Synthesis and X-ray crystal structure of the calcium channel antagonist: dimethyl 1,4-dihydro-2,6-dimethyl-4-[4'-(4H-4-oxo-1-benzopyran-2-yl)phenyl]- 3,5-pyridine dicarboxylate.

The synthesis of the title compound via the Hantzsch method from 4'-flavone carboxaldehyde is described, and its molecular structure was determined by X-ray crystallography. The 1,4-dihydropyridine (1,4-DHP) ring adopts a boat conformation. The phenyl ring of the flavone is not exactly perpendicular to the DHP ring. Calcium antagonistic activity of this compound was evaluated in vitro by using BaCl2-stimulated rat ileum.

A comparative study of recurrent pterygium surgery: limbal conjunctival autograft transplantation versus mitomycin C with conjunctival flap.

OBJECTIVE: To compare the recurrence rate following treatment of recurrent pterygia using one of two techniques-limbal conjunctival autograft transplantation versus low-dose intraoperative mitomycin C (0.2 mg/ml) combined with conjunctival flap closure. DESIGN: Randomized clinical trial. PARTICIPANTS: Eighty-one patients with recurrent pterygia treated by limbal conjunctival autograft transplantation (n= 41) or mitomycin C combined with conjunctival flap (n= 40) participated. INTERVENTION: Limbal conjunctival autograft transplantation or low-dose intraoperative mitomycin C application with conjunctival flap technique was performed on recurrent pterygium cases. MAIN OUTCOME MEASURES: Recurrence of pterygium and postoperative complications. RESULTS: During mean follow-up periods of 16+/-1.9 and 15.5+/-1.5 months, six recurrences (14.6%) in the limbal conjunctival autograft transplantation group and five recurrences (12.5%) in the mitomycin C group were observed (P=0.77). The difference between the mean ages of recurrent (26.4+/-8.0 years) and nonrecurrent (35.8+/-11.9 years) cases for all patients was statistically significant (P=0.014). Technically, limbal conjunctival autograft transplantation seemed to be more difficult. The most frequent complication in limbal conjunctival autograft transplantation was graft edema, whereas that in the mitomycin C group was superficial keratitis. CONCLUSION: Both techniques showed similar recurrence rates in the treatment of recurrent pterygia. Although technically easier to perform, further follow-up is necessary to determine the long-term safety of low-dose intraoperative mitomycin C with conjunctival flap closure. The surgeon's familiarity with either procedure should determine the method of choice.

Secretory breast carcinoma in a boy.

Secretory breast carcinoma is a very rare condition in boys. An 11-year-old boy with secretory breast carcinoma is presented here. Modified radical mastectomy was performed by irradiation and chemotherapy because of axillary metastasis. There was no local recurrence or distant metastases in the follow-up period of 14 months.

Male breast cancer: a 22-year experience.

AIMS: To carry out a retrospective study of male breast cancer over a 22-year experience. METHODS: Data from 121 male patients with breast cancer treated between the years 1972 and 1994 at the Surgical Clinic of Ankara Oncology Hospital were reviewed. Distribution of cases according to stage was: 2.5% stage I, 28.9% stage II, 55.4% stage III and 13.2% stage IV (AJCC staging method). The surgical treatment for 23 of the patients (19%) was Halsted's radical mastectomy or modified radical mastectomy. Seventy-three cases (60.3%) had total mastectomy without axillary node dissection and 25 (20.7%) had local tumour excision only. Seventy-two of 121 patients had adjuvant treatment. RESULTS: In general the prognosis of men with breast cancer was worse than for women. In the analysis of patients in stages I, II and III-A (operable disease group), the 5-year survival rates were 73% in axillary node-negative patients and 77% in those with tumours sized under 5 cm (P<0.001). In these patients, univariate analysis demonstrated that axillary status (relative risk of death in positive status vs. negative=3.6), tumour size (relative risk in T3 vs. T1-2=2), surgical treatment type (relative risk in simple mastectomy vs. radical mastectomy=1.9) and adjuvant chemotherapy (relative risk if no chemotherapy=1.4) were statistically significant factors associated with survival. CONCLUSIONS: Cox's regression model revealed that axillary status, tumour size and type of surgical treatment were the most important independent prognostic factors (P<0.001).

