Dorsal wrist syndrome repair.

Dorsal wrist pain with or without a palpable dorsal wrist ganglion is a common complaint. Watson developed the concept of the dorsal wrist syndrome (DWS) which is an entity encompassing pre-dynamic rotary subluxation of the scaphoid and the overloaded wrist. We reviewed 20 cases of DWS treated surgically. There were nine males (11 wrists) and nine females (nine wrists). Post-operative follow-up ranged from five to 67 months (mean, 37 months). At operation, we observed SLL tears in eight wrists and dorsal ganglia in 12 cases. Following surgery, 12 cases reported being pain free, five had mild pain, two moderate pain and one case reported severe pain. Post-operative extension/flexion was 73/70 average. Post-operative grip strength was 28 kg average. We believe that excision of the posterior interosseous nerve and the dorsal capsule including the ganglion, if present, provides pain relief in DWS.

Enchondroma protuberans of the phalanx: a case report.

Enchondroma protuberans, which is defined as an exophytic enchondroma of a long bone, is a rare condition. There have been 6 previously reported cases with only 1 case occurring in the hand. We report a case of an enchondroma protuberans of the ring finger middle phalanx treated by marginal excision of the soft-tissue component, intramedullary curettage, and placement of calcium phosphate cement. There was no evidence of local recurrence at the 1-year follow-up examination.

Duplicate extensor tendons of the thumb mimicking rupture of the extensor pollicis longus tendon.

We report a patient with duplicate extensor pollicis longus tendons, who complained of inability to extend the thumb fully because of impingement of the musculotendinous junction of the duplicate tendons. Extension of the thumb recovered completely after Lister's tubercle was excised and the extensor retinaculum was released. Selective windowing with highlighting of the soft tissue in the axial T1-weighted magnetic resonance image at the level of the distal radioulnar joint showed two tendons in the third extensor compartment.

Technique of intramedullary fixation for arthrodesis of the wrist in rheumatoid arthritis.

We did 18 arthrodeses of the wrist in 16 patients with rheumatoid arthritis using an intramedullary fixation technique. There were 15 women and one man, whose ages at operation ranged from 47 to 71 years (mean 58). Follow up ranged from 13 to 68 months (mean 27). The operative technique consists of a combination of intramedullary placement of two Kirschner (K)-wires and an autogenous bone graft. At follow up bony union was apparent in all cases. K-wires came out of the metacarpal joints in two cases. Paraesthesiae in the median nerve distribution occurred in two cases which both recovered within three months.

Rotational radioulnar dissociation: a case report and review of the literature.

We report a rare case of radioulnar dissociation in an 11-year-old boy associated with fracture of the medial humeral epicondyle. Open reduction and Kirschner wire fixation was performed for the epicondylar fracture. The radial head was reduced closed, and the distal radioulnar joint reduced spontaneously. The mechanism of this unusual injury appears to be hyperpronation of the forearm and valgus stress to the elbow.

VLA-4-dependent and -independent pathways in cell contact-induced proinflammatory cytokine production by synovial nurse-like cells from rheumatoid arthritis patients.

Nurse-like stromal cell lines from the synovial tissue of patients with rheumatoid arthritis (RA-SNC) produce, on coculture with lymphocytes, large amounts of proinflammatory cytokines. In the present paper, we analyze the molecular events necessary for the induction of cytokine release from RA-SNC cells, and particularly the roles played by cell adhesion and the transmigration (also known as pseudoemperipolesis) of lymphocytes. For this purpose, the effects of various mAbs on the binding and transmigration of a human B-cell line, MC/car, were examined using a cloned RA-SNC line, RA-SNC77. To analyze the role of lymphocyte binding and transmigration on upregulated cytokine production by the RA-SNC77 cells, we used C3 exoenzyme-treated MC/car cells, which could bind to RA-SNC77 cells but could not transmigrate. Treatment with anti-CD29 or anti-CD49d mAb significantly reduced binding and transmigration of the MC/car cells. In contrast, the neutralizing anti-CD106/vascular cell adhesion molecule 1 mAb did not show any inhibitory effect. Likewise, none of the neutralizing mAbs against CD11a, CD18, CD44, CD49e, or CD54 showed significant effects. Binding of C3-treated or untreated MC/car cells to RA-SNC77 cells induced comparable levels of IL-6 and IL-8 production. In addition, the enhanced cytokine production by RA-SNC77 cells required direct lymphocyte contact via a very late antigen-4 (VLA-4)-independent adhesion pathway. These results indicate that, although both the VLA-4-dependent/vascular cell adhesion molecule 1-independent and the VLA4-independent adhesion pathways are involved in MC/car binding and subsequent transmigration, only the VLA4-independent adhesion pathway is necessary and sufficient for the enhanced proinflammatory cytokine production by RA-SNC77 cells. The transmigration process, which is dependent on Rho-GTPase, is not a prerequisite for this phenomenon.

