Assessment of enthesitis in ankylosing spondylitis by power Doppler ultrasonography.

OBJECTIVE: To evaluate the relationship between power Doppler ultrasonography (PDUS) assessment and clinical variables including enthesitis index, pain threshold and disease activity parameters, and to document grey-scale US findings of the 13 entheses examined. DESIGN AND PATIENTS: A total of 390 entheses were examined in thirty patients with AS, and clinical variables of the Maastricht Ankylosing Spondylitis Enthesitis Index (MASES), anthropometric measurements, disease activity and functional parameters were documented. A total MASES score by palpation (t-PS) and algometric pressure pain threshold (t-PPT) was obtained. Grey scale and PDUS examination of 13 entheses were performed. Grey-scale changes such as altered tendon echogenity, calcification, cortical reactive changes and bursitis were noted, and flow on PDUS was graded semi-quantitatively. RESULTS: Cumulative power Doppler (t-PDS) score significantly correlated with t-PS and t-PPT. Ultimate correlations were found between power Doppler scores and pain, disease activity and disability parameters. Changes in grey scale combined with PDUS were more prevalent in lower-extremity entheses. The intraobserver agreement of flow signal grading was excellent (kappa = 0.82). Clinical and sonographic results were concordant for three regions, but were discordant for four regions where tenderness was accepted as the sole clinical manifestation of enthesis. CONCLUSION: Pain or tenderness is associated with increased vascularity of entheses. Power Doppler US examination of the entheses may be useful and complementary to the clinical evaluation, and further research is needed to assess its role in diagnosis and follow-up of disease course.

Power Doppler assessment of overall disease activity in patients with rheumatoid arthritis.

PURPOSE: To examine synovial vascularity and flow patterns in hand and wrist joints--metacarpophalangeal (MCP) joints and ulnar stiloid (USTL) regions--of patients with rheumatoid arthritis (RA) using power Doppler sonography (PDUS) and spectral Doppler analysis and to assess the accuracy of PDUS in detecting overall disease activity in RA patients. METHODS: Two hundred forty MCP joints and 48 USTL regions in 24 RA patients were examined. Patients were categorized into 2 groups--active and inactive--according to the American College of Rheumatology remission criteria. Resistance indexes (RIs) were measured. RESULTS: Flow signals were detected in 50 MCP joints (in 13 patients) and 24 USTL regions (in 16 patients) and spectral analysis was performed in 46 MCP joints (12 patients) and 23 USTL regions (16 patients). The sensitivity and specificity of PDUS in detecting disease activity in RA were 92% and 40%, respectively. There was a negative correlation between flow signal number and RI, with higher scores of flow signals corresponding to lower RIs. CONCLUSION: PDUS appears to be a reliable method for assessing inflammatory activity in rheumatoid synovium.

Endometriosis: sonographic spectrum.

Endometriosis is the presence of functional endometrial tissue outside the uterine cavity and the myometrium. It is an important cause of infertility and pelvic pain in women older than 25 years. Endometriomas are relatively common and can mimic other adnexal masses, and ultrasonography is often the first choice of imaging. This pictorial essay presents the sonographic appearance of endometriomas and similar-appearing adnexal masses and their characteristics, which may aid in their differentiation on routine ultrasonographic examination.

Recurrent retroperitoneal cystic lymphangioma.

Retroperitoneal cystic lymphangioma is a rare congenital malformation. The majority of lymphangiomas are present at birth and nearly all present before the age of two years. We report a case of giant cystic retroperitoneal lymphangioma in a patient who first presented with symptoms at the age of 7, underwent surgery, and who then suffered a recurrent mass 11 years later.

Familial Mediterranean fever responds well to infliximab: single case experience.

The most common arthritic involvement in familial Mediterranean fever (FMF) is acute recurrent monoarthritis; however, sometimes spondyloarthropathy-like findings or typical ankylosing spondylitis may also ensue. Reported here is our favorable experience with infliximab in an FMF patient who had been resistant to colchicine and disease-modifying antirheumatic drugs (sulfasalazine and methotrexate) treatments. A 72-week follow-up of the patient yielded complete remission of the febrile abdominal episodes, and spondylitis responded well. The patient's bilateral aseptic necrosis of the femoral head deteriorated and caused hip pain, discomfort, and disability. Overall, we believe that tumor necrosis factor (TNF) alpha has an important role in the disease pathogenesis and also that anti-TNF may represent a promising robust treatment alternative in FMF.

Effects of sildenafil on major arterial blood flow using duplex sonography.

