Gestosis and the psychosomatic phenomenon--an empirical investigation.

Patients suffering from psychosomatic diseases in the strict sense of the term (asthma, ulcers, colitis etc.) have characteristic object relations which we call the 'relation blanche'. The present investigation aims at finding out whether similar features can be observed in patients suffering from early and late gestosis. The results indicate that there is a definite connection between gestosis and the group of strictly psychosomatic diseases, thus confirming our hypothesis that gestosis does not primarily represent a neurotic conflict situation nor a psychotic breakdown.

A 4-year follow-up study of in-patient psychosomatic patients.

The subjects of the inquiry were 13 psychosomatic patients who had all undergone psychotherapy in the special research unit of the University Clinic for Psychosomatics in Giessen (FRG) in 1971. Data collection took place at the beginning (time a) and end (time b) of in-patient treatment, and then again 4 years later (time c). The same test battery was administered each time. The results indicate that a significant change in the patients' self-image came about during in-patient treatment and the catamnesis carried out 4 years later attests to the stability of this development. The question of whether this change reflects the patient's adjustment to the ideals of his therapist--conformity with the 'norm'--or can be interpreted as a criterium for true ego development in the sense of an expansion of his psychic possibilities, is discussed.

["Spatial shift alignment (SSA)"--a new method for determining blood flow velocity in video capillary microscopy].

A new technique (spatial shift alignment [SSA]) for measuring capillary blood velocity in human capillaries in the skin is described. A capillaroscopic image is digitized by means of a frame grabber. Within the lumen of a suitable capillary limb a region of interest known as a slot is placed. At a frame frequency of 25 per second, this slot is copied in a free part of the frame grabber store along a horizontal axis. A static space-time image with light diagonal bands representing the plasma gaps in their passage through the capillary limb is obtained. By determining the gradient of the diagonal bands, a measure of the capillary blood velocity is obtained. The SSA system was validated by means of a simulated model using known fixed velocities between 0 and 1.0 mm/sec (r = 0.979, n = 24). The procedure can be employed with any image processing systems, and enables continuous monitoring of the velocity measurements. Since it is time-saving and just as accurate as the frame-to-frame method, SSA could be widely applied to the measurement of blood cell velocity in capillaroscopy.

[Cephalic duodenopancreatectomy for endocrine tumor of the ampulla of Vater and of the minor papilla].

OBJECTIVES: Endocrine tumors of the ampulla of Vater and minor papilla are rare. This study describes the mode of presentation and evaluates the correlation between pathological features and prognosis. PATIENTS: Between 1982 and 1998, 6 patients (3 M, 3 F, mean age: 47.6 years, range: 36-58) for whom a diagnosis of endocrine tumor of the ampulla of Vater or minor papilla was made between 1982 and 1998 after histological examination of an operative specimen of pancreaticoduodenectomy. RESULTS: One patient was detected incidentally, two had a Zollinger-Ellison syndrome, two had pain and one had obstructive jaundice with pain. The tumor was located in the ampulla of Vater in 5 cases and at the minor papilla in 1 case. All patients underwent a pancreaticoduodenectomy, with histological examination showing tumor diameter varying from 5 to 40 mm and positive lymph nodes. Five patients had a well differentiated endocrine tumor and one a poorly differentiated tumor. All patients had positive Grimelius staining. The secretory profile analyzed by immunohistochemistry was heterogeneous. Median duration of follow-up was 51 months (range: 6 months-16 years) with all patients currently still alive. The patient with a poorly differentiated tumor had diffuse liver metastases, the others were disease-free. CONCLUSION: This study demonstrates the frequency of metastatic spread to adjacent lymph nodes and the inconsistent secretory profiles of these tumors. Pancreaticoduodenectomy may offer long term disease-free survival in well differentiated tumors, and such histology may be useful in advising on prognosis.

[Treatment of aggressive primary lymphomas of the digestive system by intensive chemotherapy. Experience of the Study Group on Lymphomas in Adults with the LNH-84 protocol].

Ninety-one patients with primary gastrointestinal lymphoma included in the LNH-84 multicentric regimen were analyzed to determine the efficacy and the toxicity of intensive combination chemotherapy and of surgical debulking. All these patients had aggressive histological subtypes: 56 patients had localize disease, stage IE or IIE, and 35 patients had stage IV disease; 40 patients had bulky disease. Specific sites included stomach (43 cases), small bowel (35), ileo-cecum (13), colon (10), and rectum (6). Although 71 patients (78%) had an attempted surgical resection, only 28 (31%) had complete tumor excision. Patients were treated with 3 or 4 cycles of ACVB chemotherapy followed by sequential consolidation. Nine patients died during chemotherapy. With a median follow-up of 3 years, 10 patients have relapsed and predicted 4-year survival of the entire group is 62%. In stage IE or IIE patients, survival and disease-free survival are similar in patients who underwent complete resection or incomplete or no surgical resection prior to the administration of chemotherapy. Prognostic factors predicting for survival were similar to the entire group of patients treated with the LNH-84 regimen. Aggressive gastrointestinal lymphoma patients treated with intensive chemotherapy have outcome and prognostic factors comparable to those of other similarly staged aggressive lymphoma patients. Surgical resection prior to the administration of combination chemotherapy did not influence survival or disease-free survival in patients with localized disease.

[Generalized BCG infection in mixed and severe immunologic deficiency. Unfavorable outcome in spite of an attempted bone marrow transplant].

