The emergence of international telemedicine: no ready-made solutions exist.

Sending images to the far end of the world and establishing a diagnosis that may save one human life is a technological success which is very extremely gratifying, scientifically. But it's only the first, though essential, stage towards developing an international telemedicine network. The difficulties of conforming the telemedicine network to technical standards, administrative regulations, and existent national and supranational legislations are presented in this paper; there are often no ready-made solutions to these problems.

[Course of ischemic risk in treated atheromatous hypertensive patients. The PRIHAM study].

Hypertension is one of the major cardiovascular risk factors. However seldom occurs alone in a patient and the antihypertensive treatment should be taken into account when assessing global cardiovascular status. The PRIHAM study was an open study which enrolled 3,216 hypertensive atheromatous patients who already has had a clinical ischaemic accident. Patients were followed-up for 3 years. The lowering effect on cardiovascular risk, starting from the fifteenth month was confirmed to the end of 36-month study. The effect on blood pressure was observed from the third month. Stabilized blood pressure observed within the first year persisted throughout the three years without tachyphylaxis. It was effective on SBP (-14.4%) and on DBP (-15%) while maintaining a stable heart rate both in the supine position or immediately upon standing or after 3 minutes in the standing position. Amongst diabetic and dyslipidemic patients, an absence of deleterious metabolic effects was noticed, parallel with the effectiveness. Tolerance was globally judged as good or very good by the investigating practitioners and the patients expressed an improvement in their feeling of well-being. In conclusion, from the level of effectiveness obtained and the low incidence of adverse effects, urapidil appears well adapted and easy to manage in a population with a particularly high cardiovascular risk.

[Effect of buflomedil perfusion on peripheral tissue oxygenation during transluminal angioplasty in patients with advanced arteriopathy of the lower limbs].

Randomized double-blind placebo controlled trial performed in two parallel groups of patients, aged 43 to 92, suffering from advanced arterial disease of the lower limbs to be treated with angioplasty. The aim was to compare the efficacy of infusions of buflomedil with placebo on peripheral tissue oxygenation following lower limb angioplasties. Treatments consisted of 3 hour infusion of buflomedil or isotonic saline combined with arterial disobstruction. The major outcome measures were transcutaneous oxygen tension (TCpO2) correlated to ankle systolic pressure index (SPI). An increase of TCpO2 values (p < 0.01) as well as the mean individual variations of TCpO2 (p < 0.05) before and after angioplasty were observed in the buflomedil-treated group, whereas SPI values varied similarly in both groups. No adverse event was shown. In conclusion, buflomedil infusions during angioplasties of the lower limbs may improve tissue oxygenation subsequent to arterial disobstruction and are well tolerated.

Influence of estrogens and progesterone on the venous system of the lower limbs in women.

Treatments with estrogens and progestogens are suspected of causing vascular complications either directly or by metabolic consequences. Although many studies have demonstrated an increased incidence of arterial and deep venous thrombosis, since 1970 the dose of estrogens and progesterones have been lowered with a proportional lowering of side effects. After classification of estrogens and progestogens, we studied their peripheral vascular effects. In our study, we demonstrated that the effects of estroprogestogen treatment on the superficial venous system depend on the dose of estrogen and progestogen.

[Weight indices during the first 18 months of life in low birthweight infants].

A prospective study was carried out among 190 low birthweight neonates (less than 2,500 g) in order to assess their ratio of total body fat by 2 adiposity indices: Weight/Height3 (W/H3) at birth and Weight/Height2 (W/H2) subsequently. Adequate for gestational age (AGA) girls with weight indices (W/H3) above the 10th percentile (AGA/I+) had a lower height than AGA/I- girls on the 6th (p less than 0.01) and 9th (p less than 0.05) months of corrected chronological ages. Mean weight and height of small for gestational age (SGA) neonates with weight indices (W/H3) lower than the 10th percentile (SGA/I-) were similar to the SGA/I+ as soon as 3 months of corrected chronological age. SGA/I+ infants experienced a progressive increase in low weight indices (W/H2) until 15-18 months without significant differences with SGA/I- at 3, 6, 9 months of corrected chronological age. Meanwhile, low indices (W/H2) were more frequently observed in SGA/I- at 15-18 months. Thus the early characteristics in weight indices disclose a lower ratio of body fat in SGA infants than in normal children, at least up to age 15-18 months.

Acne: evolution of the clinical practice and therapeutic management of acne between 1996 and 2000.

According to epidemiological studies, acne is a common condition affecting 80% of young people between 12 and 18 years of age. It is also the most common reason for consultation in private dermatology practices. Yet, paradoxically, very few studies have been concerned with the profile and therapeutic treatment of acne patients. The aim of this study was to compare both the evolution of the profile of acne patients and the therapies used by dermatologists between 1996 and 2000 through a survey. Two questionnaires had to be filled in by the dermatologists for all patients consulting for acne, the first in 1996 and the second in 2000. The purpose of these questions was to obtain information about the demographic characteristics of the patients, a clinical evaluation of the patients' acne, their quality of life and finally their treatment. The statistical tests used Chi2 tests and variance analysis. The alpha risk was fixed at 0.05. Two thousand two hundred and eighty four patients were included in the survey in 1996 and 2363 in 2000. The mean age was 21 7.6 in 1996 and 20.5 7.7 in 2000, the majority of whom were female (64.4% in 1996 and 63.6% in 2000). In 1996 37.4% were students, and 52.4% in 2000. For 81.1% of the patients, the consultations were spontaneous on their own initiative both in 1996 and 2000. As regards topical treatment, a decrease of antibiotic prescriptions was noted between 1996 and 2000 associated with a decrease of benzoyl peroxyde use. Concerning the systemic treatments, an increase of doxycyclin and isotretinoin prescriptions was reported together with a decrease of minocycline treatment. Prescription of zinc salts and hormonal treatments were stable. The frequency of biological investigations remained similar and low. The repercussions of acne on the patient's quality of life were more numerous in 1996 compared to 2000 but they remained equally intense for those who reported them. The profile of acne patients evolved slightly between 1996 and 2000 and interestingly the treatments were modified to take into account the information in the literature such as bacterial resistance or severe side effects.

