Correlations between ventricular arrhythmias and electrolyte disturbances after acute myocardial infarction.

Seven 24-hour ECG recordings and blood samples were taken within 3 weeks in 42 patients who had suffered an acute myocardial infarction (AMI). Ca, K and Mg concentrations in serum, and K and Mg in the erythrocytes, were determined by atomic absorption spectroscopy. One half of the patients were infused with 81 mval/day MgSO4 for 3 days. In patients who exhibited intense electrolyte alterations 10-20 days after AMI, there was a significantly higher rate in the frequency of couplets and/or tachycardia in the 2- to 20-day period after AMI. In patients infused with MgSO4, the fluctuation in serum electrolytes and the rate of arrhythmias were significantly reduced.

Enzyme catalytic concentrations in human plasma after a marathon.

Blood was obtained from 11 males participating in the Berlin marathon 1986, directly before and after the marathon, and on the three following days. Several observations were made: a) catalytic concentrations (activity) of creatine kinase (CK), lactate dehydrogenase (LDH), alanine aminotransferase (ALT), aspartate aminotransferase (AST), and alkaline phosphatase (AP) increased directly after the marathon or on the three following days; b) Cholinesterase (CHE), amylase (AML) and gamma glutamyltransferase (GGT) decreased directly after the marathon; c) the time course of AP and LDH isoenzyme activity after the race indicated an elimination from plasma to lower values than those originally observed before the run.

Alanine Transaminase

A BASIC program for calculation of often needed parameters in intensive care medicine.

For rational therapeutic decision making in intensive care patients with artificial ventilation and the use of vasoactive drugs, parameters such as pulmonary shunt, cardiac index and other determinants of gas exchange and circulation are often necessary. For this purpose, a BASIC program suitable for microcomputers was established which facilitates control of therapy. The formulae needed for the calculations are described, and the program is discussed.

Critical Care

Alterations of electrolytes in serum and erythrocytes after myocardial infarction.

Electrolyte concentrations in serum and erythrocytes of 56 myocardial infarction (MI) patients and 50 patients having had an orthopedic operation or an accident were measured up to 2 weeks after the event. Several hours after the MI, considerable percentages of patients had had pathologically low serum concentrations of either K, Ca or Mg. In the erythrocytes of the Mi group, K was decreased and Ca increased during the entire period of investigation, whereas the serum electrolytes K and Ca had normalized by day 15, at which time only half of the patients with pathologically low serum Mg had reached the normal Mg range. By correlation analysis it was shown that the more the Ca concentration in the serum was decreased after MI, the less was the concentration of brain muscle creatine kinase.
