[Ventilation problems in emphysema patients].

It may be difficult to decide if respiratory/ventilatory failure in patients with emphysema has resulted from a reversible process, such as pneumonia, acute on chronic bronchitis or cardiac decompensation. Therefore, we favour early institution of mechanical ventilation. Intermittent mandatory ventilation (IMV), continuous positive airway pressure (CPAP) and the new modes of pressure support (PS) help the patient to recover the strength of his musculature. Measures for bronchodilatation, antibiotics and optimal nutrition (including fat calories) allow to successfully wean most patients from the respirator. The prognosis for survival, however, is very uncertain and in general not very promising.

[Peridural morphine in the treatment of postoperative pain (author's transl)].

The action of peridural morphine (1.5, 3.0 and 5.0 mg) as compared to placebo was studied in the patients who underwent inguinal hernia repair or lower extremity surgery under peridural anaesthesia. Morphine produced a dose-dependent intensive and long lasting segmental analgesia which was statistically significantly superior to placebo at all dosages. This action was however accompanied by a high incidence of urine retention and vomiting. We did not find any respiratory or circulatory depression. Nevertheless, it is accentuated that under different clinical conditions this depression might be highly probable.

[Indications for plasma substitutes].

During severe hemorrhage the O-2-transport capacity of blood is reduced due to the loss of hemoglobin. This reduction is, however, fully compensated down to 25% hematocrit by augmentation of the minute volume consequent on the decrease of blood viscosity due to hemodilution, though this holds only on condition that myocardial contractibility and total blood volume are not impaired. The compensation of lost intravasal fluid is therefore prevailing importance, but the substitute need not be blood and may consist of a suitable colloid solution. Physiogel, a modified fluid gelatin, has proved to be excellent for this primary substitution of blood losses. The plasma derivatives should be reserved for special indications, particularly for treatment of oncotic deficit, and here highly concentrated (10-20% albumin) preparations are needed. Blood transfusions are indicated when the hemoglobin content falls below the critical limit of 25% hematocrit.

Correlated morphometric and biochemical studies on the liver cell. I. Morphometric model, stereologic methods, and normal morphometric data for rat liver.

The basic morphological properties of liver cells are defined in the form of a morphometric model to permit integrated quantitative characterization of functionally important parameters. Stereologic methods which allow efficient and reliable quantitative evaluation of sectioned liver tissue are presented. Material, obtained by a rigorous three-stage sampling procedure from five normal rat livers, is systematically subjected to this analysis at four levels of magnification. This yields quantitative data which are expressed as "densities," i.e. content per 1 ml of tissue, as "specific dimensions" related to 100 g body weight, and as absolute dimensions per average "mononuclear" hepatocyte. Base line data relating to the normal rat liver are presented for the entire spectrum of parameters. As examples, 1 ml of liver tissue contains 169 x 10(6) hepatocyte nuclei, some 90 x 10(6) nuclei of other cells, and 280 x 10(9) mitochondria. Hepatocyte cytoplasm accounts for 77% of liver volume, and the mitochondria for 18%. The surface area of endoplasmic reticulum membranes in 1 ml of liver tissue measures 11 m(2) of which are (2/3) of the rough form carrying some 2 x 10(13) ribosomes. The surface area of mitochondrial cristae in the unit volume is estimated at 6 m(2). The validity and applicability of the method are discussed, and the data are compared with available information from other studies.
