Assessment of renal maturity by assisted morphometry in autopsied fetuses.

BACKGROUND: The kidney is one organ which presents distinct morphology depending on GA; the classic references for kidney measurements were established several years ago and it is not certain if they are still accurate. AIM: To evaluate the renal maturity using a computer-assisted morphometry in autopsied fetuses within 20 to 40 weeks of GA. STUDY DESIGN: Microscopic hematoxylin and eosin stained renal sections from 67 stillborns were utilized for the morphometric analyses. RESULTS: It was possible to promote an adjustment in the formula used to calculate the number of mature glomeruli layers to the GA (GLN = 0.212GA + 0.0169) in autopsy material and it was different from the literature (Sr = 0.709; p < 0.001). Regarding the nephrogenic zone, it decreases with the GA as confirmed by the regression equation NZ = 490.7-(11.9 x GA) (p < 0.001; Sr = -0.685). CONCLUSION: In conclusion, our data contribute to the evaluation of renal maturity and GA in autopsied fetuses, showing particular importance in autolyzed fetuses, in which the parameters used in the present study can still be applied. The correction of the formula for counting mature glomeruli layers for each GA and the addition of new morphometric parameters for the evaluation of renal maturity increase the precision for the analysis of spontaneous abortion and autopsy material, improving the correlation with pathological processes in clinical findings and in the other organs.

Bronchogenic carcinoma of squamous cells in a young pregnant woman.

Lung cancer continues to be the most incident neoplasia in the world and also the most frequent cause of death by cancer, with more than 900000 new cases per year. Women present greater risk of developing bronchogenic carcinoma, possibly because of differences in metabolism of the carcinogens associated with tobacco and also because of hormonal differences because estrogens can promote the appearance of lung cancer, including during pregnancy. We have described the case of a 25-year-old patient who developed squamous cell carcinoma of the lung during her third pregnancy, with a progressive worsening of her general condition and progression until death. We believe that the pregnancy could have contributed to the evolution of the disease, especially because of the increased levels of gestational hormones, particularly the estrogen, because its receptors have an important role in regulating growth and in the differentiation of several tissues facilitating like this, the development of the neoplasia, and complicating its early diagnosis.

Histopathologic changes in human true vocal folds: a postmortem study.

Several articles describe the gross pathology alterations of the true vocal cords (TVC), but the histopathology descriptions in adults are rare in the literature and are normally associated with neoplasia. The aim of this study is to identify the TVC morphological lesions in adult autopsies. This was a cross-sectional study of 266 adults' larynxes consecutively autopsied from 1993 to 2003. Histochemical and immunohistochemical methods were used for morphological analysis. Considering the morphological analysis, 97 TVC (36.5%) were considered normal. The following lesions were found in the TVC: thickening of the basal membrane of the TVC in 81 patients (30.5%), inflammatory reaction in 66 patients (24.8%), hemorrhage in 19 patients (7.1%), and ulceration in 3 patients (1.1%). Our study demonstrated that the TVC microscopic lesions are frequent, present different morphological patterns, and showed different causes from what had been described until nowadays by the literature. Despite this difference in the morphological pattern, we were able to identify a common lesion pathway for ethiological different diseases.

Inflammation and glandular duct dilatation of the tongue from patients with chronic Chagas disease.

The purpose of this study was to evaluate morphologically the tongue of individuals with chronic Chagas disease (CD) in comparison to the non-chagasic ones. Twenty-four protocol cases of autopsies were selected. They were subdivided into CD patients (10 cases) and non-chagasic ones (14 cases). The morphometric analysis was accomplished for the tongue muscle and salivary glands duct lumen area. In three CD patients, perineuritis was found, and two of them showed megaesophagus and megacolon. The intensity of the inflammation in the von Ebner's glands, the tongue muscles, and the salivary glands duct lumen area was significantly higher in the CD patients. We concluded that the CD patients show salivary glands duct dilatation, which probably would have a relation with alterations in the autonomic nervous system. The inflammation found in CD patients is in accordance with that described in comparative studies on the digestive tract and heart. These morphological findings suggest that the histopathological analysis of the tongue associated with other organs, or even in an isolated manner, can add in the diagnosis and pathogenesis of the CD chronic phase.

Relationship between the morphologic alterations of vocal cords from adult autopsies and the cause of death.

PURPOSE: The purpose of this study was to identify the possible alteration in the thickness of the epithelium basal membrane of the vocal cords and correlate it with the cause of death. METHOD: Larynxes collected from adult autopsies during the period of 1993 to 2001 were utilized. We used the hematoxylin-eosin and periodic acid-Schiff staining methods for the morphological and morphometric analysis. RESULTS: Sixty-six vocal cords were analysed; increased thickness was identified in 14 cases (21.2%), with equal proportions between the genders. Increased vocal-cord thickness was more frequent in patients of the white ethnicity (12 cases, 85.7%). Respiratory alterations were found in 10 (71.4%) of the cases with increased vocal-cord thickness. Of the patients that were maintained with mechanical ventilation before death, 7 (18.4%) had thickening of the basal membrane. Among the smokers, 9 (19.63%) had basal membrane thickening. CONCLUSION: No statistically significant differences were found between the cases in which the cause of death was related to respiratory diseases as compared to non-respiratory diseases and the thickening of the basal membrane of the vocal cords. However, new studies are needed in order to verify the etiopathogenesis of this thickening.

Morphometric study of the spleen in chronic Chagas' disease.

There is frequently an increase in spleen size in infectious systemic and chronic venous congestion. The aim of this report was to perform a comparative study of the spleen tissue of chagasic or nonchagasic autopsied patients with or without congestive heart failure. Evaluations were made of 111 cases. Connective tissue intensity, follicular density and area, and follicular arterioles wall area were determined through the morphometric study. The connective tissue was similar in all groups. The density of the lymphoid follicles was significantly less among the chagasic cases (P = 0.032). The follicular area was larger among the chagasic cases and in the chagasic group with congestive heart failure. The chagasic group without congestive heart failure presented a greater area of follicular arteriole walls. Therefore, the spleen modifications in chronic Chagas' disease could be a consequence not only of the heart failure but also of the Chagas infection itself.
