Specific and sensitive measurement of FK506 and its metabolites in blood and urine of liver-graft recipients.

A specific and sensitive assay for quantifying the immunosuppressant FK506 and its metabolites in blood and urine was developed. 32-O-Acetyl FK506 was synthesized and used as internal standard. FK506 and its metabolites were purified from the samples by solid-liquid extraction and were injected into a high-performance liquid chromatographic (HPLC) system linked to a mass spectrometer (MS) by particle-beam interface. The FK506 derivatives were separated from interfering material by use of a 100 x 4 mm C8 analytical column and water/acetonitrile or water/methanol gradient elution; they were detected by negative chemical ionization with methane as reagent gas. The limit of detection was 25 pg in a standard solution, and the limit of quantification in blood was 250 pg (extracted from 1 mL of blood). The CV was 11.3% at 5 ng, and no interferences with other drugs were found.

[Warfarin embryopathy in maternal coumarin therapy for protein S deficiency].

A case of Warfarin embryopathia is shown. The newborn, whose mother had been treated with Marcoumar (Phenprocoumon 3 mg/day) during the whole pregnancy because of a hereditary protein-S-deficiency showed the typical symptom of nasal hypoplasia. Coumarol derivates pass the placental membrane and are known as teratogenic. Recent retrospective and prospective studies show that the risk of fetal or embryonal teratogenic injury is about 25-30% if Coumarol derivates are given through the 6th to the 9th week of pregnancy. It is possible to nearly avoid those injuries when Heparin is used for anticoagulant therapy in this period and Coumarol itself is used in the lowest possible therapeutic dose.

[Ultrasound image of an unusual intrahepatic site of a bile duct cyst].

An intrahepatic cyst was found ventral and cranial to the gallbladder without choliangectasy in an infant of 8 months of age. Final diagnosis of a bile duct cyst was arrived at intraoperatively. The article discusses the possible aetiology, non-invasive diagnosis and sonographic differential diagnosis. Sonography is discussed as the method of choice in assessing intrahepatic and extrahepatic cysts and the bile ducts.

Binding sites for atrial natriuretic peptide on platelets in patients with congestive cardiomyopathy.

The aim of the present investigation was to evaluate a possible down-regulation of atrial natriuretic peptide (ANP) binding sites on platelets in patients with chronically elevated ANP plasma levels. The assay procedure was proved to be able to measure the total number of binding sites even in the presence of high ANP plasma levels. We studied 15 adult patients with congestive cardiomyopathy in comparison to 18 healthy volunteers. In the patients the median ANP plasma level (median = 375, range: 155-900 pg ml-1) was about six-fold higher than in the healthy volunteers (median: 55.5, range: 20-90 pg ml-1). The median cyclic guanosine monophosphate (cGMP) plasma level (median: 6.2, range: 2.5-21.4 pmol ml-1) was about three-fold higher than in the healthy volunteers (median: 1.8 range: 1-2.8 pmol ml-1). Despite these markedly elevated ANP and cGMP plasma levels we did not find significantly less receptors per platelet in the patients (median: 19, range: 7.2-60.2) than in the healthy volunteers (median: 24.5, range: 14.8-41.1). Furthermore, there was no difference in the dissociation constants between the patients (median: 10.5, range: 7.9-27.4 pmol l-1) and the control subjects (median: 8.9, range: 5.4-17 pmol l-1).

[The significance of respiratory syncytial virus as pathogen in respiratory tract infections in infants (author's transl)].

Ten out of twenty babies aged up to twelve months and admitted during the months February and March 1978 were shown to be infected with Respiratory Syncytial Virus. During the first 6 months of life the disease usually presents as bronchiolitis. Serologically only neutralising antibodies were demonstrable in this age group. IgA was usually raised in babies with bronchopneumonia. An allergic reaction between virus antigen and maternal antibodies is considered to occur, assuming an activation of the infant's own secretory immunoglobulin.

[Autologous cancellous bone grafting in rabbits using a fibrinogen adhesive system (author's transl)].

