[Resection of the central slip in treatment of longstanding mallet fingers (Fowler's technique). Apropos of 8 cases].

Tenotomy of the central slip of the extensor retinaculum over the proximal interphalangeal region was performed in 8 cases longstanding, flexible mallet fingers. This technique gave 6 excellent results, with one complete correction of the PIP and DIP deformities, and 2 partial results (residual distal defects of 5 and 10 degrees) without any reduction in flexion and without any complications, in particular no boutonnière syndrome. The two incomplete results were observed in two patients in whom the preoperative DIP deformities exceeded 50 degrees, a value which the authors consider to be the limit for the choice of this method. This simple (no postoperative immobilisation) technique is reliable in precise indications which are discussed here. It should take its place together with tenodermodeses and more ambitious but more complicated reconstructions such as reconstruction of the spiral retinacular ligament described by Littler.

[Remote results of the surgical decompression of the posterior tibial nerve in the neuropathies of Hansen's disease].

Results are reported of surgery to the posterior tibial nerve affected by leprosy in 50 patients. A total of 90 neurolyses were performed using a similar technique. Study of the effect of surgery on the course of plantar sensitivity showed that 3 groups of nerves can be distinguished. Nerves in group 1 (21 totally paralyzed nerves) responded poorly to surgery (38% useful results) while on the contrary nerves in group 3 (non-paralyzed nerves) represented the ideal indication, surgery being truly prophylactic. In group 2 nerves a useful result was obtained in 82%. Indications for use of this surgical procedure are discussed, and more particularly the parameters affecting the quality of the results: severity of lesion, immunologic type of disease.

[Acetabular fracture with intrapelvic luxation following an epileptic attack. Apropos of 2 cases including 1 bilateral case].

The authors have observed 3 acetabular fractures following epileptic fits in two patients. One case was bilateral with an anterior displacement on one side and a posterior one on the opposite side. The second case, which was unilateral, showed a central displacement and a transverse fracture of the acetabulum. Both cases were treated conservatively. The first had a relatively good result after 6 months, and the second died after 10 days.

[Appearance of the normal knee by MRI using a surface antenna].

NMR imaging of the knee using a surface aerial provides remarkable anatomical detail of this joint without the need for invasive procedures or stressful positioning. Results of the use of this technique on seven normal knees confirmed the precise anatomical details provided, with direct visualization of the femorotibial interline and particularly of the cruciate ligaments, menisci and cartilages. This method allows a new diagnostic approach to the knee, especially for traumatic or tumoral lesions, and greater efficacy of treatment.

[Computed tomographic study of the anatomical modifications of the wrist brought about by section of the anterior annular ligament].

The authors have analysed the action of sectioning the transverse carpal ligament by computerised tomography in 19 patients (22 wrists) with carpal tunnel syndrome. Three cuts were studied-proximal, middle and distal, the wrist being either straight, flexed or extended. The study was done pre-operatively and three months after division of the ligament. The study demonstrated the reconstruction of a new ligament which was more lax and expanded anteriorly, despite the fact that the patient wore a plaster cast for two weeks after operation, the wrist being maintained straight. The calibre of the carpal tunnel showed an increase of between 7 p. 100 and 44 p. 100 after the operation and this increase was not related to any alteration in bone topography, which was unchanged. Thus, the role of the ligament in the permanency of shape of the anterior concavity of the wrist skeleton is questionable. The flexor tendons could expand anteriorly, thanks to the laxity of the new ligament.

Neurolysis of the common peroneal nerve in leprosy. A report on 22 patients.

Thirty-two operations on the common peroneal nerve for leprous neuritis are reported. A combined medical and surgical approach to treatment is recommended, and the technique of operation is described. Recovery of motor power was satisfactory but depended on many factors, including the duration of the neuritis, the extent of the compression, the immunopathological status of the patient and the efficacy of medical treatment. The main indication for neurolysis is hyperalgesic neuritis. The only contraindication is painless long-standing paralysis; in this condition the degree of neural fibrosis prevents any hope of improvement.

[Results of the surgical treatment of chronic external laxity of the ankle by resetting capsular ligamentous tension].

The authors review 12 patients operated for tibio-tarsal instability and summarize the effects of ligamentous sections of the capsular-ligamentous plane of the lateral part of the ankle and the importance of the anterior peroneotalar fascia. In cases of tibio-tarsal instability, the authors propose a lateral ligamentous re-tension plasty with trans-osseous reinsertion and they analyse the results of this operation in a series of 13 operated ankles which were followed clinically and radiologically. The essential criterion of evaluation was a return to the same level of sports activity (7 very good results, 3 good results, 2 moderate results and one failure). The overall favourable results suggest that this technique can be indicated in a number of rare cases: sportsmen with significant disability after failure of treatment by proprioceptive re-education. In conclusion, the authors stress the value of preventative treatment of sprained ankles, which should prevent the development of instability.