PURPOSE: Erectile dysfunction (ED) is the persistent inability to achieve or maintain an erection sufficient for satisfactory sexual performance. Oral use of sildenafil citrate is effective in the treatment of ED. Although the effects of sildenafil citrate have been investigated in several systems, its effect on aortic, superior mesenteric (SMA), and carotid artery blood flow is still unclear. The aim of this study is to investigate the early phase effects of sildenafil citrate on aortic, SMA, and carotid artery blood flow using color duplex sonography (CDS). METHODS: Thirty-four patients with ED (aged 19-71) were included in this study. Peak systolic velocity, end diastolic velocity, and resistance index (RI) in aorta, SMA, and bilateral common and internal carotid arteries were measured at baseline and 45-75 minutes after the administration of sildenafil citrate using CDS. RESULTS: Statistically significant changes were observed in only 3 of 18 parameters: an increase in post drug values of bilateral internal carotid artery peak systolic velocity (Vmax) compared to baseline values and a significant decrease in the RI value of the left main carotid artery after drug administration compared to baseline values. CONCLUSIONS: Sildenafil citrate had no significant effect on aortic and SMA circulation and only caused mild changes in the carotid artery circulation. Although these alterations may be considered clinically insignificant, further studies assessing long-term effects of sildenafil are warranted.

Splenic vein aneurysm with calcification of splenic and portal veins.

Splenic vein aneurysm is a rare disease, and calcifications in the portal venous system are also rare. Here we present a case of splenic vein aneurysm associated with calcification of the splenic and portal veins, a condition that to our knowledge has not been reported before.

Hemodynamic effects of sildenafil citrate (Viagra) on segmental branches of bilateral renal arteries.

INTRODUCTION: Erectile dysfunction (ED) is one of the most encountered male health problem and sildenafil citrate is used worldwide for treatment of ED. Aim of the present study was to determine and evaluate hemodynamic effects of sildenafil citrate on segmental branches of bilateral renal arteries of cases with ED. PATIENTS AND METHODS: Total of 35 male patients aging between 34 and 56 years were recruited into study. Patients were evaluated with renal color Doppler sonography (CDS) before and after (45-75 min) peroral sildenafil citrate (50 mg) administration. Peak systolic flow rate (V(max)), end diastolic flow rate (V(min)), resistive index (RI) and pulsatility index (PI) of renal artery segmental branches were determined before and after sildenafil citrate administration. RESULTS: V(min) measured at right lower segment was significantly reduced after sildenafil administration (p = 0.030). Accordingly mean RI and PI of right lower segment of renal artery slightly increased after drug administration (p = 0.007 and p = 0.012, respectively). The measurements on other segments had no significant change after sildenafil treatment when compared to baseline values. On this single dose administration no side effect was recorded. CONCLUSION: Findings of the present study demonstrated that peroral sildenafil citrate usage had slight effects on hemodynamic parameters of lower segmental branch of right renal artery. This result could not be interpreted as robust evidence for hemodynamic changes of renal arteries associated with sildenafil usage nevertheless warrants more comprehensive research with larger number of patients.

Renal multidector row CT.

Multidetector row CT is the most recent advance in CT technology. An increased number of detector rows and more powerful x-ray tubes result in faster scanning time, increased volume coverage, and improved spatial and temporal resolution. MDCT technology allows superior image quality, decreased examination time, and the ability to perform complex multiphase vascular and three-dimensional examinations.

Effectiveness of vardenafil versus papaverine in penile Doppler ultrasonography.

PURPOSE: To evaluate erectile dysfunction, penile color Doppler ultrasound is currently considered the best method. But intracavernous injection is invasive and has adverse effects, such as prolonged erection and risk of priapism. In our study we want to evaluate whether vardenafil can be used instead of papaverine in penile Doppler ultrasonography. MATERIALS AND METHODS: A total of 24 patients with erectile dysfunction underwent color Doppler ultrasound before and after intracavernous injection of 60 mg papaverine with genital and audiovisual sexual stimulation. Peak flow and end diastolic velocity were measured in the recorded waveforms obtained 0, 1, 5, 10 and 20 min after injection. The patients also underwent color Doppler ultrasound after a 10-mg oral dose of vardenafil with genital and audiovisual sexual stimulation and at least 5 days after the prior examination. The same parameters were measured at 30, 45, 60, 75 and 90 min after the drug administration. We compared the results with the values obtained after papaverine injection. RESULTS: After oral vardenafil mean peak flow velocity significantly increased starting at 30 min and achieving a maximum value at 60 min. There were no significant differences in the 2 methods in mean peak velocity 1, 5, 10 and 20 min after papaverine injection, and 30, 45, 60, 75 and 90 min after oral vardenafil administration. Although papaverine injection is the gold standard for penile color Doppler ultrasound, it has severe adverse effects such as prolonged erection which we observed in 3 patients (12.5%) and required pharmacological detumescence. After vardenafil no severe adverse effects were observed. CONCLUSIONS: Vardenafil administration achieved increased peak flow velocity comparable to that after intracavernous papaverine injection. With no prolonged erection vardenafil is a safer alternative compared to more invasive intracavernous injection and is also an alternative for patients who fear injections.

Sonographic evaluation of penile trauma.

OBJECTIVE: The purpose of this presentation is to illustrate the normal sonographic anatomy of the penis and various imaging manifestations of penile trauma. METHODS: Penile trauma cases were collected and reviewed retrospectively from our archive. RESULTS: The normal sonographic anatomy of the penis and various pathologic conditions of patients with penile trauma are discussed. CONCLUSIONS: Sonography (especially with color Doppler sonography) is a useful imaging method for evaluating penile anatomy and various pathologic features in patients with penile trauma.

