Exaggerated urinary excretion of aquaporin-2 in the pathological state of impaired water excretion dependent upon arginine vasopressin.

The present study was undertaken to determine whether urinary excretion of aquaporin-2 (UAQP-2) is of value to diagnose the pathological state of water retention and hyponatremia. UAQP-2 under ad libitum water drinking was 429 fmol/mg creatinine in the patients with water retention, a value significantly greater than that of 153 fmol/mg creatinine in the normal subjects. An acute oral water load test (20 mL/kg BW) was performed in 7 normal subjects (22-25 yr old) and 10 patients with water retention and hyponatremia (55-75 yr old). The percent excretion of the water load was only 30% in the patient group compared with 70% in the control group (P < 0.01). In the control group, minimal urinary osmolality was as low as 131 mosmol/kg H2O, which was responsible for the decrease in plasma arginine vasopressin (AVP) levels after the reduction in plasma osmolality. In the patient group, minimal urinary osmolality was 320 mosmol/kg H2O, and free water clearance remained below 0.6 mL/min after the water load. This impaired water excretion was consistent with the nonsuppressible levels of plasma AVP despite hypoosmolality. The nadir of UAQP-2 was obtained at 60-90 min. The minimal UAQP-2 was reduced to 284 fmol/mg creatinine, a value significantly greater than that of 76 fmol/mg creatinine in the control group. Similar results were obtained in the 6 patients with hypopituitarism, who had impaired water excretion and marked hyponatremia. Water excretion was totally normalized after the replacement of hydrocortisone (excretion of water load, 31% vs. 102%; P < 0.01). Hydrocortisone replacement also significantly reduced the minimal UAQP-2 from 225 to 49 fmol/mg creatinine after the acute oral water load, a value comparable to that in the control subjects. These results indicate that UAQP-2 is a potent marker to diagnose the pathological state of impaired water excretion and hyponatremia, dependent upon AVP, in patients with water retention and hypopituitarism.

[The effects of aging on the relationship between changes in body weight, serum lipid levels, and blood pressure].

The effects of aging on the relationships between changes in body weight, serum lipid levels (total cholesterol, HDL-cholesterol, triglyceride, beta-lipoprotein), and blood pressure (systolic pressure, diastolic pressure) were studied. The subjects were 17,689 Japanese (aged 19 to 88 years) who had received annual examinations at health centers for two consecutive years. After the date were adjusted for sex, body mass index, and serum lipid levels at baseline, changes in serum lipid levels and blood pressure per kilogram change in body weight were estimated for 3 age groups (under 45, 45 to 64, and 65 or older). Positive relationship between changes in body weight and blood pressure were noted for all 3 age groups. Although total cholesterol, triglyceride, and beta-lipoprotein levels all decreased with weight loss in the under-65 groups, total cholesterol in the 65-and-over group did not change significantly. The increases in total cholesterol, triglyceride, and beta-lipoprotein levels were associated with weight gain in the under-65 groups, but not in the 65-and-over group. HDL-cholesterol levels in all age groups decreased significantly with weight gain. These results suggest that aging affects the relationship between changes in body weight and serum lipid levels but not that between changes in body weight and blood pressure.

Identification of anterior uveal tumor border by transscleral transillumination and an ophthalmic endoscope.

PURPOSE: To report a method for identifying the borders of anterior uveal tumors in deeply pigmented eyes. METHOD: The borders of the anterior uveal tumors were marked by transscleral transillumination with a fiberoptic light and observed from the vitreous cavity using an ophthalmic endoscope. RESULTS: The ophthalmic endoscope disclosed a bright orange spot by transscleral transillumination of a normal uvea in deeply pigmented eyes. Two anterior uveal melanomas and one adenocarcinoma of the nonpigmented ciliary epithelium did not transmit light through the tumors. CONCLUSION: With an ophthalmic endoscope and transscleral transillumination, the borders of ciliary body tumors can be identified and clearly demarcated, even in deeply pigmented eyes.

[A case report of coronary artery bypass grafting (CABG) and graft replacement of the ascending aorta for coronary artery disease associated with dissection of the ascending aorta].

A 64-year-old woman waiting for CABG for triple coronary artery disease admitted to our hospital due to chest oppression. One week after the admission, pericardiotomy was performed and blood in the pericardial effusion was found. Emergent chest CT scan revealed dissection of the ascending aorta (DeBakey II) with occluded false lumen. Because her condition was stable, concomitant operation of graft replacement of the ascending aorta and triple coronary artery bypass grafting were performed 3 months after the medical therapy. Her postoperative course was uneventful and the patient remains asymptomatic.