[Plate thermography in the evaluation of idiopathic carpal canal syndrome. Apropos of 32 cases].

During a period of eight months thirty-two cases (twenty-two unilateral and five bilateral) of idiopathic carpal tunnel syndrome were studied at the Saint-Louis Hospital, Paris, France. The patients were found to benefit from the use of the preoperative technique, thermography in addition to routine clinical exams and complementary exams such as radiographs and E.M.G. The period of intervention was from the fifteenth day through the second and thirds months. By comparing the clinical and functional results before and after surgical treatment through the use of images from patients in the same condition, it can be concluded that the use of this simple technique can provide valuable information and is harmless in the care of idiopathic carpal tunnel syndrome. It is demonstrated that thermography permits: the ability to foresee the action of treatment; the ability to survey the effects of treatment and therefore obtain a valuable prognosis; and finally, a method of deciding on other acrosyndromes.

[Cervical myelography and x-ray computed tomography in arm paralysis of traumatic origin].

Results of cervical myelography by the lumbar approach were compared with those of CT scan imaging after subarachnoid metrizamide by the lumbar route, in 16 patients operated upon for post-traumatic cervical spine lesions. Examination in each patient included the last four cervical and first thoracic roots on the injured side, making a total of 80 roots. Lesions detected by myelography and CT scanning are described, comparison of semiologic data with gross findings at operation showing poorer diagnostic accuracy for myelography (86%) than for myelography and a CT scan combined (97%). Sensitivity of the former varied between 70 and 90%, depending on whether doubtful forms were classed as normal or pathologic, but was lower than that of the CT scan (96%). This also applied to specificity (90% as against 96%) and diagnostic accuracy (86% to 90% as against 97%). Combining myelography with a CT scan after subarachnoid contrast injection provides complementary very precise data allowing improved therapeutic results.

[X-ray computed tomographic analysis of structural changes of the carpal bones, after section of the anterior annular ligament].

Computed tomography analysis was conducted in 29 patients with 34 carpal canal syndromes to determine structural modifications of the wrist during the immediate preoperative period and two and three months after operation. The carpus was analyzed at 3 levels of the section: proximal at the entrance orifice of the canal, median and distal at the orifice of exit, with the wrist at rest and then in maximum active flexion and extension. Results showed that the increase of canal caliber (and therefore the decrease in local increase in pressure) continued during the second and third postoperative months, at rest as well as in flexion or extension. It appears, further, that opening of the canal does not alter structural organization of wrist bones or ligaments, the increases in caliber obtained varying in the same sense whatever the position of the wrist or the level of the section.

[Isolated compression of the motor branch of the ulnar nerve].

This paper reports four cases of isolated compression of the deep branch of the ulnar nerve, and reviews the various etiologies (tumor, malformation and microtraumatism). The authors stress the importance of the electric examination, particularly conduction velocity in the palm in localizing the nerve lesion. Surgical exploration in these cases revealed a Giant cell tumor, two synovial cysts, and an abnormal fibrous band stretching between the flexor digiti quinti brevis and the opponens digiti quinti. Postoperative courses were uncomplicated and every patient recuperated quickly and completely. Early surgical exploration in such cases is mandatory.

[Benign osteoblastoma of the hand. A case report. Review of the literature].

One case of benign osteoblastoma, localized on the proximal phalanx of right thumb is reported. Three points are emphasized: the low frequency in the hand, only six observations are reported in the English literature, one by Jaffe (1932) and four by Lichtenstein (1964) and one from Mosher and coll. (1978). The non-specificity of the signs and X-ray are must consider chondroma, giant cell tumor, chondrosarcoma or aneurysmal cyst. The lesion is benign and so the treatment must be conservative.

[Evaluation of carpal tunnel and subepineural pressures in leprous neuropathies of the median nerve at the wrist].

The authors measured the carpal tunnel and intraneural pressures with a soft catheter in fifteen cases of leprous neuritis of the median nerve at the wrist. In the carpal tunnel, the pressure was about 40 mm of mercury, while within the nerve, the pressure was about 25 mm of mercury. In five healthy male volunteers, the same pressures were measured: the results were much lower (2 to 4 mm of mercury). The authors call attention to the high pressures found in acute leprous neuritis. They conclude that the sooner a surgical procedure (careful neurolysis) is performed, the better the results, before irreparable nerve damage occurs.
