Neuropathology in Alzheimer's disease: target of pharmacotherapy.

The pathogenesis of AD is complex and not yet fully understood. A number of factors, including amyloid plaques, NFTs, and inflammatory processes, are likely to contribute to development of the disease. Acetylcholine and glutamate are intimately involved in learning and memory. Hypotheses implicating defects within both neurotransmitter systems in AI) are recognized. This knowledge coupled with ongoing discoveries about the multiple pathophysiologic pathways involved in development and progression of AD has given rise to several plausible therapeutic targets. Therapies addressing some of these targets (ie, acetylcholine, glutamate) have already shown clinical efficacy in treating AD while other targets continue to be investigated.

Overview of Alzheimer's disease.

In conclusion, many researchers from multiple disciplines are addressing AD at many levels, ranging from prevention, to cellular alterations, to treatment modalities, and coping and care giving with the consequences of the ravages left by the disease. Although much has been learned in the last several decades of active research, much remains to be learned. In its current status, AD is a chronic disease without a cure, which provides ample opportunities for nursing management throughout the course progression.

Care of individuals with Alzheimer's disease in the new millennium.

The incidence of Alzheimer's disease (AD), the leading form of dementia, is expected to increase exponentially in the next 50 years. The nursing and medical care of individuals diagnosed with AD, including the health-related challenges faced by all aging individuals, are discussed.

Disruptive behaviors of older adults in an institutional setting. Staff time required to manage disruptions.

This study, as part of a larger project, examined the actual time recorded by nursing staff to manage 36 disruptive behaviors in older adults who are institutionalized. Disruptive behaviors were defined as socially unacceptable or isolating, observable actions with negative consequences. A prospective study was conducted using a sample of 153 patients in a Veterans Affairs institution (mean age = 72.6, SD = 10.8). Data related to time to manage disruptive behaviors were collected during 21 consecutive shifts for each patient. Nursing staff did not always intervene in all disruptive behaviors that occurred. When they did intervene, total time to manage a disruptive behavior ranged from 5.7 to 201.5 minutes (mean = 23.1 minutes, SD = 31.9).

Dementia in the new millennium.

Alzheimer's disease, the leading form of dementia, is expected to increase exponentially in the next 50 years. An overview of types of dementia and the typical presentation of AD are described. In addition, rationale for early assessment and methods for diagnosing the disorder are detailed.

Autopsy 101 (CE).

Although autopsy remains the only definitive way to confirm and sometimes diagnose disease, the autopsy rate has continued to decline during the past 3 decades. Despite numerous publications citing its value, the nursing literature lacks information addressing the benefits and implications of the autopsy to nurses who provide direct care to older adults. This article provides geriatric nurses with information about the benefits, purpose, process, and barriers of autopsy to better inform nurses as they ease family concerns and assist physicians in obtaining consent for autopsy.

Enriching geropsychiatric nursing in advanced practice nursing programs.

The need for advanced practice nurses with knowledge and skills in geropsychiatric nursing is real and looming larger than ever. By 2030, the number of older adults with major psychiatric illnesses is predicted to reach 15 million. Preparing advanced practice nurses with mental health and geropsychiatric nursing expertise is essential. This paper describes three innovative curricular models for addressing geropsychiatric nursing in graduate advanced practice nursing programs. These models can be implemented with vision, planning, modest resources, and the cooperation of the faculty.

Living with fair use and TEACH: a quest for compliance.

With the expansion of Web-based courses in nursing education, faculty members are faced with a greater responsibility to be copyright compliant. This article reviews the changes in copyright law. The Conference on Fair Use (CONFU), the passage of the Digital Millennium and Copyright Act (DMCA), the passage of the Technology Education and Copyright Harmonization (TEACH) Act, and recent legal cases will be discussed. The strategies and resources the authors used to secure copyright permission while designing a Web-based continuing education course will also be described.
