Capillary rarefaction characteristic of the skeletal muscle of hypertensive patients.

There is evidence that the rarefaction of the capillary bed is typical for the skeletal muscle of spontaneously hypertensive rats. We were therefore interested to learn whether there is also a rarefaction in skeletal muscle of human hypertensives. The number of capillaries was morphometrically analysed and counted in the quadriceps and the pectoralis major muscles of human normotensives (n = 12) and hypertensives (n = 15). The clinical diagnosis and certain pathological criteria, such as blood pressure (with or without antihypertensive therapy), heart weight, left ventricular wall thickness, the state of kidney arterioles and brain, and heart vessels, were used to classify the patients into two groups. The dissected tissue samples were prepared according to the GMA method and the capillary numbers per area were counted using light microscopy (250 x). The quadriceps muscle had a capillary density (per 2.5 mm2) of 442 +/- 51 in normotensives and 277 +/- 41 in hypertensive patients; in the pectoralis major muscle we counted 477 +/- 30 in controls and 232 +/- 28 in hypertensives. The rarefaction in the quadriceps muscle ranged by about 37%, in the pectoralis major muscle by about 51%. It is suggested that the reduction of the capillary surface area caused by the capillary rarefaction reduces the transcapillary fluid exchange and in that way prevents an overperfusion of the terminal vascular bed.

[The post-burn process--a determinant in local surgical treatment of deep burns].

Afterburning has been studied in the animal experiment by use of intravital dye indicators and by measuring partial oxygen pressure. After third degree burn the oxygen conduction of the damaged skin increased. The afterburning was seen to be finished on the 5th day after the burn trauma. The histological changes in the depth of the afterburn area reaches to the subcutis. The extend of the afterburn area was not related to the applied temperature or the duration of the experimental burn injury.

[Experimental microbiological criteria for the banking of full-thickness skin].

Experimental studies were conducted into the biological durability of homologous full skin grafts. Colony counts for that purpose were determined in deep-frozen specimens in weekly intervals. Specimens were stored at various temperatures (+4 degrees C, -20 degrees C, -70 degrees C, -80 degrees C), with and without preceding quick freezing. Storage at -70 degrees C with protracted freezing gradient of 1 degree C/minute proved to the most favourable variant with due consideration of microbiological criteria. The physiological resident flora of skin grafts after thawing was almost unchanged under these conditions.

[Significance of duodenogastric reflux in the pathogenesis of primary stomach cancer].

Duodenogastric reflux has often been discussed as a causative factor in the pathogenesis of gastric carcinoma. The author's clinical examinations of 135 patients with primary cancer of the stomach revealed slight increase in duodenogastric reflux. Yet, apart from these findings, no correlations were found to exist between reflux incidence and tumour localisation nor between histological tumour type and tumour size.

[Mortality and morbidity following surgical therapy of perforated gastroduodenal ulcers].

A retrospective study was conducted into 136 patients who had received surgical treatment for perforated gastroduodenal ulcers, with the view to establishing postoperative lethality and morbidity (comparing simple suturing with definitive ulcer surgery). Factors of relevance to prognosis were taken into consideration, including age of patients, number of concomitant diseases, time from perforation to surgery, presence of peritonitis, ulcer location, and length of ulcer history.

[Mixed freeze preserved heterologous/autologous full-thickness transplants following 3d degree burns in animal experiments].

On five minipigs third-degree burns were applied. The surgical skin coverage was established after escharectomie by freeze-preserved heterological/autological intermingled full-thickness skin grafts. The freeze-preservation of the Makaka-skin (full-thickness skin) was achieved in a commercially available freezer at -27 degrees C. We could not observe a accelerated or even complete re-epithelialization as it is described in the Chinese papers. The burnwunds healt in form of scare areas covering about half of the previously burned area.

[Experimental determination of the time-dependent extent of after-burning with reference to possibilities of the plastic surgery reconstruction of 3d degree burns].

The present experiments try to answer the question as to the time-dependent extent of the after-burning process after full-thickness burn (third degree). For an early plastic surgical treatment it was of interest to determine the most early time of escharotomy. The time-dependent spreading of the after-burning area reached its maximum five days after the burn injury. The after-burning area was marked by intravenous injections of Patentblau which caused distinct intravital colouring. Subsequently no further progress could be observed. In the present experiments we suggest this time as the earliest time for plastic covering in case it would be dependent upon the end of the after-burning process.

[Application of mixed homologous/autologous full-thickness skin transplants preserved by freezing following 3d degree burns in animal experiments].

In 5 white piglets 3rd-degree burns were applied. After escharectomy the wound-dressing was done by intermingled homo/autografts. In variation of the Chinese method reported, in our experiments the homografts consisted of full-thickness skin preserved by freezing (-27 degrees C). In all piglets this procedure caused a perfect epithelialization and brought good cosmetic and clinical results.
