Unified model for the ultrafast decay of pyrimidine nucleobases.

Ultrafast decay processes detected after absorption of UV radiation in gas-phase pyrimidine nucleobases uracil, thymine, and cytosine are ascribed to the barrierless character of the pathway along the low-lying 1(pipi*) hypersurface connecting the Franck-Condon region with an out-of-plane distorted ethene-like conical intersection with the ground state. Longer lifetime decays and low quantum yield emission are on the other hand related to the presence of a 1(pipi*) state planar minimum on the S1 surface and the barriers to access other conical intersections. A unified model for the three systems is established on the basis of accurate multiconfigurational CASPT2 calculations, whereas the effect of the different levels of theory on the results is carefully analyzed.

Analysis of changes in recent tuberculosis transmission patterns after a sharp increase in immigration.

We conducted a population-based molecular epidemiological study of tuberculosis (TB) in Madrid, Spain (2002 to 2004), to define transmission patterns and factors associated with clustering. We particularly focused on examining how the increase in TB cases among immigrants in recent years (2.8% in 1997 to 1999 to 36.2% during the current study) was modifying transmission patterns. Mycobacterium tuberculosis isolates obtained from patients living in nine districts of Madrid (1,459,232 inhabitants) were genotyped. The TB case rate among foreign-born people was three to four times that of Spanish-born people, and the median time from arrival to the onset of treatment was 22.4 months. During the study period, 227 (36.3%) patients were grouped in 64 clusters, and 115 (50.7%) of them were in 21 clusters with mixed Spanish-born and foreign-born patients. Three of the 21 mixed clusters accounted for 21.1% of clustered patients. Twenty-two of 38 (57.9%) immigrants in mixed clusters were infected with TB strains that had already been identified in the native population in 1997 to 1999, including the three most prevalent strains. Factors identified as independent predictors of clustering were homelessness (odds ratio [OR], 2.3; 95% confidence interval [95% CI], 1.2 to 4.5; P = 0.011) and to be born in Spain (OR, 1.8; 95% CI, 1.2 to 2.6; P = 0.002). The results indicated that (i) TB transmission was higher in Spanish-born people, associated mainly with homelessness, (ii) that foreign-born people were much less likely to be clustered, suggesting a higher percentage of infection before arriving in Spain, and (iii) that an extensive transmission between Spanish- and foreign-born populations, caused mainly by autochthonous strains, was taking place in Madrid.

Correlation between pain, disability, and quality of life in patients with common low back pain.

STUDY DESIGN: Correlation among previously validated questionnaires. OBJECTIVES: To determine the correlation between pain, disability, and quality of life in patients with low back pain. SUMMARY OF BACKGROUND DATA: The Visual Analogue Scale (VAS), and the Roland-Morris (RMQ), Oswestry (OQ), and EuroQol (EQ) Questionnaires are validated instruments to assess pain, low back pain-related disability, and quality of life. METHODS: The study was done in the primary care setting, in Mallorca, with 195 patients who visited their physician for LBP. Individuals were given the VAS, RMQ, OQ, and EQ on their first visit and 14 days later. RESULTS: Median duration of pain when entering the study was 10 days (P25,P75: 3, 40). On day 1, simple correlation was r = 0.347 between VAS and RMQ, r = -0.422 between VAS and EQ, and r = -0.442 between RMQ and EQ. On day 15, simple correlation was r = 0.570 between VAS and RMQ, r = -0.672 between VAS and EQ, and r = -0.637 between RMQ and EQ. Multiple linear regression models showed that, on day 1, the VAS score explains 12% of the RMQ score and the VAS and RMQ scores explain 27% of the EQ score. On day 15, the VAS score explains 33% of the RMQ score, and the VAS and RMQ scores explain 58% of the EQ score. On day 1, a 10% increase in VAS worsens disability by 3.3% and quality of life by 2.65%. On day 15, a 10% increase in VAS worsens disability by 4.99% and quality of life by 3.80%. Prestudy duration of pain had no influence on any model. All these correlation coefficients and models are significant at the P < 0.001 level. The OQ had lower correlation values with the other three scales, and only two of them were significant. CONCLUSION: Clinically relevant improvements in pain may lead to almost unnoticeable changes in disability and quality of life. Therefore, these variables should be assessed separately when evaluating the effect of any form of treatment for low back pain. The influence of pain and disability on quality of life progresses while they last, and doubles in 14 days. In acute and subacute patients, this increase is not dependent on the previous duration of pain.

Chemometric studies of chemical compounds in five cultivars of potatoes from Tenerife.

A statistical study of correlation and multivariate analysis on the chemical composition of five cultivars of potatoes harvested in Tenerife was carried out to establish the relationships between the chemical compounds and, therefore, to differentiate the samples according to traditional and recent importation potatoes, cultivars, and species/subspecies. A large number of significant correlations between the chemical compounds were found, which suggests biochemical relationships among them. After factor analysis, the dimension space was reduced from 24 variables to eight factors, accounting for 77.2% of the total variance. Starch, moisture, organic acids, and metals are the variables that make it possible to characterize the system without losing very much information. Total differentiation of potato samples according to the criteria species/subspecies and cultivars was obtained using discriminant analysis with all the variables. However, with only four variables (weight of tuber, starch, amylose, and glucose + fructose) it is possible to differentiate between the traditional and recent importation potatoes.

Hemispheric lateralization of language in epileptic right-handed children with unihemispheric discharge.

The epileptic discharge of one hemisphere interferes with brain function. The present study assesses the linguistic disabilities of epileptic patients owing to discharge in one hemisphere. Subjects were classified by age and gender. A significant deterioration of linguistic abilities appeared in adult subjects discharging in the left hemisphere. In children, however, a more symmetric hemispheric pattern of epileptic discharge interference appears with age up to 14 years. Unihemispheric epileptic discharges could also provide information about the degree of lateralization that a given function presents in different developmental stages. This lateralization appears clearly established from the age of 14 to 21 years until late adulthood. These results imply that hemispheric specialization of language is not completed until adolescence. This absence of hemispheric specialization for language in childhood was statistically more significant in boys than in girls, suggesting that the maturation of cortical areas related to language occurs earlier in girls than in boys. Finally, the familial history of left-handedness is a factor delaying the age of left lateralization of language in right-handed children.
