptic vesicles. Based on these results, a model linking gamma-aminobutyric acid (GABA) synthesis by MGAD to GABA packaging into synaptic vesicles by proton gradient-mediated GABA transport is presented. Activation of MGAD by phosphorylation appears to be mediated by a vesicular protein kinase that is controlled by the vesicular proton gradient.

Reduction in hMSH2 mRNA levels by premature translation termination: implications for mutation screening in hereditary nonpolyposis colorectal cancer.

We have used single-strand conformational polymorphism (SSCP) and heteroduplex analysis to examine DNA from 50 colorectal carcinoma patients coming from families meeting the Amsterdam criteria for hereditary nonpolyposis colorectal cancer (HNPCC) or having developed colorectal carcinoma at age 45 or younger. We identified mutations in 12 of these patients, with seven of these being novel mutations. We examined four of the truncating mutations using in vitro transcription and translation (IVTT) assays and determined that the mutation causing an in-frame deletion of exon 5 could easily be detected by the IVTT assay, but the three mutations resulting in premature translation termination were not detected because the steady-state levels of the mutant allele transcripts are too low. Our findings suggest that some but not all mutant hMSH2 alleles have significantly lower steady-state mRNA levels than the normal allele. Under ideal circumstances, where lymphoblastoid cell lines are available for RNA extraction, IVTT may be useful for detecting truncating mutations. However, our data suggest that caution should be taken in using IVTT in routine screening of clinical samples for truncating HNPCC mutations, as many mutations may go undetected.

Molecular-based approach to the differentiation of mealybug (Hemiptera: Pseudococcidae) species.

The rDNA internal transcribed spacer (ITS) region of 4 mealybug species, Pseudococcus viburni (Signoret), P. longispinus (Targiono-Tozzetti), P. calceolariae (Maskell), and P. similans (Lidgett), was isolated by polymerase chain reaction (PCR) amplification, cloned, and sequenced. In this region of the genome there were numerous differences, including nucleotide substitutions, insertions, or deletions between P. viburni, P. longispinus, and P. calceolariae, whereas P. calceolariae and P. similans were very similar. Based on sequence differences between the ITS regions, we designed PCR primers that were able to differentiate the 4 mealybug species and that correlated with morphological differences found between adult females of these species. The PCR amplification by using the species-specific primers enabled the differentiation of not only adult females but also eggs, juveniles, and adult males, which was not previously possible by using conventional identification methods.

Safety and efficacy of metformin in a restricted formulary.

OBJECTIVE: To assess the efficacy and safety of metformin (MET) in the Veterans' Administration Northern California Health Care System during the period from June 1995 through April 1996 when its use required approval by Endocrinology. STUDY DESIGN: A retrospective review of patient charts and computerized pharmacy and laboratory records. Patients served as their own historical controls. PATIENTS AND METHODS: Patients receiving MET (n = 251) were identified from the pharmacy database. On-line laboratory data, including the intermediate outcome variable HbA1c, were retrieved by computer for the interval 4 months prior to the initial prescription to May 1996. Clinical data including weight and blood pressure were obtained from chart review. RESULTS: Of 228 patients whose charts were available for review, 29 reported side effects, and 12 discontinued use due to these side effects. No patients were identified with lactic acidosis. Both baseline and treatment data on HbA1c were available on 164 patients. Mean HbA1c (%) data (unpaired), expressed as mean +/- SE, were as follows: between 4 months pretreatment and 1 month pretreatment, 9.41 +/- 0.19 (n = 103 tests); between 1 month pretreatment and baseline, 9.41 +/- 0.19 (n = 110 tests); 3 months of treatment, 8.79 +/- 0.16 (n = 157 tests, P < 0.05); 6 months of treatment, 8.30 +/- 0.17 (n = 79 tests, P < 0.0001); 9 months of treatment, 8.72 +/- 0.24 (n = 70 tests, P < 0.05), compared to pretreatment values. Similar analysis of unpaired weight and blood pressure data in 152 patients did not reveal any reduction in these clinical parameters over this treatment period. Serum lipids were unchanged on treatment (by paired analysis), but the number of tests was limited. CONCLUSION: In this setting, MET provided sustained beneficial effects on glycemic control and was well tolerated. Any effects on weight, blood pressure, and serum lipids were not demonstrable in this analysis. We conclude that MET can substantially improve outcome of diabetes care.

Immobilized lipoxygenase in a packed-bed column bioreactor: continuous oxygenation of linoleic acid.

