The multimeric pattern of von Willebrand's factor (vWF) can be demonstrated also in the severe form of the disease.

The multimeric analysis was carried out on the plasma of 18 patients with severe von Willebrand's disease (vWD) using two different types of agarose: Seakem HGT(P) and Seaplaque LGT. No pattern was found in any of the patients using Seakem HGT(P). On the contrary, by Seaplaque LGT, a multimeric pattern was found in four patients belonging to three different families, indicating that it is possible to identify and characterize variable multimeric patterns also in type III vWD.

Acquired selective factor X deficiency in acute nonlymphocytic leukemia.

A case of acute nonlymphocytic leukemia (ANLL) and selective acquired FX deficiency at presentation is reported. The deficiency was not corrected by the infusion of fresh frozen plasma, but the patient's plasma had no neutralizing activity in vitro. The cause of the deficiency is unknown.

Recurrent mutations and three novel rearrangements in the factor VIII gene of hemophilia A patients of Italian descent.

Hemophilia A (HA), a common inherited bleeding disorder in humans, is due to the deficiency or absence of the factor VIII (FVIII) activity. The cloning of the FVIII gene has made molecular probes available for the characterization of the basic defect in this disease. In this study we describe six different mutations in the FVIII gene detected by DNA analysis of 100 HA patients of Italian descent. In two of them, with a severe clinical picture, we identified two novel deletions, one in the middle of the FVIII gene from exons 7 to 22 and the other encompassing the entire factor VIII gene. Both of these patients produced antibodies to factor VIII. In a patient with mild HA we detected a duplication of exon 13, which is a rearrangement not yet described within the FVIII gene. A possible explanation for the mild phenotype in this patient is that the molecular defect results in the production of an unstable FVIII protein with residual 10% FVIII activity. Screening by Taq I restriction endonuclease detected three mutations that were further characterized by direct sequencing on amplified DNA: a C-T substitution at codon 1960, in exon 18, converting the codon for arginine to a non-sense codon; and a G-A substitution at codon 2228 and 2326, in exons 24 and 26 respectively, resulting in the substitution of glutamine for arginine. All three of these mutations have been previously described. The non-sense mutation and the codon 2228 G-A mutation was found in patients with severe HA, while the codon 2326 G-A mutation was associated with a quite severe condition. These results confirm that the molecular bases of HA are very heterogeneous and provide further evidence that recurrent mutations are not uncommon in this system.

Prothrombin-antibody coexistent with lupus anticoagulant (LA): clinical study and immunochemical characterization.

The patient is a 23 y.o. man with acute nephritis and bleeding at presentation. Laboratory data consistent with the diagnosis of systemic lupus erythematosus. A lupus anticoagulant was found: tissue thromboplastin inhibition test (TTIT) ratio 3.4; diluted Russell viper venom (DRVV) ratio 2.6. Hypoprothrombinemia (FII:C less than 1%; FIIR:Ag 5%) was present; prothrombin survival time (FII concentrate infusion 60 U/kg): t1/2 approximately to 9 hours. A prothrombin antibody was identified: it is not neutralizing but forms an immunecomplex with prothrombin. The antibody was characterized as IgG2, IgA, k, lambda. The prothrombin survival time indicates that the hypoprothrombinemia is due to the clearance of the prothrombin-antiprothrombin complex in vivo.

Emergency treatment with recombinant tissue plasminogen activator of pulmonary embolism in a pregnant woman with antithrombin III deficiency.

Thromboembolic complications during pregnancy are frequent in patients with congenital antithrombin III deficiency. We report on a 29-year-old patient with congenital antithrombin III deficiency and severe pulmonary embolism treated with recombinant tissue plasminogen activator. The diagnosis of antithrombin deficiency is retrospective. This case indicates that the risk of thrombolytic therapy in this clinical setting might have been overemphasized.

Evaluation of factor VIII pharmacokinetics in hemophilia-A subjects undergoing surgery and description of a nomogram for dosing calculations.