[A case of quadricuspid aortic valve associated with aortic steno-insufficiency].

A case of quadricuspid aortic valve associated with aortic steno-insufficiency was described. A 60-year-old man who had been shown to be suffering from heart murmur was admitted to our hospital. Both aortogram and echocardiogram showed a significant aortic insufficiency with mild stenosis. The aortic valve revealed 4 cusps consisting of 3 equal cusps and a smaller cusp and it had a poor coaptation and mild calcification at each commissure. The coronary orifices were normally located. Aortic valve replacement was carried out with a 25 mm St. Jude Medical valve. Histological findings showed edematous and fibrous changes. His postoperative clinical course was uneventful. Quadricuspid aortic valve is rare and the literatures were reviewed with 33 reported cases in Japan.

Metastasizing uterine leiomyoma. A case with cardiac and pulmonary metastasis.

A case of histologically benign cardiac and pulmonary metastases from a uterine leiomyoma in a 45-year-old woman is reported. The solitary cardiac tumor consisted of five lobules with a stalk attached to the anterior papillary muscle of the right ventricle, which occupied the right ventricular cavity and almost completely obstructed the pulmonary main trunk in the systolic cycle. Multiple small nodules were found throughout both lungs. The cardiac tumor was resected at open heart surgery and open lung biopsy of the pulmonary lesion was simultaneously performed. Both of them were histologically identical to the apparently histologically benign uterine leiomyoma which had been resected five years previously. Based on the clinical and laboratory findings, it is postulated that antegrade metastases via the venous system resulted in the cardiac metastasis from the uterine leiomyoma and the secondary pulmonary metastasis from the cardiac tumor. In contrast to the light microscopic findings, ultrastructural examination suggested the possibility of malignancy associated with the presence of immature smooth muscle cells. This is the first reported case of a so-called benign metastasizing leiomyoma occurring in the heart.

Intracardiac ectopic thyroid mass.

Intracardiac ectopic thyroid mass is an extremely rare occurrence. We describe a case of this rare disorder. The mass was clearly visualized by echocardiography, MRI, and cineangiography. The thyroid mass was excised successfully, and 50 months later the patient was well.

[Usefulness of the prick test for anaphylactoid reaction in intravenous fluorescein administration].

We examined whether a prick test was a valuable method in comparison with an intradermal skin test for predicting an anaphylactoid reaction to intravenous injection of fluorescein solution. Fifteen hundred cases were tested. The number (rate) of positive reactions to the prick test with 10% and 1.0% fluorescein solution was 2 (0.1%) and 0 (0.0%), respectively. In contrast, positive reaction to the intradermal skin tests with 10% and 0.1% fluorescein solution was observed in 686 cases (45.7%) and 13 cases (0.9%), respectively. Fluorescein angiography (FAG) was performed in 1,499 of the 1,500 cases. Adverse reactions such as nausea, cough, cold sweat, urticaria, and shock were noted in 85 cases (5.7%). Typical anaphylactoid shock occurred in one case (0.07%), which was one of the two cases positive to the prick test with 10% fluorescein. In the other positive prick test case, FAG was cancelled because of the high probability of anaphylactoid shock. The results suggest that a prick test with 10% fluorescein solution can markedly cut down the false positive reactions and can be a useful test for the prospective diagnosis of anaphylactoid reactions to intravenous fluorescein administration.

[A case of isolated congenital mitral insufficiency].

A 1-year and 8-month-old girl admitted to our hospital because of wheeze and dyspnea. Echocardiogram and cardiac catheterization confirmed isolated congenital mitral insufficiency with pulmonary hypertension. She was treated with reconstructive surgery consisting of suture of clefted anterior mitral leaflet and annuloplasty. Her postoperative course was uneventual and mitral regurgitation was remarkably improved.

[The problem of 19 mm St. Jude Medical valve prosthesis in the small aortic annuls--19 mm St. Jude Medical valve vs 23 mm St. Jude Medical valve].

