Perangiographic rupture of a right posterior communicating artery aneurysm.

Contrast material was seen to extravasate twice from a posterior communicating arterial aneurysm during injection of the internal carotid artery under local anesthesia. The patient made full recovery. The aneurysm was then successfully clipped. The authors review related reports. The increase in distal arterial pressure is thought to be the cause of the aneurysmal rupture.

[Voice discrimination in patients with cerebral cortical lesions].

A study of voice discrimination by unilateral brain-damaged patients is presented. Performance of this task is a very sensitive index of the presence of a cerebral lesion. The worst results are observed when there is a lesion of the right hemisphere and particularly when the right temporal lobe is involved.

[Amnesia syndrome due to tumor infiltration].

The authors present their anatomo-clinical observation of amnesic syndrome occurring with brain tumoral infiltration. On the clinical level, the disorder is characterized by a memory encoding deficit. Anatomical examination shows a bilateral lesion of the mamillary bodies.

Treatment of diabetic coma with low-dose injections of insulin.

Twenty-one patients in severe diabetic coma were treated with small doses of insulin at a rate of 4.1 units per hour (total dose about 100 units per 24 hours). Using single doses of 4 to 10 units by the intravenous or intramuscular routes the fall of blood glucose was steady in all cases. In the treatment of diabetic coma this regimen of insulin administration has proved simple, safe and effective since 1946. Main dangers during recompensation of diabetic coma are: hypovolaemia with oliguria -- anuria, dysequilibrium syndrome with cerebral edema and hypokalaemia. Therefore early intensive and adequate intravenous fluid and electrolyte replacement is the most important part of treatment. Most of the cases in this study were undiagnosed diabetics (14) and elderly patients (9). Three patients older than 65 years and a 56-year old diabetic died. In this context the most important aspects of treatment to avoid death are: prevention of diabetic coma and adequate fluid and electrolyte replacement especially in geriatric patients.

[What should the "organicist" know about the cooperation with the psychoanalyst? (author's transl)].

Important tasks accrue to the family physician when attending a patient concurrently taking psychoanalytic treatment. The specific difficulties possibly ensuing are pointed out. These difficulties are explained to be the results of the recurrence of infantile behaviour patterns on the part of the patient, such as commonly occur in every analysis. The possibilities of overcoming these difficulties and of organizing a favourable cooperation between the family physician and the psychoanalyst are described.

[Variation in the dimensions of the cerebellomedullary cistern after ventriculo-cardiac shunting].

Various morphological changes of the skull and brain were observed after surgical treatment for internal hydrocephalus. An interesting variation is described by the authors, at the site of the cisterna magna, the size of which can vary. Generally it is the last one to dilate when an internal hydrocephalus occurs, but it is the first to regress after inserting a drainage of the cerebrospinal fluid. This behavior of the cisterna magna is discussed, illustrated and reviewed in the literature, after presentation of three clinical histories. From a physiopathological point of view the cisterna magna is considered to be an intermediate formation between the ventricular and the subarachnoid systems.

[Neuropathological lesions in posttraumatic internal hydrocephalus].

The authors described the lesions associated with post-traumatic internal hydrocephalus in 4 cases who died after a prolonged post-traumatic coma. The internal hydrocephalus, quite severe in all cases, and the demyelination in the periventricular white matter in 3 cases, are thought to be produced by a chronic increase in the intraventricular pressure, probably secondary to malfunction of circulation and resorption of the cerebro-spinal fluid.

[Neuroradiology of hydrocephalus in newborns and infants].

Up-to-date views on the various types of infantile hydorcephalus are exposed from a practical neurosurgical point of view. The neuroradiological procedures are divided in 3 groups: the neuroradioisotopes, the angiographies and the pneumographies. They are described and analyzed. Indicators and possibilities are outlined. Their complementary value is stressed.

[A new observation of crossed aphasia in a right-handed man].

The observation of a crossed aphasia is presented in a right handed man. These troubles of the language appeared, following an anterior right temporal lobectomy for ablation of an oligodendroglioma. They are in accordance with the classic data of the litterature as much as by their clinical features than by their evolution.

[Anomia and alexia during a closed craniocerebral injury].

Observation of a traumatic aphasia in a young man. After a coma and a period of mental confusion, language difficulties are limited, during a few weeks, to a severe global deficit in reading and naming. Meanwhile objects and pictures are perfectly recognized and the meaning of some words can be understood correctly. The commentary is made with reference to previous reports of similar case histories. Clinical and neuropsychological data suggest a focal lesion in the left temporal lobe, probably on the 2nd and the 3rd gyri.

[Cause and treatment of hydrocephalus in newborns and infants].

The history and classification of the various types of hydrocephalus are outlined and the mechanisms as well as causes of infantile hydorcephalus are discussed. Acoording to aetiology the possible operative treatments are described and special mention is made of ventriculocardiac shunting with its indications, advantages and complications. Finally, the results that may be expected from operative treatment of infantile hydrocephalus are studied and the neuropsychological aspect is emphasized.
