Spontaneous rupture and emergency repairment of the renal pelvis.

A case of spontaneous rupture of the renal pelvis is very rare and unusual occurrence. Our case was a young woman who did not present with any disease that was a factor for pelvic rupture. The diagnosis was confirmed by abdominopelvic CT, and emergency repair of the renal pelvis was performed and a ureteral stent was inserted. Appropriate antibiotherapy was ordered, and the recovery was rapid. The differential diagnosis of this ease is discussed and the literature is reviewed:

Unexpected complication after cystometry in the hypocompliant urinary bladder: formation of a knot in the double lumen urethral catheter--a case report.

Urodynamic evaluation is frequently used in the follow-up of the treatment and diagnosis of incontinence, which develops in connection with a neurogenic or non-neurogenic reason. There is no identified serious complication during or after urodynamic evaluation, present in the literature up to date. Hematuria, due to the urethral catheter, the development of oedema in the urinary bladder wall and the development of urinary bladder spasm as a result of catheter irritation, are some of the complications, which may occur. In this paper, twist and knot formation in the double lumen urethral catheter after cystometry of a patient with a hypocompliant urinary bladder, has been presented.

Effect of sildenafil citrate on viability of flaps: an experimental study in rats.

Various pharmacological agents have been used to try and elucidate the pathophysiology of ischaemia and necrosis of flaps. Their most important disadvantage is the need for relatively high doses given systemically, which increases the risk of potential side effects. Topical or local agents are more useful. Sildenafil citrate, the specific inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE 5) was studied as an antianginal drug during the late 1980s, but is now used for its effect on erectile function in men. Sildenafil citrate causes dilatation of peripheral arteries and veins and the inhibition of the thrombus-forming ability of platelets in vivo. Our study was designed to test the efficacy of sildenafil citrate on the viability of flaps.

A huge bladder cystine stone.

An unusual cystine stone that occur only in patients, who have cystinuria is presented in 24-year-old man. Radiographs showed a giant bladder stone shadow, 8.0 x 10.0 x 5.0 cm in size. The literature was reviewed to identify the giant stones as well as the huge cystine stone which is exceptional yet as.

Transurethral electrovaporization of the prostate: is it any better than standard transurethral prostatectomy? 5-year follow-up.

BACKGROUND AND PURPOSE: At present, transurethral resection of the prostate (TURP) is regarded as the most effective treatment for benign prostate hyperplasia (BPH). In the present randomized study, we compared TURP with transurethral electrovaporization of the prostate (TUVP). PATIENTS AND METHODS: A series of 77 BPH patients who underwent operation between 1996 and 2001 were included in the study, 40 who had TURP with a standard resection loop and 32 who had TUVP with a Spike loop. All available patients were evaluated preoperatively, at 1 month and 3 months after the operation (N=73), and 5 years after the operation (N=44). The International Prostate Symptom Score (IPSS), sexual function questionnaire, uroflowmetry (Qmax), postvoiding residual urine volume (PVR), and transrectal ultrasonography were used. In addition, postoperative serum sodium concentration, serum hematocrit, duration of catheterization, and operation time were compared. RESULTS: In the TUVP group, the mean IPSS decreased from 17.3 to 6.5, the PVR from 88 to 35 mL, and the prostate volume from 39 to 24 cc, while the Qmax increased from 6.3 to 12.9 mL/sec and the average flow from 2.6 to 7.9 mL/sec. In the TURP group, the mean IPSS decreased from 17.6 to 6.1, the PVR from 95 to 38 mL, and the prostate volume from 38 to 23 cc, while the Qmax increased from 5.9 to 13.2 mL/sec and the average flow from 2.4 to 8.3 mL/sec. Thus, significant improvements were observed in both groups, and the differences between them 5 years later were not significant P>0.05). Postoperative serum hemoglobin and hematocrit were significantly lower in the TURP group (P>0.05). However, no difference was seen in serum sodium concentrations. Also, there was no difference in the duration of the operations. The catheterization period was 22+/-5.7 hours in the TUVP group, while it was 75.7+/-10.5 hours in the TURP group (P<0.001). When morbidities observed during follow-up were evaluated, no significant difference was seen. CONCLUSIONS: The TUVP procedure is as effective as TURP with similar morbidity. The advantages of TUVP are that the urethral catheter is withdrawn earlier, hospitalization is shorter, and bleeding is less. Its disadvantage is that tissue cannot be sampled for histologic diagnosis. In our opinion, in order to exploit the advantages of this technique, TUVP should be preferred in elderly patients without any suspicion of prostate cancer for whom bleeding during operation is a source of risk.

Using porcine acellular collagen matrix (Pelvicol) in bladder augmentation: experimental study.

PURPOSE: Evaluate the rabbit augmented bladder with Pelvicol. MATERIALS AND METHODS: Twenty New Zealand rabbits were divided into 4 groups. Bladder augmentation was performed using a 10 x 10 mm sized porcine acellular collagen matrix. The material was placed on the dome of the bladder wall as a patch with 5-0 polyglycolic sutures. The bladder was resected on the 7th, 14th day, 30th and 90th days, and processed for histological analysis. RESULTS: No stone formation was found in the first, second and fourth weeks. In the first week, there was inflammatory appearance and roughness in the reconstructed area when compared to other sites on the bladder wall. The material could not be seen in some bladders because of acute inflammatory reaction. The normal bladder epithelium was found on the part of the bladder wall that follows the surface of the eroded material. In the second week, edema was observed through the bladder wall. Perivesical fat tissue increased and it was not easy to distinguish it from the surrounding area. In the fourth week, the bladder wall was thickened and there was a sensation of hardness present. The inner and outer surface of the material was darker than in the other bladders. In the third month, there was no inflammatory reaction; however, there was micro calcification and irregular detrusor regeneration. CONCLUSIONS: Pelvicol cannot be suitable material for bladder augmentation because of the resultant micro calcification, thickening of the bladder wall and irregular development of detrusor regeneration.
