[Neuropathologic data in drug-resistant partial epilepsy. Report of a series of 195 cases].

This report concerns the histopathological findings in 195 consecutive surgical specimens of patients with chronic drug-resistant epilepsy. Six pathological groups were identified corresponding to 63 tumors, 51 developmental lesions, 39 Ammon's horn sclerosis, 27 non specific changes, 11 glial or gliomesodermal scars and 4 Rasmussen's encephalitis. In 14 patients, 2 distinct abnormalities were observed with Ammon's horn sclerosis as one part of the dual pathology. All tumors were of low histopathological grade and included 30 dysembryoplastic neuroepithelial tumors, 17 gangliogliomas, 5 subependymal giant cell astrocytomas of tuberous sclerosis, 4 pleomorphic xanthoastrocytomas, 3 astroblastomas, 3 astrocytomas and 1 oligodendroglioma. Developmental lesions included 22 hamartomas, 12 focal dysplasias, 5 microdysgenesis, 1 nodular cortical heterotopia, 1 arachnoid cyst and 10 cavernomas. These findings emphasize two pathological features. Firstly, well-defined changes were present in all specimens. Secondly, the most common lesions were respectively tumors and developmental abnormalities.

[Systemic granulomatous reaction in hip prosthesis. Apropos of 2 anatomoclinical cases].

PURPOSE OF THE STUDY: The production of particulate wear debris is a recognised complication of joint arthroplasty. Focus was made on local tissue reactions. Systemic distribution of wear debris are less know. We report two new cases of distant granulomatous reaction. MATERIAL: The first case concerned a 71-year-old man with lymph node histiocytosis incidentally discovered during the staging of a prostatic carcinoma. The second case concerned a 61-year-old man with a visceral granulomatosis reaction (liver, spleen and lymph node) associated to hepatic ans splenic enlargement. METHODS: We realised an histological analysis of several tissue specimens for these two patients and for one case an inductively coupled plasma-mass spectrometry technique (ICPMS). RESULTS: In all specimens we identified a foreign body granulomatous reaction characterized by the presence of particles coming from hip arthroplasty. The ICPMS identified titanium in spleen. DISCUSSION: Distant granulomatous reaction are generally localised in regional lymph nodes and discovered by accident. A systemic granulomatous reaction attributable to wear particles raises the question of long term biocompatibility of prosthetic materials. CONCLUSION: Besides local reaction, it is of utmost importance that physicians involved in prosthetic field take into consideration the possible systemic adverse effect of wear particles from arthroplasty.

Frequency and prognostic evaluation of 3p21-22 allelic losses in non-small-cell lung cancer.

Previous loss of heterozygosity (LOH) studies of chromosome 3p loci have displayed a 60% deletion frequency in non-small-cell lung cancers (NSCLC), as opposed to small-cell lung cancers, in which the 3p deletion is consistently found. However, the high stromal-cell admixture found in NSCLC and the use of the Southern-blot method lead to under-evaluation of this frequency. In this study, we used a very precise microdissection technique followed by PCR amplification of 6 3p21-22 polymorphic genomic sequences to analyze LOH in 86 NSCLC and in normal adjacent tissue. We found the sensitivity of the microdissection-PCR-based LOH technique higher than the sensitivity of the Southern-blot technique: 87% of the squamous-cell carcinomas and 84% of the large-cell undifferentiated carcinomas showed a clear LOH for a 3p21-22 locus. All doubly informative cases but 4 showed concordant deletion at all 3p21-22 loci. The analysis of 3p microsatellite sequences displayed only 2 cases of genomic instability, one of them also displaying features of tumoral heterogeneity as regards the instability genotype. Four carcinomas in situ adjacent to these NSCLC showed the same allelic profile as the invasive tumors. The only prognostic factors in this study were the disease stage and histology. The 3p21-22 deletion was not related to the stage of the disease and did not appear to be a significant prognostic factor of survival. 3p21 loss appears, so far, to be the most frequent and the earliest genetic alteration described in NSCLC, but does not seem to carry significant prognostic information in invasive tumors.

[Primary synovial sarcoma of the mediastinum. A case report with immunohistochemistry, ultrastructural and cytogenetic study].

A biphasic synovial sarcoma occurring in the anterior and inferior mediastinum in a 19-year-old woman is reported. A biopsy showed a mesenchymal proliferation and the tumor was first misdiagnosed as a hemangiopericytoma. Inefficacy of chemotherapy led to a tumorectomy. Histologic and immunohistochemical studies on multiple samples showed a biphasic tumor. Ultrastructural study confirmed the presence of epithelial elements and cytogenetic analysis disclosed a translocation t(X;18) (p11;q11), leading to a diagnosis of synovial sarcoma. Synovial sarcoma of the mediastinum is very rare and to our knowledge has not been previously studied with the help of cytogenetics. Given the biphasic pattern of the tumor and its mediastinal location, it can be confused with mesothelioma. This stresses the interest of chromosome analysis in the study of tumors histologically difficult to classify.