Primary hydatid disease of sacrum affecting the sacroiliac joint: a case report.

STUDY DESIGN: A case report of hydatid disease of the spine. OBJECTIVE: To describe an unusual case of hydatid disease of the sacrum affecting the sacroiliac joint and to discuss imaging, differential diagnosis, and treatment. SUMMARY OF BACKGROUND DATA: Hydatidosis or echinococcosis affecting the spine is rare and has a characteristic geographic distribution. Signs of sacroiliac joint involvement and accompanying neurologic deficits cause difficulties in differential diagnosis of this rare condition. METHODS: A case of 38-year-old female patient with low back pain and sciatica was presented. RESULTS: Plain radiographs, computed tomography, and magnetic resonance imaging scans revealed destructive expansive lesion located on the right sacrum and extended through the right sacroiliac joint. Surgical enucleation of the cysts was performed together with mebendazole treatment and histopathologic examination confirmed hydatidosis. CONCLUSION: This unusual disease should be kept in mind in the differential diagnosis of sacroiliac pain and sciatica, especially in endemic areas.

Evaluation of metacarpophalangeal joint synovitis in rheumatoid arthritis by power Doppler technique: relationship between synovial vascularization and periarticular bone mineral density.

OBJECTIVE: To evaluate intra-articular vascularization and flow patterns of the metacarpophalangeal (MCP) joints in rheumatoid arthritis (RA) patients by using power Doppler technique. The relationship between the flow patterns and the regional bone mineral density (BMD) measurements of MCP joints using dual-energy X-ray absorptiometry (DXA) was also assessed. METHODS: Fifteen patients (mean age 48.5 +/- 13.8) with a disease duration of 6.8 +/- 4.9 years were included into the study. Thirty MCP joints of three healthy subjects were also assessed as controls. Clinical activity of MCP joint inflammation was classified according to a modified index for synovitis activity based on joint swelling, warmness and pain. Ultrasonography was performed using LOGIQ 7 unit by means of a 6-13 MHz linear array transducer. Gray-scale US and power Doppler sonography (PDS) examination was performed. Using spectral Doppler the quality of flow was determined and the indices of pulsatility (PI) and resistance (RI) were measured in longitudinal plane. The erosive scores of the MCP joints were determined on the plain hand radiographs using Sharp/van der Heijde method. Hand BMD, MCP BMD and the ratio of MCP BMD to whole hand BMD (rBMD) were measured using a Lunar DPX densitometer. RESULTS: Total 150 MCP joints of 15 patients and 30 MCP joints of three healthy controls were examined in both hands. Flow was determined in 31 joints (10 moderately active, 21 active) in eight patients whereas seven patients had no quantifiable flow in MCP joints. MCP joints of healthy controls had no quantifiable flow. Mean PI of these joints were 2.17 +/- 2.37 in moderately active and 2.12 +/- 1.93 in active MCP joints. RI was 0.76 +/- 0.12 in moderately active and 0.75 +/- 0.15 in active MCP joints. There was no significant difference in RI and PI of these active and moderately active MCP joints. There was a significant correlation between erosive scores and PI, RI of total MCP joints (r = 0.40 and r = 0.41, P < 0.05). PI and RI also negatively correlated with rBMD (r = -0.46, P < 0.01 and r = -0.36, P < 0.05, respectively). PIc was 4.07 +/- 3.26 and RIc was 0.88 +/- 0.16 in the neighboring soft tissues which were significantly higher than PI and RI of MCP joints (P < 0.001). CONCLUSION: Power Doppler ultrasonography (PDUS) is a useful method demonstrating synovial vascularization and flow patterns and offers new alternatives for monitoring disease activity and measurement of therapeutic response. Flow patterns had intimate correlation with intra-articular bone and cartilage destruction.

MRI findings of intraosseous lipoma: case report.

Intraosseous lipoma is one of the rarest benign bone tumors and usually presents in the fourth to fifth decades. Except for minor symptoms, they are usually asymptomatic. Metaphysis of long bones, especially proximal parts of femur and fibula, are involved in most cases. We present intraosseous lipoma to discuss the benefits of MRI and contrast-enhanced MRI findings for differential diagnosis.

Rupture of Baker's cyst producing pseudothrombophlebitis in a patient with Reiter's syndrome.

This is an unusual case of pseudothrombophlebitis resulting from rupture of Baker's cyst in a patient with Reiter's syndrome. The patient presented with a swollen, painful left calf and persistent itching on the skin of the calf. Ultrasonography showed a ruptured popliteal cyst with minimal hemorrhage and fluid collection within the fascial compartments and gastrocnemius muscle. Color Doppler ultrasound showed a patent popliteal vein and artery and duplex Doppler scans revealed a normal flow pattern. In conclusion, the clinical picture of deep vein thrombosis and that of pseudothrombophlebitis are difficult to distinguish by clinical examination and necessitate detailed examination by imaging techniques. Persistent pruritus on calf skin resulting from irritation of inflammatory synovial fluid may be an important clinical feature.
