Early postsurgical bacterial contamination of the airways: a study on 28 open-heart patients.

One pre- and two postoperative cultures of tracheo-bronchial secretions were obtained from 28 cardiac patients, subjected to open-heart surgery. Four patients received preoperative antibiotics, and all but one received postoperative prophylactic antibiotics. Preoperatively, only one patient had potential pathogens; after surgery (mean intubation time 4.2 h), four patients (14.3%) had organisms; and after 19 h of intubation, 28% of the patients had potential pathogens in their tracheo-bronchial secretions. Only three of the seven organisms recovered from the last sample were clearly sensitive to the antibiotics given prophylactically; and two of these organisms were Group A beta-haemolytic streptococci. The early presence of organisms in the airways after intubation, the high incidence of colonization, and the ineffectiveness of prophylactic antibiotics in preventing this contamination are pointed out. The factors that may possibly influence colonization of airways among these patients are commented on.

The influence of carboxyhaemoglobin on the oxygen-binding capacity of blood. A comparison of manometric (Van Slyke) and optical (Co-Oximeter) measurements.

Oxygen content measurements determined optically by the Co-Oximeter correlated closely with those obtained using the manometric technique of Van Slyke. In those samples with a high content of carbon monoxide, the agreement between the two methods could be improved, by taking into consideration the concentration of carboxyhaemoglobin (COHb), since this plays no part in oxygen transport. The calculated values of the oxygen-binding capacity varied widely, with mean values less than the theoretical figure of 1.39 ml O2/g Hb. This difference was greater in those samples with a higher content of COHb. When the amount of haemoglobin combined with COHb was taken into consideration the mean corrected values approximated to the theoretical value of 1.39, thus supporting this as the mean value for the oxygen-binding capacity of haemoglobin, although the large individual variability persisted.

Mechanical ventilators as vehicles of infection.

Twenty-five patients from the intensive care unit who had been subjected to tracheostomy and require mechanical respiration were studied. Data are presented concerning the bacteriological controls performed on the tracheal exudates, various parts of the ventilators (Engström Model 200 and Bennett PR-2), and the Wright spirometer. The germs found were mainly pseudomonas, enterobacteriaceae germs and bacteria from the acinetobacter group. There was a high incidence of infection in the tracheostomies of these patients. We found a cause and effect relationship between the contaminating bacteria isolated from the cannula and the bacteria isolated from the tracheal exudates in the case of pseudomonas aeruginosa. However, this was not found to be the case where enterobacteriaceae were concerned. Acinetobacter calcoaceticus were isolated almost exclusively in the ventilators. The study calls attention to the need for rigorous disinfection of ventilators to prevent hospital cross infections.

[Beneficial effect of morphine on the threshold of electrical fibrillation of the ventricle. Experimental study].

The action of 2-3 mg/kg of morphine on the threshold of electrical fibrillation of the ventricle (TVF) in anesthetized dogs has been studied. The basal TVF was 10.5 +/- 4.9 mA, increased to 20.2 +/- 7.7 mA (P 0.005) with the administration of this drug. Comparatively, the effect of morphine on the TVF is similar to that of propranolol and bretilium and more effective than procainamide. The mechanism by which it acts is not well known. Perhaps it is due to a homogenization of refractory periods in the ventricle as well as a possible morphine-catacholamine interaction. Morphine in dosis of 2-3 mg/kg could occupy a place among the antiarrhythmic drugs of choice with antifibrillatory action, with important usefulness in the field of anestesiology of cardiopaths.