The continuous oxygenation of linoleic acid (LA) by immobilized lipoxygenase (LOX) was studied. Enzymatic oxidation was carried out in a recirculating packed column reactor using immobilized LOX as the stationary phase and LA as the substrate. The column, when packed with LOX immobilized in either a calcium alginate sol-gel matrix or a phyllosilicate sol-gel matrix, is equivalent to five continuous stirred tank reactors (CSTRs). The reactor cascade was calculated from the residence-time distribution for the reactor. Based on mass-balance calculations, a set of mathematical equations for predicting the concentration of oxygenated product generated in each CSTR was calculated. Product formation in the packed column reactor was simulated and results calculated with the model were compared with the experimental results. The data indicated that product yield (hydroperoxyoctadecadienoic acid, HPOD) increased asymptotically with reaction time. Experimentally, when the bioreactor was packed with calcium alginate sol-gel-immobilized LOX, an initial linear increase in HPOD production with time was observed, but reached a steady state. For the bioreactor packed with phyllosilicate sol-gel-immobilized LOX, initial HPOD production increased more rapidly but reached a lower steady-state concentration. From these data, a simple computer simulation model was developed to determine the process kinetics of this reactor design.

The metallo-disintegrin ADAM10 (MADM) from bovine kidney has type IV collagenase activity in vitro.

The metallo-disintegrins (ADAMs) are a family of mammalian proteins with significant amino acid sequence identity and a domain organisation similar to the snake venom metalloproteinases (reprolysins). They have been implicated in a wide variety of processes such as cell-cell and cell matrix adhesion and proteolysis of the extracellular matrix in a wide variety of cell types. They may also be involved in events such as the processing of plasma membrane proteins, proteolysis in the secretory pathway and pro-cytokine conversion processes. Due to the close relationship of the ADAM proteins with snake venom enzymes which have been demonstrated to be type IV collagenases, we investigated whether purified bovine ADAM10 could cleave basement membrane type IV collagen. We show here that ADAM10 purified from bovine kidney can cleave a basement membrane collagen type IV preparation as assessed by SDS-PAGE analysis and novel epitope recognition with a specific antibody to type IV collagen. The demonstration that a metallo-disintegrin displays a type IV collagenase activity may be relevant to tumour metastasis and may have general relevance to extracellular re-modeling in renal pathology and a variety of other pathological states where compromise of the basement membrane is involved.

Genetic ablation of flowers in transgenic Arabidopsis.

We have created transgenic Arabidopsis plants in which a gene encoding the cell-autonomous diphtheria toxin A chain (DT-A) was expressed under the control of the LEAFY (LFY) promoter. This promoter is active both in emerging leaf primordia and young flowers, with the highest activity in flowers. The majority of LFY::DT-A plants had normal vegetative development but lacked flowers, demonstrating that relatively widespread activity of a promoter does not exclude its possible use for ablating selected tissues, as long as differences in activity levels between different tissues are significant. We also found that flowers were replaced by empty bracts in LFY::DT-A plants, suggesting that flower-derived signals normally suppress bract development in Arabidopsis.

Epidemiology of invasive pneumococcal infections in infants and young children in Metropolitan Santiago, Chile, a newly industrializing country.

AIM: To study the epidemiology of invasive pneumococcal infections in infants and young children in Santiago, Chile, as a representative pediatric population in a newly industrializing country where pneumococcal conjugate vaccines may be used in the future. METHODS: A 5-year retrospective laboratory-based review (1989 to 1993) was followed by a 3-year prospective laboratory and hospital surveillance study in two of the six health administrative areas of Santiago to detect all hospitalized cases of invasive pneumococcal disease (defined as Streptococcus pneumoniae isolated from blood, cerebrospinal fluid or another normally sterile site) among infants and children (0 to 23 months of age in the retrospective and 0 to 59 months of age in the prospective study). RESULTS: During the 5-year retrospective survey the incidence of invasive pneumococcal disease was 90.6 cases per 10(5) infants 0 to 11 months old and 18.5 cases per 10(5) toddlers 12 to 23 months old. Similar rates (60.2 per 10(5) infants and 18.1 per 10(5) toddlers) were recorded during the 3 years of prospective surveillance. Among the 110 cases in children 0 to 59 months of age detected during the 3-year prospective surveillance, 2 clinical forms, pneumonia and meningitis, accounted for 87.2% of all cases; 13 of the 49 pneumonia patients (26%) had empyema as a complication. Notably 40 of the 110 cases (36.4%) occurred before 6 months of age (63.4% of the 63 infant cases). Serotypes 1, 14, 5 and 6B were the most prevalent. Overall 76 and 69%, respectively, of S. pneumoniae isolates were antigenic types that would be covered by the 11- or 9-valent conjugate vaccines under development. CONCLUSIONS: Invasive pneumococcal infections in Santiago, Chile, exhibit an epidemiologic pattern intermediate between that of developing and industrialized countries. The high burden of disease in early infancy dictates that an accelerated immunization schedule (beginning in the perinatal period) or maternal immunization with pneumococcal vaccines should be explored.

