[Compensatory mechanisms of haemolysis after surgery with the heart-lung-maschine (author's transl)].

In 20 patients, suffering from various diseases, the extent of haemolysis and the correlation between the rate of haemolysis and various mechanisms of erythrocyte damage in the course of open heart surgery was studied. We investigated haemotological parameters, electrolytes, lactate dehydrogenase before, and 1, 3, 5, and 7 days after operation. Besides free haemoglobin, haptoglobin and haemopexin were determined 5 and 15 minutes after starting perfusior with the heart-lung machine and after finishing. The best index of haemolysis was free haemoglobin in the serum. With increasing duration of perfusion the rate of haemolysis increased. Compensating mechanisms were postoperative increase of haptoglobin and haemopexin.

[Hemodynamic effects of aldosterone antagonists in patients with mitral stenosis (author's transl)].

In 12 patients with mitral stenosis of a severity that surgery was required, some hemodynamic parameters after intravenous injection of 400 mg Kaliumcanrenoat were studied. The hemodynamic changes were noted after 15 min and reached maximal values after 60 min. The increased values of pulmonary wedge, pulmonary artery and right atrial pressures were significantly reduced and the cardiac index and stroke volume index augmented, with unchanged systemic arterial pressure. Thus, a remarkable relief from pulmonary congestion followed, accompanied by an increase of the left-ventricular working index.--These results encourage the use of aldosterone antagonists in certain cases suffering from severe mitral stenosis.

[The new calcium antagonist Ro 11-1781 in the treatment of ventricular tachycardia due to atrial fibrillation].

In 9 patients with tachyarrhythmia and atrial fibrillation the effect of the i.v. application of the calcium antagonist Ro 11-1781 on the av-conduction was investigated. All patients received 1 mg/kg during 2-3 minutes. A significant decrease of the ventricular rate at the end of the injection period was noted. The maximal decrease of the av-conduction of 10-32 % of the initial ventricular rate was observed 5-10 min after the injection. During 60 min the initial heart rate before application of the drug was not yet reached again. The registrations prove the inihibiting effect of Ro 11-1781 on av-conduction in atrial fibrillation, which was sufficient for therapeutic use in our cases concerning the reduction of heart rate and duration of the effect. No serious side effects were observed.

[Quantitative analysis of the peripheral arterial blood pressure curve in patients with peripheral obstructive cardiomyopathy after premature beats (author's transl)].

Changes of the brachial artery pressure curves after mechanically released extrasystoles were investigated in 20 patients suffering from hypertrophic obstructive cardiomyopathy. Diagnosis was confirmed by heart catheterisation and laevocardiography. At the average, the postextrasystolic beat showed a more pronounced reduction of the pressure at the systolic dip and the second systolic peak (19.7 +/- 14.2 and 17.2 +/- 13.8) than at the first systolic peak and the incisure due to the aortic valve closure (8.4 +/- 10.5 and 12.8 +/- 9.9). The diastolic pressure decreased of about 11 +/- 4.9%.--The ejection time of the postextrasystolic beat increased of about 0.029 sec (at rest 0.335 +/- 0.047 sec). Compared to the presystolic beats these changes were significant.

[Haptoglobin in the course of surgery with the heart lung machine (author's transl)].

In 20 patients, who underwent open heart surgery, the results of quantitavely and semi-quantitavely determined haptoglobin (hp) were evaluated. A rapid method (Rapi tex-Hp-Test, Behring-Werke) was used. Five minutes after the heart lung machine was cut off hp decreased slightly, and increased about 200% at the 4th day after operation, compared to the preoperative value. These changes were less pronounced in patients with prosthetic heart values. Blood transfusions did not influence the results. There is a significant correlation between the results of hp determined quantitatively and semi-quantitatively.

[The significance of coronary artery calcification seen by roentgenoscopy (author's transl)].

The detection of coronary artery calcification by roentgenoscopy is an important hint at possible ischemic heart disease. By the advent of image intensification and magnification, this examination can easily be performed. If no calcifications are visible, an ischemic heart disease must not be excluded. The presence of calcification in the coronary arteries however strongly favours the diagnosis of arteriosclerotic heart disease. The roentgenoscopical search of coronary calcification is considered to be a valuable procedure since it is inexpensive, noninvasive and widely applicable.
