The sentinel lymph node biopsy in breast cancer.

OBJECTIVE: To evaluate the possibility and accuracy of this new diagnostic approach to the breast cancer disease in our centre. MATERIAL AND METHODS: Since March 1999, every patient presenting with a cT1-T2 N0 breast carcinoma was scheduled for a sentinel lymph node search. An injection of Tc-99 labelled nanocolloïd with a dose of 1 mCu was injected either intramammary or intradermally. The patients have been divided into two groups: in group I, they received their injection intramammarily the day before the operation; because of several failures in identifying the sentinel lymph node (SLN), the protocol was modified, the patients receiving their injection the day of operation, intradermally (group II). Once a lymphoscintigraphy done, the SLN was identified at operation using a detection probe, after the primary tumour had been removed. A routine axillary dissection was then performed to remove the rest of the lymph nodes. All the nodes were then checked routinely for metastatic cells. The SLN was also screened by semi-serial slides and by immuno-assay. RESULTS: From March 1999 till March 2001, sixty patients presented consecutively with a T1 or T2 biopsy proven breast carcinoma with no clinical lymph nodes. They were all scheduled for a sentinel lymph node search according to the protocol. Mean tumour size was 9.9 mm (ranging from 4 to 23 mm). Fourteen patients (group I) received their injection intramammarily but we failed to identify the sentinel node in five patients (35%). The remaining forty-two patients (group II) received their injection intradermally. Sentinel nodes were then identified in forty-three patients (93%). Positive SLN were discovered in eleven cases by routine examination (13 positive nodes among 104 harvested sentinel nodes, i.e. 13%). Micro metastases were discovered in three other SLN by immunohistology. In total, 605 lymph nodes were evaluated through the axillary dissection, representing a mean number of 10.08 lymph nodes per patient. For four patients, positive lymph node were discovered in the axillary dissection while SLN were negative (6.6% of false negative). CONCLUSIONS: During this learning curve period, it appears that the method for screening the SLN is reliable, since the figures encountered are similar to those of the literature. By adding a perioperative blue dye injection, it might be possible to reduce the percentage of false negative results. It is difficult to assess, at present, the impact SLN could have on survival.

[Gyrate atrophy of choroid and retina, and hyperornithinemia].

Gyrate atrophy of the choroid and retina is a rare degenerative disease, characterized biochemically by a marked increase in blood ornithine levels, due to deficiency of ornithine S-amino transferase. 4 men aged 35, 36, 48 and 62 years are described with different stages of myopia, night blindness and loss of peripheral vision, which progressed to tunnel vision and partial blindness. Onset of the disease was at ages 3, 10 and 15 years, respectively, while in the 4th patient there was delayed expression starting at about age 50. Most had posterior subcapsular cataracts, and the ocular fundus exhibited demarcated circular areas of chorioretinal degeneration. So far the only patients described in Israel have been of Iraqui origin. Our fourth patient originated from Istanbul, and he may represent a hitherto undescribed variant with a much delayed expression of the disease.

Single P100 visual evoked potential analyses in man.

Standards evoked potential averaging leads to a loss of information inherent in the moment to moment variability of the amplitude and latency of single evoked potentials. In order to extract this information, attempts were made to develop procedures which would allow recording of single visual evoked potentials in at least a selected group of subjects. In 11 out of 25 non-selected subjects, 43% to 86% of all stimuli (reversal checkerboard pattern elicited single visual evoked potentials (VEP) which were recognized as such by a group of independent observers. The mean amplitude and latency obtained by directly measuring the peak to peak amplitudes and peak latencies of these single VEPs (arithmetic averaging) were compared to those obtained following conventional time-locked averaging of the same data. During long-term continuous stimulation, the arithmetic averaged VEPs increased in amplitude to a steady state while the time-locked averages of the same sets of responses decreased in amplitude. This reduction was found to be closely related to the latency jitter. It may provide a better understanding of the phenomenon of habituation. This finding was confirmed in model single VEPs obtained by summing on-going pre-stimulus EEG activity with a time-locked average VEP stationary wave form. The variability of the true single VEPs was found to be less than the variability of the model single VEPs. The latency and amplitude parameters of the true single VEPs were strongly correlated with each other while those of the model single VEPs were not. These findings show that single VEPs have an inherent variability which may reflect brain processing.

Computer programs for calibrating control electrode voltages and for conditioning working electrodes for a neurochemical voltammetry system.

Two computer programs are described which are used to set voltages, calibrate the system, and cyclically condition electrodes electrically in an electrochemical system. This voltammetry system is normally used for measurement of monoamines in neurochemical studies. The system uses an Apple II, a potentiostat, and a DAS-5 interface between the two. The calibration program is used to establish the numerical values needed by the DAS-5 interface from the computer to yield desired potentiostat voltages, to calibrate the potentiostat, and to set output voltages of the system to be safe for the electrode and neural tissue before the potentiostat is turned on. Another program is used to condition electrodes as required in some cases to establish discriminably different oxidation potentials for different electroactive compounds (monoamines and ascorbate).