Generalized BCG infection can occur after vaccination in patients with a severe combined or T-cell immunodeficiency. In the reported case, generalized BCG infection developed in an infant with a severe combined immunodeficiency disease and presented mainly with hematological manifestations. This infection led to death of the patient in spite of an histocompatible bone marrow transplantation performed late in the course of the disease and followed by a minimal graft-versus-host reaction. Based on this case report and others from the literature, it is suggested that neonatal BCG vaccination should be performed after knowing at least the complete family history. Although the presented patient died, it should seem reasonable to treat such patients with an early immunological reconstitution and with anti-TB antibiotics.

[Value of intraductal biopsies and intraductal cytology in the etiological diagnosis of biliary stenoses].

Endobiliairy tissue specimens were obtained for cytologic and histologic examinations at the site of stenosis after percutaneous biliary drainage or endoscopic retrograde sphincterotomy in 49 patients with benign (10 cases) or malignant (39 cases) biliary stricture. Final etiologic diagnosis was obtained by surgery (18 cases), autopsy (one case), aspiration cytology of metastasis (2 cases), serology (2 cases), and in 26 cases, was determined by the course and outcome of the disease. As the sensitivity of each method, cytology and histology, was weak, it was necessary to associate them to improve results. Accurate etiologic diagnosis was achieved in 77 p. 100 of cases. The specificity of the method was satisfactory and specimen examination allowed to correct and confirm the initial etiologic diagnosis in 12 and 65 p. 100 of cases, respectively. The specificity of the method and its low morbidity prompt us to recommend it during transhepatic or retrograde therapeutic drainage as it can serve as a guide for later treatment. On the other hand, achievement of a transhepatic drainage or an endoscopic sphincterotomy only to obtain endobiliairy tissue specimens means taking into account the poor sensitivity of the method and the complications inherent to transhepatic drainage and endoscopic sphincterotomy. The practice of obtaining specimens by the retrograde technique without sphincterotomy or under cholangioscopy should be developed.

[Cancer and gastric sarcoidosis].

A 63-year-old woman treated 28 years ago for pulmonary sarcoidosis was operated upon for gastric cancer. Histology showed combined lesions of gastric sarcoidosis and adenocarcinoma. This is an exceptional association, and the relation between the two affections is discussed, the most likely hypothesis being a sarcoid reaction to the cancer.

Antigenic activity in monkey and human of purified low molecular weight fractions of Timothy pollen.

Small molecular weight fractions obtained from dialysis of crude Timothy pollen extract were shown to carry immunogenic activity in monkeys. Three fractions were sequentially purified through gel filtration with Siphadex G-10 and C-M cellulose 52. Monkeys immunized with these fractions produced almost exclusively skin-sensitizing antibodies which were shown to be of the IgE class and differing in antigenic specificity. The analysis of sugars and amino acids contained in two of these fractions showed large qualitative and quantitative differences. It is assumed that these fractions might represent oligomeric subunits of larger antigenic molecules. Challenge of patients allergic to Timothy pollen by means of skin tests and nasal provocation tests further confirms the absence of cross-antigenicity among the 3 active fractions. These results seem to indicate heterogenicity of IgE antibodies as developed following sensitization with crude Timothy pollen antigen.

[Benign tumours of the liver and oral contraceptives (author's transl)].

The pathologic features, the terminology, the etiolgoy, and the causal relations of benign tumours of the liver associated with the use of oral contraceptives were reviewed during a study on six new cases. According to the literature the following points are emphasized: these tumours can be divided into two distinctive conditions, they are commonly called focal nodular hyperplasia and liver cell adenoma, whereas the name hamartoma seems rarely appropriated, they occur without any use of oral contraceptive, but this practice is probably responsible for their increasing number.

[Cytodiagnosis by needle aspiration-biopsy in pancreatic diseases : a preliminary study (author's transl)].

In this preliminary work, the first results concerning the cytodiagnostic of pancreatic diseases by needle aspiration are reported. 37 aspirations were carried out on 30 patients. 16 during the operation and 21 by transcutaneous way using arteriography in 16 cases, ultrasonic scanning in 3 cases, tomodensitometry in 2 cases, as a guide. In 7 patients, both way were used. Cancer of the pancreas was diagnosed by this technique in 10 cases out of 14, without accident. No results were falsely positive. Skill and good technique would seem to allow to decide between pancreatitis and cancer in doubtful cases and diagnose cancer of the pancreas sufficiently early.

[Verbal behavior and communication experience of psychosomatic patients in the first psychoanalytic interview in connection with the concept of "pensée opératoire'].

This pilot-study was concerned with differences in speech behaviour and communication as experienced in psychoanalytically oriented initial interviews. In line with the ideas of Marty, Fain, de M'Uzan, David and Sami-Ali, 38 patients were divided into 3 diagnostic groups according to the extent of "pensée opératoire" and the level of functioning of the psychic sphere in each case. The communication experienced was recorded with the help of a communication questionnaire developed in Giessen. The speech behaviour for the interview as a whole and in its development over time was registered automatically and record in 29 categories by the Giessen Speech Analyzer. The following results are reported: --The two groups with more extensive "pensée opératoire" (A1, A2) differed from the character-neurotic patients in 13 of the 29 speech categories registered. The differences between groups A1 and A2 were not significant. --The communication as experienced by the patients did not differ between groups, but the therapist experienced the patients from the 3 diagnostic groups differently. The information gathered from the therapist in the communication questionnaire agreed by and large with the data collected automatically. --For the development of the interview over time, characteristic changes in the frequency of various speech categories were found but these were in the main unrelated to the diagnostic groupings.
