Cross-over comparison of the fixed combination of hydrochlorothiazide and triamterene and the free combination of furosemide and triamterene in the maintenance treatment of congestive heart failure.

The fixed combination of hydrochlorothiazide 25 mg and triamterene 50 mg and the free combination of furosemide 40 mg and triamterene 50 mg were compared as maintenance treatment in patients who were in a stable condition after an episode of congestive heart failure. All the patients were on digitalis. The trial was of the cross-over design. The treatment order was randomized at each centre. Each treatment lasted for 8 weeks. Of the 37 patients entering the study 30 were suitable for analysis. Both treatments were equally effective in maintaining bodyweight at the same level and in influencing symptoms. The free combination decreased the blood pressure slightly more than the fixed combination, especially the systolic pressure. One-third of the patients were hypertensive at the outset (diastolic pressure greater than 95 mm Hg) and in them the effects of the two treatments were not significantly different. The majority of the patients (80%) preferred treatment with the fixed combination because it caused slower and less frequent micturition.

Two cases of monoclonal immunoglobulins with antistreptolysin activity: clinical and laboratory study.

In two patients with a persisting, high antistreptolysin titre the antistreptolysin activity in both cases resided exclusively in a monoclonal IgG component in the serum. This component had all the characteristics of a true antibody. A history in both patients of arthritis with or without angina suggested that the monoclonal antibodies were reactive in origin, although definite proof was lacking. In one case there was a suggestion of incipient myeloma. Whenever an extremely high antistreptolysin titre persists after antibiotic treatment the possibility of paraproteinaemia should be considered.

Evaluation of the coagulation/fibrinolysis balance in patients with colorectal cancer.

The coagulation and fibrinolysis profile was evaluated in 40 patients with newly diagnosed untreated colorectal carcinoma (24 males, 16 females; 29 patients without and 11 patients with metastases). Fibrinogen, von Willebrand factor antigen, FVIII:C, thrombin-antithrombin III complex (TAT III), fibrin monomers (FM), plasminogen activator inhibitor-1 (PAI-1) and D-dimers were tested. None of the patients had clinical or laboratory evidence of serious hemorrhage or thrombosis. The results of global routine coagulation tests (aPTT, PT) were not significantly changed. Significant elevations were found for median fibrinogen, von Willebrand factor antigen, FVIII:C, TAT III and D-dimers, compared with a healthy reference group. These results confirm earlier reports of an enhanced level of both coagulation and fibrinolysis markers in carcinoma patients and might be helpful in trying to understand the impact of several relatively new sensitive coagulation and fibrinolysis parameters in colorectal cancer. Moreover the position of the coagulation/fibrinolysis balance might be an explanation for the elevated incidence of thrombotic events in patients with cancer.