IL-12-induced production of IL-10 and interferon-gamma by mononuclear cells in lung cancer-associated malignant pleural effusions.

Interleukin (IL)-12 enhances natural killer (NK) activity and induces interferon gamma (IFN-gamma) production. Recently, it was shown that IL-12 induces IL-10 production by human T cells and NK cells, as a negative feedback for IL-12-induced immune responses. In the present study, in order to investigate the functions of host immune cells existing in contact with cancer cells, we examined the effect of IL-12 on the induction of non-major histocompatibility complex (MHC)-restricted killer activity and of IFN-gamma and IL-10 production by pleural and peripheral blood mononuclear cells (MNC), isolated from 40 lung cancer patients and 20 control subjects. IL-12 induced significant killer activity in pleural MNC from lung cancer patients, as well as those in peripheral blood, against a small cell lung cancer cell line (SBC-3). In lung cancer patients, pleural MNC incubated with IL-12 produced more IFN-gamma than blood MNC. In addition, when stimulated with both IL-12 and IL-2, pleural MNC produced more IL-10 than blood MNC. This is the first study reporting that MNC from pleural effusions of patients with lung cancer can produce both type 1 (IFN-gamma) and type 2 (IL-10) cytokines following exposure to IL-2 and IL-12. These observations suggest that control of IL-10 production at the microenvironment level may be important for the efficacy of human lung cancer immunotherapy with IL-12.

Total video endoscopic thyroidectomy via the anterior chest approach using the cervical region-lifting method.

Endoscopic surgery offers superior cosmetic results compared to open procedures and is strongly preferred by many patients, especially women. We performed total endoscopic thyroidectomy via the anterior chest approach using a neck skin-lifting technique in which the skin is lifted by a large number of hooks to create the working space. This method is quite flexible and can be modified based on the size of the space needed. The fine hooks leave no scar on the anterior neck, the skin incisions are small, and the scars are completely covered by patients' undergarments. Endoscopic thyroidectomy is suitable for benign thyroid nodules, but some malignant foci diagnosed by frozen section usually can be managed without conversion to an open procedure. Women under 45 years of age with nodules <2 cm who have no evidence of lymphatic spread or local invasion are ideal candidates for this procedure.

Periprosthetic fracture of the tibia associated with osteolysis caused by failure of rotating patella in low-contact-stress total knee arthroplasty.

Periprosthetic fracture of the tibial plateau associated with osteolysis resulting from mechanical failure of the rotating patellar component after total knee arthroplasty with the New Jersey Low-Contact-Stress (LCS) knee (DePuy, Warsaw, IN) has not been reported previously. A 67-year-old woman with rheumatoid arthritis of the left knee had a LCS prosthesis implanted without cement, using a rotating patellar component. Seven years later, a fracture of the lateral tibial plateau occurred owing to an osteolytic defect with no traumatic accident. The rotating patellar bearing over-rotated and locked; consequently, wear occurred between the patellar metal tray and the femoral component. Immunohistochemistry revealed CD68-positive macrophages in the osteolytic region and phagocytosis of metal particles. The osteolytic region was filled with autogenous bone, and all components were exchanged and cemented. The patient's condition became satisfactory with relief of pain.

Scapulohumeral rhythm: relationship between motion velocity and rhythm.

The relative contributions of scapulothoracic and glenohumeral motion at different rates of shoulder motion were studied through adduction to abduction in the scapular plane. Nineteen shoulders of 10 healthy individuals (all men, 24-30 years of age) were analyzed using an image intensifier and a high-resolution digital video system. High- and low-speed motion consisted of 2 and 4 seconds per one cycle, respectively, from abduction to adduction in the scapular plane. Glenohumeral and scapulothoracic ratios were fixed at low speed and these results agree with the finding of other researchers. Ratios at high speed were not fixed and differed significantly from those at low speed. Ratios were high at the beginning of abduction or adduction and at angles beyond 40 degrees abduction, then decreased according to the arm movement. Glenohumeral motion at high speed was more dominant at the beginning of abduction or adduction beyond the setting phase, then became less dominant according to the arm movement, compared with the motion at low speed.

Lack of reported mutations in a Japanese case of toxic multinodular goiter.

We describe a toxic multinodular goiter that was subjected to genomic sequencing of the thyroid-stimulating hormone receptor and the alpha-subunit of the stimulatory guanine nucleotide-binding protein. No mutation was detected in either gene. While frequent occurrence of thyroid-stimulating hormone receptor gene mutations in toxic multinodular goiters has been reported in areas such as part of Europe with a relatively high prevalence of iodine deficiency, toxic multinodular goiters in areas with an abundance of dietary iodine such as Japan seem unlikely to involve mutations of this gene.