[Inflammation and subcutaneous calcification of venous origins].

There have been few descriptions up to now of calcifications in chronic venous insufficiency, other than in cases where venous insufficiency is complicated be severe trophic disorders and in particular ulcers. It was therefore felt to be of interest to assess the presence of calcifications in venous insufficiency without trophic disorders. This study was based upon 40 cases recruited in the phlebology out-patient clinic of the Notre Dame de Bon Secours Hospital. Calcifications of the lower limbs were found in 7 patients, either by palpation, routine X-rays or ultrasonography. The etiopathogenic mechanisms of this occurrence not having been elucidated, a number of hypotheses are put forward on the basis of acquired data concerning: the process of formation of ectopic calcifications, changes in subcutaneous tissue, the ultimate consequences of venous stasis and of raised venous pressure, due essentially to anoxia and inflammation. One hypothesis can thus be put forward: that of inflammation. The release of cells and mediators of inflammation, the production of free radicals, causing damage to the cells of connective tissue and to the organic framework (collagen fibres) and changes in the chemical environment could combine to result in the formation of calcifications in subcutaneous tissue. However, inflammation has not been proven to be the primary etiological factor.

[Comparative study of the effectiveness of continuous or intermittent courses of a phlebotonic drug on venous disorders disclosed or aggravated by oral, estrogen-progesterone contraceptives].

In patients with venous disorders related to oral contraceptives, venotonic treatment should be prescribed for as long as contraception is maintained. 2,295 patients between the ages of 20 and 40 years with venous insufficiency revealed or aggravated by oral contraceptives were randomised to receive venotonic treatment for one month either continuously or in parallel with active contraceptive treatment, from the 10th to the 28th day. The results demonstrated the values of prolonged treatment of venous disease, as its effects persisted and were reinforced with time. The possibility of intermittent prescription would appear to improve compliance and lower the cost of treatment while ensuring comparable long-term efficacy.

[Patients with the highest cardiovascular risk are the least compliant with their antihypertensive therapy (DESIR study)].

AIM OF THE STUDY: To identify the sociodemographic and clinical profile of hypertensive patients who are not compliant with their antihypertensive treatment. METHODS: each cardiologist described his or her next 4 hypertensive patients from a clinical standpoint and gave them a self-administered compliance questionnaire developed by the French Committee to Fight Hypertension, which they returned directly to the analysis center using a postage-paid reply envelope. RESULTS: 1965 patients 63.9 +/- 12.1 years old, 55.3% of whom were male, were included in the study. According to the specific questionnaire, compliance is definitely satisfactory in 35.9% of patients, is probably satisfactory in 28.3%, is probably poor in 19.4% and is definitely poor in 16.4%. Poor compliance is more frequent among men (38.1 vs. 33.4%; p < 0.05), overweight or obese patients (35.8 and 43.0% vs. 30.0%; p < 0.001), diabetics (46.7 vs. 32.3%; p < 0.0001), dyslipidemic patients (39.3 vs. 31.8%; p < 0.001), smokers (50.2 vs. 33.8%; p < 0.0001), those whose father died of cardiovascular causes before 55 years of age (51.6 vs. 34.1%; p < 0.0001) or those with a previous history of CV events (40.6 vs. 32.8%; p < 0.001). The rate of poor compliance increases with the number of risk factors: 27.3% with no risk factor other than high BP, 32.2% with one, 37.2 with 2 and 51.5% with 3 or more (p < 0.0001). Multifactorial analyses confirm the independent effect of obesity, diabetes, smoking, and father's CV death before age 55 on patient compliance. CONCLUSION: patients with the highest CV risk are those who are the least compliant with their antihypertensive treatment. These results raise the question of the appropriateness of the prevention information given to the most at-risk patients.

[Effect of veinotonyl 75 on the capillary permeability test using technetium albumin in cyclic orthostatic edemas].

The purpose of this trial was to evaluate the clinical effect of Veinotonyl 75 in 42 women, 42 +/- 11 years old, with orthostatic cyclic edema. The main evaluation criterion was the result of a capillary filtration test with 99 mTc albumin which was performed at day 0 and at day 30 after treatment. At day 0, all the women had abnormal results ie an albumin retention > or = 8% (mean: 14.3% +/- 4.8%). At day 30, 75% of the patients were normalized in the Veinotonyl 75 group versus 38% in the placebo group (p < 0.02). The albumin retention in Veinotonyl 75 group was under the pathological level (mean: 5.2% +/- 4.8%) while it was over in the placebo group (mean: 9.4% +/- 7%). This study confirms the activity of Veinotonyl 75 on capillar protein hyperfiltration and on return venous circulation.