We compared the results of 19 mm SJM aortic valve replacements with those of 23 mm SJM aortic valve replacements conducted the same period. [Material & Method] The subjects were 21 patients who underwent valve replacement with a 19 mm SJM prosthesis (the SJM19A group). The age ranged among 16 and 65-year-old, mean of 51. The body surface area was 1.24 to 1.76 m2, mean of 1.48 m2. The post-operative follow-up period was 156 months at maximum and 44 months on average. We compared the SJM19A group with the SJM23A group by chest X-ray and echocardiography. [Results] Two patients in the SJM19A group died soon after surgery. Of the other patient, 19 were categorized NYHA I and one in NYHA II classification in their late phase. Arrhythmia of Lown IVa developed in one patient. The cardio-thoracic ratio decreased from preoperative 60% in the late phase after surgery (p < 0.002) in the SJM19A group, although there was no significant difference with that in the SJM23A group. Echocardiographic improvement in left ventricular hypertrophy was considerable between before and late after surgery in the SJM19A group, while it was not significantly different between the SJM19A and SJM23A group. The mean value of aorta-left ventricle pressure difference in the late stage was 32 mmHg in the SJM19A group and significantly different from the value in the SJM23A group (p < 0.001). This pressure difference tended to be greater as the follow-up period was progressed, while the percentage decrease in the myocardial mass of the left ventricle tended to decreased with longer follow-up period. This data suggests that an increasing level of aorta-left ventricle pressure difference should raise little problem in the mid-term late stage after surgery but possibly cause a serious problem in the long term. Clinical observation should be continued over a long period of time after surgery.

[Successful scartectomy and cryoablation for ventricular tachycardia occurring late after correction of tetralogy of Fallot].

A 26-year-old man had a total repair of tetralogy of Fallot at 1 year of age, and had redo surgery for restenosis of the right ventricular outflow tract and small residual VSD at 11 years of age. After the second operation, AV block developed and an endocardial pacemaker system was implanted. For the last 3 years, he had mild febrile episodes, cough, occasional hemoptysis and paroxysmal ventricular tachycardia. Because of his refractory tachycardia and suspected infective endocarditis, he was admitted for further study. Blood culture revealed Peptostreptococcus, echocardiogram showed vegetation around intravenous pacing lead, and electrophysiological study demonstrated delayed potential on the left side of the right ventricular outflow tract. He underwent scartectomy and cryoablation of the focus of the tachycardia which was reconfirmed by epicardial and endocardial mapping during the operation, which involved removal of the endocardial lead and new outflow tract patch repair. His postoperative course was uneventful without any antiarrhythmic drugs. Pathological examination of the scar showed myocardial fibrosis and replacement by fatty tissue which was different from the pathological characters of the arrythmogenic right ventricular dysplasia.

Effects of ventilation and pleural effusion on measurements of airway thermal volume and blood flow in dog lungs.

We studied the effects of ventilation and pleural effusion on measurements of airway thermal volume (ATV) and pulmonary blood flow (PBF) by using the airway gas thermometry method of V. B. Serikov, M. S. Rumm, K. Kambara, M. I. Bootomo, A. R. Osmack, and N. C. Staub (J. Appl. Physiol. 72: 944-953, 1992) in 39 anesthetized dogs with or without lung edema or pleural effusion. To examine the differential effects of increased-pressure and increased-permeability lung edema on accuracy and sensitivity of ATV and PBF, two models of lung edema were induced by intravenous infusion of a Dextran 70 solution and alloxan monohydrate, respectively. Dogs were hyperventilated for 3 min by using a wide range of minute ventilation (VE) to produce two steady-state conditions of airway temperature. Higher levels of VE increased an estimated amount of ATV. The ATV produced by hyperventilation at VE values of 559, 158, and 72 ml.min-1.kg-1 was consistent with the gravimetric total lung mass, the blood-free wet lung weight, and the extravascular lung water volume, respectively. The coefficient of lung thermal conductivity, a practical index of the rate of heat conduction through tissue from lung vessels, was related to the ratio of the decrease in expired air temperature to VE, and estimated PBF was consistent with the thermodilution cardiac output. Pleural effusion had little effect on measurements of ATV and PBF. However, ATV and PBF showed increased variation in dogs with dextran-induced lung edema.

[Mitral valve replacement for congenital parachute mitral valve].

A one-year-old boy was admitted with refractory congestive biventricular heart failure for medical treatment. On echocardiogram and cardiac catheterization revealed severe mitral stenosis from parachute deformity with pulmonary hypertension. During the operation, a single round orifice of 7 mm in diameter was detected in the mitral valve and adhered chordae were attached to a large single papillary muscle which was located at the posteromedial portion of the left ventricle. An isolated muscle band which was not attached to the mitral valve was observed at the anterolateral wall of the left ventricle. The mitral valve was replaced with 16 mm Carbo-Medicus prosthesis. Postoperative catheterization revealed residual pulmonary hypertension which was responsive to Imidarine infusion. He was discharged from the hospital without any sequelae, and has been on regimen including anticoaglant and vasodilator.