Completion thyroidectomy for differentiated thyroid carcinoma.

Completion thyroidectomy is defined as the surgical removal of the remnant thyroid tissue following procedures less than total or near-total thyroidectomy. The extent of surgical management for differentiated thyroid carcinoma (DTC) is controversial. Although some authors advocate subtotal thyroidectomy with lower complication rates, total or near-total thyroidectomy and completion thyroidectomy have been defended by others because of the improved survival and lower morbidity that is comparable with subtotal thyroidectomy. In this study, the incidence of residual tumor and surgical complication rates in patients who underwent completion thyroidectomy were investigated. The medical records of 165 patients undergoing completion thyroidectomy for DTC were reviewed. Seventy-seven (46.6%) of these patients were found to have residual tumor in the remaining thyroid tissue. Anaplastic transformation developed in two of these patients. Permanent bilateral recurrent laryngeal nerve palsy occurred in three patients, and permanent hypoparathyroidism was seen in one patient. We recommend completion thyroidectomy as an efficient and safe method of surgical treatment with a low complication rate for DTC.

A chronic cavitary pulmonary histoplasmosis case from Turkey.

Histoplasmosis, a disease endemic to North America, is caused by Histoplasma capsulatum. To the best of our knowledge, the chronic cavitary pulmonary histoplasmosis patient described here presents the first case in which the fungus was isolated from sputum culture in Turkey.

Limbal-conjunctival autograft transplantation in cases with recurrent pterygium.

This prospective study was undertaken to evaluate limbal conjunctival autotransplantation in the management of cases with recurrent pterygium. At present, new surgical techniques to prevent pterygium recurrence following surgery are in investigation. In recent years, it has been postulated that pterygium is due to hypofunction of limbal stem cells. Thirty-one out of 49 patients with recurrent pterygium were treated by limbal-conjunctival autograft transplantation and the other 18 treated by Czermak technique, including two line of limbal cauterization intraoperatively, and used as a control group. During a mean follow-up period of 10 months (ranging 3-18 months), 4 recurrences (13.3%) in the limbal-conjunctival autograft transplantation group and 9 recurrences (50%) in the control group were observed. We conclude that this was a successful method to prevent secondary recurrence in the management of recurrent pterygium patients under 40 years of age.

Hydrochloric acid for treating metabolic alkalosis.

Six patients with severe metabolic alkalosis were treated with intravenous hydrochloric acid (HCl) infusion. HCl was given through a central venous catheter, at a concentration of 0.1 mEq per ml. At least two of the following criteria were considered for initiation of the therapy: An arterial pH of greater than 7.45, a base excess (BE) of greater than +7 mmol/L, a PaCO2 of greater than 50 mmHg. The HCl amount was calculated using the BE formula, however, two thirds was infused for avoiding excessive acid loading. Patients were monitored by the blood gases, serum electrolytes, hemoglobin, hematocrit, bilirubin determinations and blood smear findings. While a significant decrease was noticed in pH and BE values, moderate changes were detected in PaCO2 due to different ventilatory status of the cases. All laboratory test results remained within normal limits and no complication was encountered. The advantage of the therapy is that less volume is needed for the correction of alkalosis, particularly in the cases requiring fluid restriction. HCl therapy, moreover, is a safe and time-saving method because of having rapid response to the treatment in the critically ill surgical patients.

Mechanical complications of subclavian vein catheterization. A prospective study.

Subclavian vein catheterization, a timesaving, convenient and easy method of central venous access is not free of complications. In this report 150 consecutive infraclavicular subclavian vein catheterization attempts were evaluated with regard to noninfectious complications. 95.3% of the patients were successfully catheterized. While the overall complication rate was 29.3%, major complications occurred in 2.6%. The complication rate in right-sided attempts (35.5%) was significantly higher than in left-sided attempts (12.5%) (p less than 0.01). All of the major complications occurred in right-sided attempts. Left sided attempts seem to be safer unless specific contraindications exist. Malpositionings were the most common complications (16%) and were easily identified and managed by routine postprocedure chest X-rays. The mortality rate was 0%. As a result we think that this procedure will still remain as a valuable central venous catheterization method in experienced hands with proper indications and prompt treatment in the case of complications.
