Excision of rhinophyma with a laser scanner handpiece--a modified technique.

Rhinophyma is a slowly progressive, benign dermatological disorder of the tip of the nose. The most widely accepted theory is that rhinophyma is the end stage result of chronic acne rosacea. The primary reason for excision of rhinophyma is cosmetic deformity. However, nasal obstruction may present, as rhinophyma may obstruct nasal vestibules. Surgery is the treatment of choice for rhinophyma. Medical therapy has not given satisfactory results. The surgical treatment modalities are divided into two main groups. The first is complete excision, with primary closure for small lesions, or skin grafting for large lesions. The second group includes incomplete excision followed by re-epithelialization from the remaining glandular epithelium. The carbon dioxide (CO2) laser has been advocated for excision of rhinophyma. Newer instrumentation, such as Swiftlase and SurgiTouch (ESC Sharplan) or Ultrapulse (Coherent) is more effective in vaporization and carbonization resulting in better hemostasis (Ries and Speyer, 1996). In this report, 7 patients with rhinophyma treated with the Swiftlase are reviewed. The surgical technique, the complications and the follow-up are discussed.

QT prolongation and torsades de pointes associated with concurrent use of cisapride and erythromycin.

Prolongation of QT interval may lead to serious, potentially life-threatening, ventricular tachyarrhythmia, such as torsades de pointes. The cause may be an inherited or an acquired malfunction of ion channels at the myocardial cell membrane. Metabolic abnormalities, starvation, nervous system injury, and drug administration cause the much more frequent acquired long QT syndrome (LQTS). Types Ia and III antiarrhythmic drugs account for the majority of these life-threatening events, whereas a number of drugs widely used in otolaryngology, such as antibiotics and antihistamines, have been recently implicated. A case of a life-threatening ventricular tachyarrhythmia after the concurrent administration of cisapride and erythromycin is presented. Reviewed are drugs commonly prescribed in otolaryngology, as well as the associated risk factors that potentially lead to LQTS.

Intranasal laser-assisted dacryocystorhinostomy with the use of a surgical microscope.

PURPOSE: Obstruction of the nasolacrimal duct is a serious chronic condition, treatment for which has not yet been successfully resolved. Several methods have been proposed for the management of chronic epiphora. A modified method of intranasal laser-assisted dacryocystorhinostomy with the use of a surgical microscope is described. MATERIALS AND METHODS: The procedure is a modification to the West and Veis-Claus-Guttich method. The primary and late results of this method are presented for 53 patients over the past 7 years with chronic epiphora caused by stenosis of the nasolacrimal duct. Selection criteria included chronic epiphora, nasolacrimal duct stenosis due to Down syndrome, empyema of the lacrimal sac, and recurrence from previous operations. Patients were operated on under general anesthesia. RESULTS: Fifty-one patients (96.3%) were free of symptoms postoperatively (mean follow-up time, 6 months). CONCLUSION: The simplicity of this method compared with traditional methods is correlated with excellent postoperative results and patient satisfaction.

Long-term results of a new stapedotomy prosthesis.

The long-term results of a new, home-made stapedotomy prosthesis are presented. The innovations of this prosthesis concern both portions, design for attachment to the incus and shaft. Our series include a total number of 42 individuals with stapes fixation, operated using the new prosthesis, with a medical follow-up period between eighteen months and five years. Our innovations offer a proper and safe insertion of the prosthesis into the oval window associated with excellent manipulation and handling. These advantages could be of major importance for younger surgeons and trainees. Postoperative hearing results are similar to those achieved with other commercially available pistons.
