"Adenomatous polyps of the gallbladder" adenomas of the gallbladder.

The finding of adenomatous polyps of the gallbladder is a rare occurrence and an unusual clinical problem. Among 2,145 patients who underwent cholecystectomy for gallbladder disease only 9 (0.4 per cent) presented with adenomatous polyps. There were 6 women and 3 men, aged 17 to 70 years. Preoperative ultrasonographic diagnosis was made in only 1 of 7 patients with gallstones, in contrast polypoid lesions within a gallbladder without stones were easily confirmed by both ultrasonography and oral cholecystography in the remaining 2 patients. All polyps were 1.0 cm or less in size and without histologic evidence of malignant change. The clinical significance of this rare condition is discussed, with particular reference to a possible role in development of gallbladder carcinoma. Surgical treatment should be advocated regardless of clinical manifestation when the polyp exceeds 1.0 cm in diameter or rapid growth of the lesion is seen on ultrasonographic follow-up examinations.

The possible role of sphincteroplasty and surgical sphincterotomy in the pathogenesis of recurrent common duct brown stones.

The hypothesis has been tested that postcholecystectomy common duct stones of the brown subtype are a consequence of three factors: bile infection, old age and previous sphincterotomy. It was found that: (i) 27 of 39 consecutive patients with recurrent common duct stones had brown stones. Nineteen of these 27 patients (70.3%) had previous sphincterotomy or sphincteroplasty: (ii) six of 15 patients with stone and bile analysis both at the time of cholecystectomy and at the second operation and who had sterile operative bile and non brown stones at the first operation, formed brown stones after T-tube drainage and after the onset of bile infection; (iii) patients with both intra and postoperative negative bile culture (n = 39 out of 137) had a lower mean age (50.5 years) and less frequently had a sphincterotomy than did individuals with a negative culture at operation, who subsequently had bile infection (n = 37; mean age 58.5 years; sphincterotomy in 88.8% of cases). In addition, in a follow up study of 105 patients with sphincterotomy and with sphincteroplasty (including ERCP or i.v. cholangiography in all cases), mean follow-up interval 6.1 years, 11.3% of patients had brown recurrent common duct stones. It is suggested that, since brown recurrent common duct stones are secondary to bile stasis and infection and the duodenum is going to be colonized by bacteria with increasing age, sphincterotomy (and subsequent stricture), facilitating bile contamination and bacterial overgrowth, could be one of the major determinants of brown recurrent common duct stones (RCS) formation. In particular, more than 11% of the patients with a sphincterotomy are going to form in the future RCS of the brown subtype.

Newborn chemical exposure from over-the-counter skin care products.

Human exposure to potentially toxic environmental chemicals is a major concern to our society. Over-the-counter (OTC) skin care products are a potential source of environmental chemical exposure to newborns. A prospective study was undertaken to define existing newborn bathing practices and quantitate environmental chemical exposure from OTC skin care products. One hundred-ninety six mothers of infants discharged from our newborn nursery were asked to respond to a questionnaire concerning newborn bathing practices and OTC skin care product usage. Complete questionnaires were returned by 23% of the mothers. The average newborn had 8 +/- 3 (+/- SEM) skin care products applied with the resultant exposure to 48 +/- 18 different environmental chemicals. Forty-seven percent of our respondents used a baby powder. The average one month old was bathed 4 times a week and was shampooed 3 times weekly. Seventy-eight percent of the respondents reported that their newborn had a rash but only 45% of those reporting a rash sought professional medical advice. Thirty-seven percent of the mothers reporting a rash made a change in bathing practice or product selection. In view of our findings, the human newborn is exposed to a large number of environmental chemicals thorough topical OTC preparations. The efficacy, transcutaneous absorption and potential toxicity of these products should be investigated.

Ovarian and plasma PGE and PGF levels in naturally ovulating brook trout (Salvelinus fontinalis) and the effects of indomethacin on prostaglandin levels.

Plasma and ovarian levels of PGE and PGF were measured by radioimmunoassay in naturally ovulating brook trout (Salvelinus fontinalis). At 12 hours post-ovulation, PGF levels in plasma and ovarian tissue were significantly elevated over levels in gravid controls. Plasma PGF levels were still elevated 5-7 days post-ovulation. Indomethacin treatment (10 micrograms/gm body weight) of ovulatory brook trout significantly decreased plasma and ovarian PGF levels by 12 hours post-ovulation. No significant differences were observed in plasma PGE levels, though gravid controls contained significantly elevated ovarian PGE levels as compared to ovulated fish.

Ovarian and plasma prostaglandin E and F levels in brook trout (Salvelinus fontinalis) during pituitary-induced ovulation.

Prostaglandin E (PGE) and F (PGF) levels in plasma, ovarian tissue and coelomic fluid from ovulating brook trout (Salvelinus fontinalis) were measured using radioimmunoassays. The results demonstrate a significant increase in both plasma and ovarian PGF levels by the completion of ovulation (compared to gravid controls). Plasma PGF levels remained elevated 24 h following ovulation in females retaining ovulated eggs. Coelomic fluid from fish just completing ovulation contained PGF levels that were significantly higher than plasma PGF levels of corresponding fish. There were no significant variations in plasma PGE levels throughout ovulation; however, the ovarian PGE levels of gravid controls were significantly higher than those of fish just prior to, during or following ovulation.

Spontaneous contractility in the human lymph vessels.

During the diagnostic lymphangiography of the lower limbs, the motility of the lymph vessels was investigated in 79 consecutive patients, by means of a particularly performed serial radiography, aided by photographic magnification of films. Because of technical difficulties, a rather high incidence of imperfect examinations occurred. In 26 out of 35 (74.3%) technically satisfactory observations obtained from patients with no obstacle to lymph flow, evident morphological modifications were demonstrated, surely caused by intrinsic spontaneous contractility. Only in few cases a contractile activity, somehow resembling a true peristaltic wave could be demonstrated; usually the lymphatic "pulse" appeared irregular and greatly varying and no specific rhythm was detected. Our findings suggest that: intrinsic contractility may be an important determinant of lymph flow; the pattern of contractility is strongly influenced by the anatomical feature of the lymphatic chain, whose lymphangions (intervalve segments), appear continuously varying in size and shape.