The changes in remodelling of autologous cancellous bone after reimplantation using a fibrinogen adhesive system (FAS) consisting of highly concentrated fibrinogen, thrombin and factor XIII were investigated in 23 rabbits. A 5 x 6 mm sized piece of bone was removed from the iliac crest and reimplanted in the same place with and without FAS. Histological, tetracycline fluorescence and radiological evaluation were carried out after 3, 6 and 9 weeks. The control group (without FAS) showed considerable breakdown of the bony trabeculae due to regression of the osteocytes in the centre of the reimplanted bone after 3 weeks, whereas in the FAS group bone was being built on to the vital trabeculae. After 6 weeks the reimplant in the FAS group was still clearly advanced in relation to the non-FAS group. The difference had almost disappeared after 9 weeks. Acceleration of remodelling and lack of regression of the osteocytes in the centre of autologous reimplants after the use of FAS indicates an earlier recommencement of nutrition as compared with the control group. New bone formation in autologous reimplants was clearly accelerated by the use of FAS.

[Reaction of connective tissue and striated muscle tissue to implanted ashwood (author's transl)].

In former experiments it could be shown, that new bone is formed around ashwood after implantation in the tibia of rabbits, in spite of a foreign-body tissue reaction. Equally prepared ashwood half-cylinders were now implanted in the soft tissue of rats and evaluated 2, 6, 12 and 30 weeks after implantation. Around the implants a foreign-body tissue reaction developed and demonstrates the hitherto insufficient preparation of the implants.

[Inhibition of secondary increase of intraocular pressure following cyclokryocoagulation (author's transl)].

Prostaglandin mediated transient increase in intraocular pressure following cryokoagulation of the ciliary body in rabbits was completely inhibited by preoperative administration of Aspisol (D, L-Lysin-mono-(acetylsalicylat)). To avoid additional increase in intraocular pressure in patients chosen for cyclocryotherapy, pretreatment with Aspisol could prove to be effective.

Semisynthetic cephalosporins. Synthesis and structure-activity relationships of 7-(1-pyrryl)- and 7-(1-indolyl)acetamidocephalosporin derivatives.

A series of 1-pyrrole- and 1-indoleacetamido derivatives of 3-heteroaryl-substituted cephalosporins was prepared. The most active compound in the series was 7-[[2-(1-pyrryl)acetyl]amino]-3-[[(1-methyltetrazol-5-yl)thio]-methyl]-8-oxo-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic acid (6), which showed comparable potency in vitro and in vivo to that of cefazolin, and, in addition, was more potent than cefazolin against Enterobacter sp. and Providencia stuartii.

[The technic of fibrin glue in cancellous bone transplants (author's transl)].

The paper deals with the clinical experience in the use of a biological adhesive system in conjunction with bone transplants to close large bone defects. The gluing technique has been exactly described, 11 patients with a follow-up between 9 and 21 months are being presented. The application of this system which consists of highly concentrated native fibronogen, thrombin, and clotting factor XIII results in an acceleration of homogenous incorporation of the implants. No deterioration of wound healing occurred. The advantages of this method are: exact local hemostasis, adhesive strength, and plasticity.

Adult

[Long-time follow-up of rare liver tumours in infancy (author's transl)].

Two rarely-observed tumours of the liver are discussed: firstly, primary liver carcinoma in a 5-month-old infant, and secondly, myxoma in a newborn infant. Both tumours were removed surgically. The child with primary liver cell carcinoma was treated for 2 1/2 years with cyclophosphamide. She has been followed-up for 8 years and can be assumed to be cured. Both children developed normally. The pathogenesis of liver tumours at this early age is discussed.

[The action of high-concentrated fibria on bone healing (author's transl)].

The influence of a biological tissue glue--based on fibrinogen clotting--on bone healing has been investigated in animal experiments. Standardized cortical defects in the tibiae of rabbits were closed with a plug of homologous or heterologous fibrin. The course of restoration has been studied using histological techniques, x-ray examinations and isotopes (Ci 99 TC). Heterologous fibrin delays bone formation, due to a local immune response. Homologous fibrin on the other hand cubances and accelerates sprouting of capillary vessels and connective tissue cells which results in rapid new formation of bone.