The pharmacokinetics of factor VIII were studied in a series of 20 hemophilia-A patients undergoing surgery. Regardless of the type of operation, elimination of factor VIII was shown to be increased only in ten cases (50%) during the post-operative period. In this subgroup of patients, factor VIII half-life, measured immediately after surgery, was considerably shorter (mean = 9.6 hr, n = 10) than that determined in the same individual during the late operative period (mean = 17.8 hr, n = 10). These findings indicate that identification of patients with increased postoperative consumption of factor VIII can be of value in reducing the risk of hemorrhage in these subjects and in exposing other subjects with no postoperative increase in factor VIII clearance to less of the deficient factor. Data from 20 subjects were analyzed to construct a nomogram allowing individualized prediction of factor VIII dosing requirements. The nomogram, which is based on the "single point after a single dose" method, uses a value of factor VIII concentration measured at 10 hr after preoperative loading dose to predict the regimen producing the desired average steady-state concentration of factor VIII (30, 60, or 90 units/dl).

Immunogenicity of a recombinant hepatitis B vaccine in hemophiliacs.

Yeast-recombinant vaccines against hepatitis B virus (HBV) are now available, but there is no information about whether or not they are immunogenic in patients with hemophilia and other congenital bleeding disorders. Twenty micrograms of a recombinant vaccine expressing the adw serotype of the hepatitis B surface antigen (HBsAg) were given to 41 patients negative for HBV markers and again after 1 and 6 months. Ten percent of the vaccinees had anti-hepatitis B surface antibody (anti-HBs) responses, with titers of 10 mIU/ml or more, 1 month after the first dose of vaccine. The percentage of anti-HBs-positive patients increased to 54% after the second dose and to 98% after the third dose, with only one non-responder. Hence, the recombinant vaccine was immunogenic, with percentages of seroconversion and anti-HBs titers similar to those achieved with plasma-derived vaccines.

Severe von Willebrand's disease: SDS agarose multimer analysis.

In severe von Willebrand's disease (vWD), the multimeric analysis of von Willebrand factor (vWF) has been hindered by the low content of the protein although different patterns have been observed by radiocrossed immunoelectrophoresis (RCIE). Cryoprecipitate and vWF concentrate were obtained from the plasma of a severe vWD patient, that on RCIE had only the more anodic forms. The sodium dodecylsulfate (SDS) agarose analysis evidences the presence of the main bands, a relative increase of the smaller forms and a lesser definition of some minor bands. These findings suggest the possibility of structural abnormalities in severe vWD. The study of cryoprecipitates could be a means to evidence the presence of multimers and the extent of the eventual pattern variability.

Local hemostasis after tooth extraction in patients with abnormal hemostatic function. Use of human fibrinogen concentrate.

Tooth extraction was carried out in 405 patients with various hemostatic disorders due to thrombocytopenia, chronic liver disease, hemophilia A and B, von Willebrand's disease and oral anticoagulants. Human fibrinogen concentrate was used as local hemostatic agent. Prophylactic replacement therapy (platelets or plasma concentrates) and antifibrinolytic agents were not administered. Oral anticoagulants were not discontinued. Minor postextraction bleeding occurred only in severe hemophilia A and occasionally in the oral anticoagulant group.

Acquired factor VIII:C inhibitor (IgG) and positive direct Coombs' test (IgM) in a patient with lung carcinoma. Clinical course and immunochemical studies.

UNLABELLED: A 73-year-old man with lung squamous-cell carcinoma simultaneously developed a factor VIII:C inhibitor and a transient positive direct Coombs' test. The factor VIII:C inhibitor was characterized as a mixture of IgG1, IgG2, IgG4 with kappa and lambda light chains. The Coombs' test was IgM-specific. TREATMENT: blood transfusions (packed red cells), cyclophosphamide and prednisone. Bleeding subsided without factor VIII-replacement therapy. Before death, in spite of the tumour-mass progression, factor VIII:C was normal.

[Carcinoma of the nasal sub-septum (author's transl)].

On the basis of 13 cases, the authors describe the anatomical characteristics and course of primary carcinomas of the nasal sub-septum. This is a transitional form between nasal carcinomas and those of the nasal fossa. The cases described were treated either by conventional radiotherapy (200 Kv) or by electron therapy. The successful results obtained (12/13) confirmed the value of irradiation in this special site, without the risk of radionecrosis. The technique should conform with precise rule, as in all tumours of the face or nasal pyramid and the quality of the results obtained is compared favourable with those of all other methods.
