[Familial thymoma].

The case reports are presented of two siblings with familial thymoma. The thymomas developed at the same age in both siblings. The tumours were removed by thoracotomy and classified histologically as identical thymic squamous cell carcinomas. The brother is still alive and well four years after treatment; the sister died of pulmonary embolism, but without residual or recurrent mediastinal mass, one year after surgery. Familial thymomas are very rare. This is the first report of a familial squamous cell carcinoma of thymus. The problem of classification and malignancy of thymomas are discussed.

Carcinoma, Squamous Cell↗

[Prevention of recurrence of small-cell lung cancer by adjuvant polychemotherapy (author's transl)].

No significant further improvement can be awaited in the results of purely surgical treatment of bronchial carcinoma. As from 1969--after preliminary studies--patients with bronchial carcinoma were treated by radical surgery and subsequently assigned within the framework of a randomized cooperative study, to a three-year course of intermittent high-dose polychemotherapy (cyclophosphamide, 5-FU, MTX and Velbe). A comparison according to the life-table method of the death rate of patients with small-cell bronchial carcinoma treated by combined polychemotherapy after surgery vis-à-vis those who received no polychemotherapy shows a rise of about 15% in the 4-year survival rate. This clear-cut improvement in survival rate emphasizes the fact that the radical operation is also indicated in these tumour forms and that the results of high-dose polychemotherapy are improved with this particular combination. With a view to confirmation and further improvement, a large international working group was established and has already submitted initial positive results concerning the applicability of a new alternating chemotherapeutic schedule. It is pointed out that the initial impetus towards undertaking such studies was given by Prof. Dr. Dr. h.c. Wolfgang Denk already in 1954 and that the consistent continuation of this combined inter-disciplinary approach to the therapy of bronchial carcinoma is founded on his scientific foresight and organisational achievements.

Carcinoma, Small Cell↗

Combination of surgery and polychemotherapy for cure in early small-cell bronchial carcinoma.

In 1969 a cooperative randomized two arm study of surgery and adjuvant chemotherapy was started. Within three years after the operation the polychemotherapy was given intermittently using a combination of cyclophosphamid + 5-FU + MTX + vinblastin. From a total of 859 patients operated for cure preliminary evaluations per 1.3.1981 found 53 patients with small cell type carcinomas. An increase of the 3-year survival rate of 13 per cent shows a clear positive therapeutic effect. This also demonstrates the importance of surgery even on small cell bronchogenic carcinoma by decreasing the tumor burden. To evaluate wether or not the number of long-term survivors can be increased furthermore, a new randomized trial was started in 1979 to compare the polychemotherapy used with a new designed intermittent polychemotherapy-schedule using alternating cyclophosphamid + CCNU + MTX, there after cyclophosphamid + adriamycin + vincristin and there after ifosfamid + VP-16; then radiation therapy and there after repeating the whole sequence.

[Surgery of bronchus carcinoma (author's transl)].

The results of surgery for bronchial carcinoma can be improved by further stressing of early diagnosis, by operative methods adapted to oncological needs and assessment of techniques which increase operability as well as by adequate and general additional therapy. Based on our material of 8,423 patients of which 2,773 (i.e. 23%) have been operated the results of the operation in relation to type of operation, histology, Feinstein's classification, TNM stages and other are demonstrated.

[Surgery of endotheracic struma].

Most cases of endothoracic struma should be considered as systemic illnesses that require thoracic surgery. Concerning cervicoendothoracic borderline cases, the same point of view is advocated, e.g., struma endothoracica falsa and vera alliata. With reference to 38 cases of true and 34 cases of false endothoracic struma, the authors summarize and discuss: 1) their experiences in diagnosis, 2) various routes of access, 3) special thoracic closure techniques, and 4) specific surgical methods on the tumor itself.

[Results after surgical treatment of solitary metastasis of the lung (author's transl)].

The surgery of solitary metastasis of the lung has special ranges of indication which refer to pathomorphology, cardiorespiratory and oncological programming. By systematical selection the own material of 45 cases shows a two year survival time of 33% and a five year of 11%. Last results with survival rate are similar to the treatment of the bronchus carcinoma of all stages and are further justifying this therapeutical consequence.

[Chemotherapy of bronchial carcinomas (author's transl)].

Since the autumn of 1969, studies of surgical adjuvant chemotherapy and of chemotherapy and of chemotherapy alone in bronchogenic carcinoma have been undertaken in a cooperative trial group. Preliminary data from 171 radically operated patients, 88 randomized control patients, and 460 inoperable patients receiving chemotherapy only are presented. Chemotherapy was carried out as follows: 1-2 weeks after surgery the first intravenous infusion of 500 ml of 5% Leavulose including 12 mg/kg of cyclophosphamide, 12 mg/kg of 5-fluorouracil, 0.5 mg/kg of methotrexate, and 0,1 mg/kg of vinblastine was administered. The protocol requires the administration of 13 such infusions within 3 years after the operation. In general, the treatment which consisted of a total of 1445 infusions administered to 171 radically operated patients, was well tolerated and caused only minor side effects. The survival rate of the treated group of patients (calculated by the life-table method) appears promising compared with the survival rate of the randomized and historical control patients. The subdivision of patients into Feinstein categories shows a great variation, indicating the importance of the clinical parameters in influencing the prognosis. Based on a total of 2752 chemotherapeutic infusions in 460 patients with inoperable bronchogenic carcinoma, it can be stated that the treatment used was surprisingly well tolerated considering the advanced stage of disease and therefore the poorer general condition of these patients. These preliminary results justify the continuation of these clinical trials in a larger cooperative study group.