Experimental model of renovascular hypertension.

OBJECTIVE: To establish a model of renovascular hypertension. METHODS: A 4/0 resorbable chromic catgut ligature was used to ligate subtotally the renal arteries of 18 dogs, forming experimental renovascular hypertension steadily. Blood pressure, plasma renin activity, the ultrastructural changes of juxtaglomerular apparatus and renal artery wall were studied after the constriction. RESULTS: It was reasonable that renal blood flow measured with an electromagnetic flowmeter was reduced by 30% after the constriction. The pathological changes of the induced renal artery stenosis were similar to those of fibromuscular dysphasia. CONCLUSION: The findings provide valuable evidence for the treatment of renovascular hypertension.

[A study on the activity of nitric oxide in alveolar macrophages from patients with lung cancer].

Nitrite and nitrate (NO2-/NO2-) in the bronchus alveolar lavage fluid (BALF) and the supernatants of incubated alveolar macrophages (AMs) from patients with primary lung cancer were measured by copper-coated cadmium reduction and Griess method. Mrna expression of AM induced nitric oxide synthase (iNOS) were analyzed by RT-PCR. There was NO2-/NO2- in BALF either from lung cancer patients or from control subjects. When compared with control group and the nontumor-bearing lung, the level of NO2-/NO2-was lower in BALF from the tumor-bearing lung [5.18+/-1.1 vs 2.47+/-0.67nmol x mg protein-1 (P< 0.01); 4.65+/- 2.46 vs 2.47+/- 0.67nmol x mg protein-1(P< 0.01)]. We also found a lower level of NO2-/NO2- in the supernatants of incubated AMs from the lung of cancer patients than from control and nontumor-bearing lung [95.03+/- 21.76 vs 63.37+/- 17.58nmol (P< 0.01); 85.61+/- 16.70 vs 63.37+/- 17.58nmol (P< 0.05)]. No significant difference existed between the MRNA expression of AM iNOS in lung cancer patients (69%) and that of control subjects (91%). After the AMs were stimulated with granulocyte-macrophage colony stimulating factor (GM-CSF), the level of NO2-/NO2- in the supernatants was significantly increased (P< 0.01); while the mRNA expression of AM iNOS from patients with lung cancer resulted in an increase of 16.85+/- 7.58% vs 33.38+/- 8.21% of control group (P< 0.05). These observation suggest that some defects of antitumor function occur in the AMs at the tumor region. GM-CSF can stimulate AMs and thus potentiate their NO activity.

MR imaging in rat glioma model and gene therapy using EGFR antisence RNA.

OBJECTIVE: To study the value of 1.5T MRI in long-term follow-up of rat C6 glioma model and the efficacy of EGFR antisence RNA therapy in vivo. METHODS: Forty-three male Wistar rats, weighing 150 to 200 g, were used for this study. They were divided into four groups: group I (7 normal rats); group II (16 rats inoculated C6 cells in the right caudate nucleus); group III (12 rats with the C6 gliomas treated with EGFR antisence RNA by in site injection); and group IV (8 rats inoculated C6 cells transfected with EGFR antisence RNA in the right caudate nucleus). Group I was examined by plain and enhanced MR scanning. Group II-IV were followed up by plain and enhanced MR scanning and were sacrificed in variable time points for pathological examination. RESULTS: Cerebral hemispheres of normal rat were shown clearly on the MR image. The tumor could be seen about 1 week after inoculation. According to the findings on the follow-up MR scan, we could observe growth of the tumor or its regression after treatment. The tumor growth was significantly inhibited in group III and group IV as compared to those in group II. CONCLUSION: The growth of the rat C6 glioma model and its change after treatment could be shown clearly in 1.5T MR imaging; EGFR antisence RNA significantly inhibited the growth of glioma in vivo.
