Bilateral simultaneous rupture of the quadriceps tendon in an adult amateur tennis player.

Bilateral simultaneous quadriceps tendon rupture is a relatively rare injury, requiring prompt diagnosis and surgical repair. It is more common in patients older than 50 years and is usually associated with underlying metabolic or inflammatory diseases. Its occurrence is very rare in healthy individuals. We report a case of bilateral quadriceps tendon rupture in a healthy young adult that occurred while he was playing tennis. This patient presented with acute knee swellings, total inability to extend the legs, and a palpable gap above the patella.

[Hip arthroscopy--indications, surgical technique and implementation].

Hip arthroscopy was first described by Burman in 1931. During the 1980's this procedure was performed by a limited group of orthopedic surgeons. Implementation and development of the surgical procedure received most attention during those years, together with the formulation of specific indications to perform hip arthroscopy. In contrast to other large joints (knee, shoulder), where accurate diagnosis is feasible using standard imaging procedures such as plain radiographs, computed tomography and magnetic resonance imaging, the hip joint often presents as a diagnostic anomaly. In specific cases hip arthroscopy can add invaluable information, which, in correlation with the symptoms of the patient, can lead to an accurate diagnosis. Hip arthroscopy has various indications including the removal of foreign or loose bodies, irrigation of an infected hip joint and evaluation of the cartilage. The list of indications is still expanding as more surgeons become familiar with the technique. Hip arthroscopy is a safe and efficient procedure in experienced hands. This review of the literature describes the surgical procedure, various clinical situations, indications and the therapeutic potential of hip arthroscopy.

[Fracture of the supracondylar process of the humerus associated with Monteggia fracture].

Fractures of the supracondylar process of the humerus have been rarely described in the medical literature. This is a bony projection that arises from the anteromedial surface of the distal humerus, and present in only one per cent of the population. We present a patient with this rare injury associated with a Monteggia fracture of the forearm. The proximity of neurovascular structures to the process makes them vulnerable to injury associated with this fracture. We present a review of the literature and treatment guidelines.

Capsular disruption as a complication of thermal alteration of the glenohumeral capsule.

Thermal energy, delivered through various means, has been used to alter the length of collagen in the unstable shoulder. Laboratory studies have shown that the collagen, following thermal alteration, has reduced strength. We present a case of radiofrequency capsulorrhaphy of the shoulder for symptomatic posterior instability in a multidirectionally lax patient that led to early catastrophic failure of the capsule, with complete capsular disruption and hyaline degeneration of the treated tissue.