Tandem orientation of the inter-alpha-trypsin inhibitor heavy chain H1 and H3 genes.

The inter-alpha-trypsin inhibitor H1 (ITIH1) and inter-alpha-trypsin inhibitor H3 (ITIH3) genes have both previously been mapped to chromosomes 3 and 14 in the human and mouse, respectively. We now present evidence that these genes are physically linked. By using cDNA probes, a recombinant DNA phage has been isolated from a bacteriophage DNA library, which contains sequences flanking the 5' end of the ITIH3 gene and the 3' end of the ITIH1 gene. Restriction endonuclease mapping, PCR analysis and DNA sequence determination of the recombinant phage and comparison to genomic DNA revealed that the genes are in tandem, 2721 base pairs apart, with the ITIH1 gene to the 5' side of the ITIH3 gene. Their respective transcriptional units are thus on the same strand of DNA and most probably arose in evolution as the consequence of a duplication of a common ancestral gene.

Carcinoid tumour complicating inflammatory bowel disease. A study of two cases with review of the literature.

Two cases of carcinoid tumour complicating inflammatory bowel disease (IBD) are presented. Both tumours were located in the appendiceal tip. The first case occurred in a man with Crohn's disease (CD), and the second one in a woman suffering from ulcerative colitis (UC). Histochemical and immunohistochemical studies were not allowed on case 1 because the tumour was not still present on serial sections of the appendix. On case 2, tumour cells were not reactive with Grimelius and Masson-Fontana stainings, but were strongly stained with anti-keratin and anti-chromogranin monoclonal antibodies (MAb), and faintly expressed neuron specific enolase (NSE), and Leu-7. Both cases occurred in inflammatory or damaged mucosa which exhibited Paneth cell metaplasia and hyperplasia and areas indefinite for dysplasia. Along with these lesions, hyperplasia of enteroendocrine cells was pointed out in the neighbouring appendiceal and colonic mucosa by means of anti-chromogranin MAb. These data suggest that the association of carcinoid tumour with IBD, albeit rare, is not coincidental and is the result of hyperplastic and dysplastic troubles that may involve enteroendocrine cells as well as such other derivatives of digestive stem cells as columnar cells, goblet cells and Paneth cells.

[Bacillary angiomatosis related to Rochalimaea quintana. Anatomoclinical and ultrastructural study of cutaneous localizations in AIDS].

We report a case of bacillary angiomatosis in a 53-year-old homosexual man with acquired immunodeficiency syndrome (AIDS). Pathological and bacteriological studies of cutaneous nodules led to the identification of a rickettsia: Rochalimaea quintana. This observation prompted us to relate the clinical presentation of cutaneous and visceral forms of this disease. Histopathological patterns are also considered. They usually consist in a lobular proliferation of capillaries with plump and sometimes epithelioid endothelial cells. Polymorphonuclear cells, histiocytes and necrotic areas may be present. The most characteristic feature is the presence of interstitial, granular and amorphous clusters of bacteria. Diagnostic problems can be raised with Kaposi's angiosarcoma which can be associated with bacillary angiomatosis. Two types of Rochalimaea have so far been isolated in this disease i.e., R. henselae which is the most frequently involved, and R. quintana. The usefulness of making such a diagnosis resides in the sensitivity of bacillary angiomatosis to antibiotics, emphasing the need to carefully look for the presence of bacterial clusters when atypical angioproliferative lesion appears in patients with AIDS.

[Neurothekeoma. General review apropos of an anatomoclinical case with immunohistochemical and ultrastructural study].

The authors report a case of neurothekeoma in a 16-year-old woman. The tumor was located in the dermis of the chin and exhibited a plexiform and multinodular architecture with dense lobules admixed with myxoid areas. Tumor cells were pleomorphic and few atypias and mitoses were seen. On immunohistochemical study, intracytoplasmic staining was observed with anti-vimentin antibody, and, to a lesser extent, with KP1 antibody. Few elements expressed S100 protein. Ultrastructural analysis showed undifferentiated mesenchymal cells. A review of 156 cases (of which 123 cases are reported in two main series) shows that neurothekeoma preferentially affects dermis in cervicofacial areas and shoulders in young women. It has a benign course and recurrence is not seen provided excision is complete. Two main histological varieties are described, i.e. cellular and myxoid, the latter being referred to as nerve sheath myxoma. Diagnostic problems can be raised with other nerve sheath tumors and melanocytic and fibrohistiocytic proliferations. Transition forms between neurothekeoma and neurinoma or neurofibroma are described. Despite confusing immunohistochemical and ultrastructural data, most authors admit that neurothekeoma can express schwannian or perineurial differentiations, and may also be derive from an undifferentiated mesenchymal cell of neural crest origin.

Isolation and characterization of the human inter-alpha-trypsin inhibitor heavy-chain H1 gene.

