[Monoamnotic twin pregnancy. Multicenter study].

OBJECTIVE: The purpose of this study was to evaluate of perinatal mortality and morbidity of monoamniotic twins. STUDY DESIGN: This was a multicenter retrospective analysis of 26 monoamniotic twin gestations identified between 1985 and 2004 in a 3 perinatal departments. Of these 26 pregnancies, 11 women were admitted electively for inpatient fetal monitoring. Overall mortality rates, the risk of intrauterine fetal death and neonatal mortality and morbidity, pregnancy complications and fetal anomalies were calculated. RESULTS: Monoamniotic twin pregnancies were diagnosed reliably prenatally by ultrasound in 22 women and at delivery in 4 cases. Of the 26 gestations, spontaneous fetal losses before 22 weeks of gestation were 4 cases. The overall loss rate and the perinatal mortality rate were 52% and 19.4%, respectively. Twenty-two women had both twins alive at 24 weeks of gestation; 11 women were admitted electively for inpatient fetal monitoring at 26-27 week of gestation. In this group there were 2 neonatal deaths. No intrauterine fetal death occurred in any hospitalized patient. In our series there were 100% incidence of prematurity, 38,5% of umbilical cord entaglement, 23% of TTTS, 3.6% of TRAP and 9.6% fetal congenital anomalies. CONCLUSION: Monoamniotic twins are at extremly risk of pregnancy complications and fetal loss. These pregnancies can be diagnosed reliably by ultrasound in most cases. Electively admitted women for inpatient fetal monitoring could be improved neonatal survival and decreased perinatal morbidity.

[Pregnancy in primigravida complicated by premature rupture of membranes in 15th week of pregnancy with successful delivery outcome of the mature newborn--case report and review of the literature].

Premature rupture of membranes and amniotic fluid leakage is a very serious complication of pregnancy. In most cases it causes the premature contractile activity of uterine muscle and premature labour. It is a very rare situation when after premature rupture of membranes at less than 20 weeks of gestation it is possible to carry pregnancy to term with a successful delivery outcome of healthy mature infant. In the study we analysed the course of pregnancy in 28 years old primigravida with PROM from 15th week of pregnancy with a successful delivery outcome of mature newborn in 37th week of gestation. Our analysis revealed that in certain cases of premature rupture of membranes it is possible to successfully prolong the duration of pregnancy till the foetus matures. It seems necessary to improve the management which allows to prolongate the duration of pregnancy in premature rupture of membranes.

[Successful termination of pregnancy complicated by fetal ascites--case report and review of the literature].

This is a case of 25-year-old woman, primigravida, admitted to our department in 27th week of pregnancy complicated by fetal ascites revealed by ultrasound examination. The patient underwent diagnostic amniopunction and viral examinations in 20th week of pregnancy. The results from these studies were normal. This pregnant woman was qualified for fetal abdomen puncture, fluid evacuation and shunt applying between fetal abdomen and amniotic sac. Further outcome of pregnancy was uncomplicated. A live female child was born by caesarean section in 38th week of gestation. The shunt was removed after delivery. The woman and the baby were discharged in a good general condition.

[The mode of anaesthesia for caesarean section in the opinion of pregnant and delivering women].

OBJECTIVES: Conduction anaesthesia is regarded as a very safe method for the mother and for a newborn. This kind of anaesthesia reduces the risk of gastric contents aspiration and does not cause the respiratory depression. However in every case it is essential to take into consideration the woman's opinion, for whom delivering of the child is the particularly important event. DESIGN: The aim of the study was the evaluation the presence and the intensification grade of side effects of the anaesthesia to the caesarean section and establishing, which kind of anaesthesia to this operation: general or conduction is assessed as the best method for the delivering women. RESULTS: In study group among 76 women after cesarean section were 49 (64.5%) after epidural anaesthesia, 22 women (28.9%)--after spinal anaesthesia, and 5 patients (6.6%) were operated in general anaesthesia. The most common side effects after anaesthesia for cesarean section were: weakness and backache. After POP the headache was much more often symptom. CONCLUSIONS: Most of pregnant women preferred epidural anaesthesia during delivering of the child by caesarean section.

[Leptin concentration in pregnant women with hypertension and its correlation with body mass index].

UNLABELLED: Arterial hypertension is one of the reasons of high rate of mother and fetal mortality. The main aim of the study was to find the correlation between the leptin concentration, the BMI value and the risk of hypertension in pregnancy. MATERIAL AND METHODS: the study was conducted in High Risk Pregnancy Department of the Medical University of Lodz in 2001-2003. The study group consists of 126 pregnant women with arterial hypertension. BMI, leptin concentration and arterial blood pressure were measured. The measurement was done every 4 weeks from 20 weeks of gestation, Leptin Elisa kit was used. The leptin measurement was done spectrophotometrically. RESULTS: In group of obese pregnant women we found the correlation between BMI and high blood pressure if the high BMI was observed before pregnancy. Leptin concentration and the risk of hypertension was higher in the group of obese pregnant women. CONCLUSIONS: In obese pregnant women the risk of HA was high and the high value of BMI before pregnancy is the risk factor of hypertension in pregnancy.

[The total antioxidative activity measured by FRAP method in women with intrauterine growth restriction treated by L-arginine].

UNLABELLED: The total antioxidative activity means the organisms ability to defend against free radical reactions. This can be changed in pathological pregnancies. DESIGN: The aim of the study was to find the correlation between total antioxidative activity in the serum of healthy pregnant women and in intrauterine growth restriction. The decrease of total antioxidative activity may be connected with high risk of cell membrane destruction. High risk of destruction of different biological cell functions by free radical processes is observed in IUGR and hypertension. MATERIAL AND METHODS: The study was taken in Medical University in Lodz in 1999-2003. The study group consists of 80 women with IUGR, the controls of 60 healthy pregnant women. L-arginine - nitric oxide precursor and acetylsalicylic acid were used in the group of IUGR. The total antioxidative serum activity was measured by FRAP method and expressed in mmol/l. RESULTS: In IUGR group total antioxidative activity was lower before treatment and it increases after the treatment. In controls, the value was higher than in IUGR group before and after the treatment. CONCLUSIONS: The significant difference between two groups and the increase of total antioxidative activity in IUGR due to the L-arginine treatment was found.

[Ultrasonographic diagnosis of cervical incompetence].

OBJECTIVE: The aim of this study was to verify clinical diagnosis of cervical incompetence by the use of transvaginal sonography. MATERIAL AND METHODS: 101 pregnant women hospitalized at the Pathological Pregnancy Department, Medical University of Lodz in years 2000-2002 because of suspicion of cervical incompetence were analysed. The mean age of examined group was 24 +/- 3.24 yr. All pregnant women underwent ultrasound cervical assessment at the 18-30 week of gestation by using Hitachi EUB 515C system. Sonographic measurements of cervical length and width were performed. The cervical length below 30 mm and width at least 6 mm were sonographic criteria of cervical incompetence. RESULTS: In 63 pregnant women cervical incompetence was not confirmed by ultrasound examination. In 38 patients sonographic features of cervical incompetence were established. 38 pregnant women were underwent elective cervical cerclage for cervical incompetence. Because of poor previous history 8 women with normal ultrasound examination were treated by cervical cerclage. CONCLUSIONS: We conclude, that clinical diagnosis of cervical incompetence should be verified by the use of transvaginal sonography to provide an objective diagnosis.