[Chronic left ventricular epicardial pacing by the perforated endocardial electrode through the cardiac vein].

We report a rare complication of endocardial pacing electrode implantation. A 64-year-old man, who was implanted transvenous pacemaker system by the other hospital one month ago, visited our outpatient clinic with the complaint of diaphragmatic twitching. Left ventricular pacing was highly suspected because of right bandle branch block pattern by 12 leads ECG. Pericardial effusion was observed by echocardiography, and angiography revealed the pacing electrode coursing through the coronary sinus and perforated the cardiac vein towards the posterior wall of the left ventricle. A new generator and a pacing electrode were implanted on the other side. Pericardial drainage was not performed because hemodynamics was stable. The patient discharged hospital on 18th postoperative day without any complication.

[Comparison of the long-term results of intra or extracardiac implanted bioprosthetic valves].

The long-term results following intra or extracardiac implanted bioprosthetic valves were compared. Forty-one patients, whose average age at the operation was 43.3 +/- 10.7 (mean +/- SD) year-old, were implanted 33 Hancock and 11 Carpentier-Edwards porcine valves, 35 in mitral, 6 in aortic and 3 in tricuspid position. Twenty-five children, whose average age was 6.0 +/- 3.5 year-old (p < 0.01) at the operation, underwent right ventricular-pulmonary artery conduit repair with 16 Hancock, 7 Carpentier-Edwards porcine valves and 2 Ionescu-Shiley pericardial valves. There were only one late death and one operative death in the patients implanted intracardiac bioprosthetic valves. Seven intracardiac and 12 extracardiac implanted bioprosthetic valves were removed at the reoperation for prosthetic valve dysfunction. The probabilities free from reoperation after 7 and 9 years of follow-up was 91% and 82%, respectively, with intracardiac implanted valves, on the other hand 51% (p < 0.05) and 8.6% (p < 0.05), respectively, with extracardiac implanted valves. Valve dysfunction in extracardiac implantation was mainly related to the stenosis due to fusion of comissure and/or calcification. Regurgitation due to tear and/or perforation was mainly associated with intracardiac implanted bioprosthetic valves.

Methicillin-resistant Staphylococcus aureus endocarditis following patch closure of a ventricular septal defect: report of a case.

We report herein the case of a 5-month-old infant who, after developing methicillin-resistant Staphylococcus aureus (MRSA) endocarditis following patch closure of a ventricular septal defect (VSD), was successfully treated by replacement of the Dacron patch with an autogenous pericardial patch. Initially, a large perimembranous VSD was repaired with a Dacron patch and after an uneventful recovery of 1 week, he began to spike intermittent fevers from 38 degrees C to 39 degrees C. Two blood cultures grew MRSA and a two-dimensional echocardiogram performed 16 days after surgery showed an irregular mass attached to the right ventricular aspect of the Dacron patch. At reoperation, a large vegetation attached to the Dacron patch was confirmed, but there was no patch dehiscence. Following removal of the patch, the VSD was repaired with an autogenous pericardial patch, soon after which the fever rapidly subsided. Imipenem, 125 mg every 6 h, and fosfomycin, 300 mg every 6 h, were administered for a total of 24 days after reoperation. The child remains well 12 months after his second operation.

New modification of the Damus-Kaye-Stansel operation.

We report a successful modification of the Damus-Kaye-Stansel operation for transposition of the great arteries when the coronary arteries are unsuitable for transfer. The procedure includes creation of a neoaorta with end-to-end anastomosis of the proximal pulmonary artery to the distal ascending aorta and creation of an aortopulmonary window between the proximal great arteries. A valved conduit is interposed between the right ventricle and the distal pulmonary artery.

[A case of uveitis due to gnathostoma migration into the vitreous cavity].

We report a 26 year-old male patient who had floaters and hyperemia in his left eye following uveitis due to gnathostoma that had migrated into the vitreous cavity. Severe iridocyclitis and mild opacity of the vitreous body were observed, together with whitish-yellow subretinal tracks accompanied by dot and blot hemorrhages in the fundus. Slit lamp microscopic examination revealed a worm which writhed in the vitreous cavity. We performed vitrectomy to remove the worm from the anterior vitreous uneventfully, followed by prompt subsidence of the inflammatory signs. The worm was identified as a third instar larva of Gnathostoma doloresi. Eosinophilia and creeping eruption did not appear throughout the follow-up period. The patient was accustomed to eat live roaches and whitebait, as well as sliced raw beef liver.