The human inter-alpha-trypsin inhibitor heavy chain H1 (ITI heavy chain H1) gene was isolated from two overlapping clones. It spans 14 kbp and is composed of 22 exons from 15 bp to 281 bp in size and has consensus splice sites. Intron sizes range from 80 bp to 2000 bp. It codes for the precursor of HC1 that is part of the serum ITI form of 220 kDa. Two major transcriptional initiation sites were identified in the 5'-flanking region, which contained putative promoter elements, but no typical TATA and CAAT boxes. mRNA for the ITI heavy chain H1 was found only in liver. The tissue-specific transcription of the gene might be due to the presence of binding sites for the hepatocyte nuclear factor HNF-5 and to the octameric motifs. A previous overlapping of cDNA clones indicated the absence of 29 bp in one of these clones. The present study shows that the 29 bp is located within the gene at the end of exon 21. A reverse-transcriptase polymerase chain reaction mapping analysis of liver mRNA identified the two types of the mRNA for ITI heavy chain H1. Accordingly, the data demonstrate that there is alternative splicing of at least one exon of the ITI heavy chain H1 gene.

Human pre-alpha-trypsin inhibitor-precursor heavy chain. cDNA and deduced amino-acid sequence.

Pre-alpha-trypsin inhibitor (P alpha I) is a serine-proteinase inhibitor of M(r) 130,000 found in human serum. This protein belongs to the family of proteins called inter-alpha-trypsin inhibitor (ITI). P alpha I is composed of a heavy chain (HC3) and of a light chain (bikunin), synthesized by two separate mRNA. Bikunin is identical to the ITI light chain, the structure of which has already been established. The HC3 is obtained from a precursor called H3. The bikunin is covalently linked to HC3 by a chondroitin-4-sulfate glycosaminoglycan. We report here the H3 full-length cDNA sequence and the deduced amino-acid sequence of the heavy-chain H3 precursor. The high degree of similarity between the nucleotide and amino-acid sequences of ITI heavy-chain families H1, H2, H3 is examined with respect to their probable structure and assembly with bikunin in the final proteins, P alpha I and ITI.

Effect of maternal hyperglycemia on NaK ATPase activity in fetal rat kidney.

The effect of moderate hyperglycemia on renal ATP production and ATPase activity of rat fetus was investigated using the experimental procedure of maternal continuous infusion of glucose during the last 5 days of gestation. Glucose-infused mothers and their fetuses showed a high level of glycemia (8.8 and 5.5 mM, respectively) and a high level of insulinemia (3 times higher than in controls). No change in either ATP or ADP concentration was detectable but an increase in NaK ATPase activity occurred without any change in Mg ATPase activity. These modifications should be the result of an enhanced Na/glucose cotransport leading to an enhanced extrusion of Na at the basolateral membrane. These results indicate that immature kidney is able to increase NaK ATPase activity to maintain Na homeostasis.

Human inter-alpha-trypsin inhibitor: full-length cDNA sequence of the heavy chain H1.

Inter-alpha-trypsin inhibitor (ITI), called inter-alpha-inhibitor, is a 220 kDa serine proteinase inhibitor found in human serum. It is composed of at least three distinct polypeptide chains. These chains, named H1, H2 and L, are an independently synthesized and proteolytically processed precursor protein. Only the complete structure of H2 and L has been established so far. We used a PCR-based cloning approach and a cDNA screening library to isolate the full-length cDNA H1. The amino acid sequences of the two heavy chains deduced from the cDNA are highly similar (40% identity). Nevertheless, the structure of the signal peptide and propeptide in the N-terminal region is different in these two chains. A complex posttranslational cleavage at both ends of H1 and H2 may be proposed prior to assembly of the ITI chains.

[Spinal leptomeningeal tumor spread disclosing a pineocytoma].

A 39-year-old female was admitted to the hospital because of a sudden meningeal syndrome followed by diplopia, cervical, dorsal and sciatic nerve pains, and right peripheral facial palsy. Cerebrospinal fluid obtained by lumbar puncture showed a protein level at 23 g/l. Myelography and magnetic resonance imaging (MRI) were in favor of a lumbar arachnoiditis. A meningeal biopsy revealed a tumour infiltration with foci of cells that were stained with anti-glial fibrillary acidic protein antibody. Cerebral MRI was performed to search for a central nervous system (CNS) primary tumour, and disclosed a pineal mass. Five months after the onset of the disease, the patient worsened her clinical state and died. Necropsy confirmed the presence of a pineocytoma with astrocytic differentiation and diffuse leptomeningeal spread. This exceptional occurrence leads us to discuss about primary tumours of the CNS with leptomeningeal spread.

The effect of spine fusion on respiratory function in Duchenne muscular dystrophy.

Serial measures of respiratory function were made in 17 patients with Duchenne muscular dystrophy who underwent segmental spine fusion and in 22 patients who did not. No significant differences were observed between the 2 groups (operated vs. nonoperated) in terms of declining respiratory function. Nonetheless, all operated patients reported either improved sitting comfort, appearance, or both. Thus, our results suggest that there are distinct benefits from segmental spine fusion; however, there was no salutary effect upon respiratory function either in the short term or after up to 5 years follow-up.
