[The contribution of food and airborne allergens in the pathogenesis of atopic dermatitis].

Food hypersensitivity and airborne allergens may play a role in the pathogenesis of atopic dermatitis (AD). The aim of this study was to evaluate the kind of food and airborne allergens which may most often induce and intensify AD lesions and also to assess the variability and the kind of allergens leading to AD. The subjects of this study were 610 persons, aged 3 months-70 years. The clinical status of the patients was estimated by an atopic dermatitis symptom score scale (SCORAD). The laboratory examinations differentiated inflammatory processes from allergic reactions. The skin prick tests (SPT), serum total IgE and specific IgE-antibody levels to chosen food products and standard airborne allergens with the immuno-enzymes method ELISA-DPC were performed. The elevated values of the total IgE were proved in 46.1% children from group 0-15 years and in 31.4% of adolescents and adult persons (above 15 year of age). On the basis of positive SPT and positive specific IgE values it was shown, that most frequent food allergens were: egg protein (13.0%), cow milk (9.5%), egg yolk (8.4%), wheat (3.6%) and chocolate (1.8%). The most often airborne allergens connected with AD were: grass (11.6%), moulds (10.2%), house dust mites (9.3%), pollen like hazel (8.0%) and weeds (6.7%), animal allergens coming from cats (7.2%) and dogs (6.1%). The food hypersensitivity was particularly manifested in children. It may be the predictor of potential future development of allergic disease as well as the indicator of the allergic march.

[Do young people feel the need for social support in our time?].

The aim of the work was determination of the connection between young people and people important in their lives among persons of the same age and among adults by the use of the "Social Support" questionnaire that is one of the four parts of translated version of German inquiry "Woman Self Image and Social Ideal". There were 713 persons included in the examinations divided into 2 groups: I--313 pupils including 128 boys and 185 girls. They were pupils of second and third forms of secondary schools in Cracow; II--400 students, 106 male students and 294 female students, all of them were from Faculty of Medicine and from the faculty of health Protection of Collegium Medicum of Jagiellonian University. The questionnaire included 22 questions and had five-grade scale of intensity of feelings: does not concern, a little, moderately, concerns, definitely concerns. The positive result of the examination is the statement that more than the half of the examined persons--both pupils and students--has a reliable friend in every situation; feels well and relaxed with this person; may trust him/her, discuss difficult problems and get keys from one's flat during being away. On the other hand the frequency of answers showing important problems in this matter is still distressingly high. The need of better understanding and care from the family was declared by about 15% of examined persons. 19% of pupils and 22% of students wanted bigger safety and close relationship. At the same time the safety, understanding and care was experienced by less than 20% of examined persons. The fact of isolation from the group and the feeling of being an outsider declared 10% of young people. The examinations show that problems with adjusting to the requirements of an adult life, finding the place and defining one's role in the surrounding reality concerns not only a part of secondary school pupils but older than that--students of Faculty of Medicine.

Importance of parenteral nutrition in patients undergoing hemopoietic stem cell transplantation procedures in the autologous system.

OBJECTIVE: The aim of this study was to assess the frequency of parenteral nutrition and to compare the impact of parenteral and oral feeding on the nutrition and clinical status of adults undergoing autologous hemopoietic stem cell transplantation. METHODS: The study involved 35 patients with neoplasm of the hemopoietic system who underwent hemopoietic cell autotransplantation at the Hematology Clinic (Jagiellonian University, Krakow, Poland). The patients' nutrition status was assessed using body mass index (BMI) values, body mass components, concentration of albumin, and total protein in blood serum. The clinical status evaluation included duration of hematologic reconstruction, concentration of bilirubin, enzyme activity (alanine aminotransferase and aspartate aminotransferase), severity of infections, and duration of hospitalization. RESULTS: Parenteral nutrition was required in 19 patients. Oral feeding was used in 16 patients. Symptoms of malnutrition on the day preceding the introduction of conditioning treatment were recorded only in patients requiring parenteral nutrition (31.6%). In the posttransplantation period, a statistically significant decrease in body mass was observed in both groups, whereas the share of fatty tissue in total body mass was significantly less in patients (men and women) fed parenterally. CONCLUSION: A supply of 25-30 kcal/kg and 1-1.5 g protein/kg/day as an element of parenteral nutrition (where 20%-30% of the energy requirement was covered by fats, 15%-20% by amino acids, and 50%-55% by glucose) helped prevent the development of malnutrition and restore the functions of the hemopoietic system at a level comparable to that for patients fed naturally.
