Assessment of cyclin D1 overexpression as a prognostic factor in soft tissue sarcomas: role of laparoscopy and core needle biopsy.

BACKGROUND: This study was designed to test cyclin D1 as a prognostic marker in patients with soft tissue sarcomas (STS), and to evaluate the usefulness of laparoscopy for determining cyclin D1 overexpression. METHODS: The records of 62 patients with STS were collected: 28 with retroperitoneal STS (RSTS) and 34 with extremity STS (ESTS). A total of 51 patients underwent surgical resection, whereas 11 did not undergo surgery because of advanced tumor stage. Preoperative-intraoperative laparoscopic staging was performed for patients judged to be resectable at preoperative imaging. RESULTS: Cyclin D1 was overexpressed in 30 (58.8%) of 51 resected patients and in 10 (90.9%) of 11 nonresected patients. Laparoscopy avoided unnecessary laparotomy in 9 (32.1%) of 28 RSTS patients. CONCLUSIONS: High tumor grade, positive surgical margins, local recurrence, distant metastases, and cyclin D1 overexpression were related to poor survival. Multivariate analysis demonstrated cyclin D1 to be the only independent factor. Laparoscopy was shown to be useful for avoiding useless laparotomies.

Arterial embolization of aneurysmal bone cysts in children.

The long-term results of six aneurysmal bone cysts in children treated by selective arterial embolization are reported. All the cysts were large and difficult to treat by surgery. To date, no surgical treatment has been required in these children and an almost complete restoration of the original shape of the affected bones has been achieved.

Hot occupation and nephrolithiasis.

We investigated the prevalence of stone disease and urinary stone risk factors in machinists chronically exposed to a hot environment and massive sweating, without interference of nephrotoxic metals or other lithogenic compounds. The study was performed at a glass plant and exposure to heat stress was estimated by the Wet Bulb Globe Temperature climatic index. The prevalence of nephrolithiasis on the entire population of the machinists was 8.5% (20 of 236), while the prevalence on the controls working in normal temperature was 2.4% (4 of 165) (p = 0.03). A high incidence (38.8%) of uric acid stones was present in the workers exposed to heat stress. Among the urinary stone risk indexes determined for 3 days during the 8-hour work shift on a randomly selected sample of 21 workers exposed and 21 workers not exposed to heat stress without any evidence of stone disease significant differences were found in uric acid concentration (722 +/- 195 versus 482 +/- 184 mg./l., p < 0.001), specific gravity (1,026 +/- 4 versus 1,021 +/- 6, p < 0.005) and pH (5.31 +/- 0.28 versus 5.64 +/- 0.54, p < 0.02), respectively. Thus, high uric acid relative supersaturation was present during occupation in hot temperatures (8.67 +/- 3.49) compared to occupation in normal temperatures (4.15 +/- 2.7) (p < 0.001). This study confirms that chronic dehydration represents a real lithogenic risk factor, mainly for uric acid stones, and adequate fluid intake is recommended during hot occupations.

Adrenal response in the pathogenesis of arterial hypertension in workers exposed to high noise levels.

Some neuroendocrine parameters known as stress indices were examined in two groups of healthy male workers in a glass factory: the first group (60 subjects) was exposed to high environmental noise levels [greater than 90 dB(A)]; the second group (52 subjects) was exposed to low noise levels [less than 78 dB(A)]. Subjects with histories of cardiovascular diseases or high arterial pressure were excluded from the study. In both groups serum catecholamines and cortisol, and urinary vanilmandelic and homovanillic acids were evaluated at the beginning and middle of morning and afternoon work-shifts, by high performance liquid chromatography with electrochemical detection. Norepinephrine, epinephrine and vanilmandelic acid were significantly increased (P less than 0.01) during work-shifts in the group exposed to 90 dB(A), compared with baseline levels and also with catecholamine levels in the group exposed to 78 dB(A). Serum dopamine, cortisol and homovanillic acid showed no significant differences. The increased stimulation of the sympatho-adrenal system in response to high and prolonged noise exposure might lead to an abnormal response of the cardiovascular system with increasing arterial pressure values.

Growth disturbances following fractures of the femur and tibia in children.

Growth disturbances can occur following fractures of the femur or tibia in children. They can produce lengthening, shortening, axial and rotational deformities. The number of variable factors involved is such that valid statistical analysis demands a very large series of well documented cases followed up until growth ceases or stabilisation is established. We hope to inaugurate such a survey in collaboration with other large centers. In the present limited survey of 174 the only statistically valid conclusions were that age and type of treatment are relevant factors in producing lengthening. In the other factors investigated the survey was not large enough to provide valid answers. It has been established that after metaphyseal or diaphyseal fractures in children disturbances of growth may occur resulting in accelerated growth, retarded growth, axial deformities and rotational deformities. The literature, although extensive, leaves many questions unanswered while on other matters, such as the role of certain factors in the pathogenesis of these disturbances, there is often conflicting data. In a brief review of the literature we decided to simplify our research by confining our observations to data relating only to single diaphyseal fractures.

Selective arterial embolisation in aneurysmal bone cysts.

The treatment of three cases of aneurysmal bone cyst by selective arterial embolism is described. Two cases had involvement of the pelvis and one of the knee. The 2-year follow up has been promising. Radiologically the lesion has been arrested with increased density of the cyst wall and intra-cystic trabecular new bone formation. Pain relief has been complete and up to now no surgical treatment has been required.

Therapeutic problems in familial poly-epiphyseal dyschondroplasia.

The writers describe the clinical features of familial poly-epiphyseal dysplasia without spinal involvement affecting two sisters who both underwent surgical treatment with a view to restoring balance in load bearing joints. The satisfactory results are illustrated and "biological" osteotomy is advised in such cases, even if painful symptoms have not yet appeared.

[Chronic non-specific disease of the respiratory system and measurable alterations in workers in the chromium-plating industry (author's transl)].

Lung function test performed on 26 chromium plating workers showed a high prevalence of obstructive disease and of change in DLCO. The connection between lung function impairment and occupational exposure to chromium is suggested by the relationship between respiratory changes and biological indices of chromium absorption. The prevalence and the severity of ventilatory impairment also show an increasing trend with the duration of the exposure, while the smoking habitus seems not to be important.

[Chromium exposure biological indices and clinical findings in chromium plating industry (author's transl)].

According to the investigations carried out on workers of two chromium plating plants, the authors believe that chromium urinary excretion allows to determine the degree of its acute absorption. Moreover, the renal clearance of diffusible chromium allows the evaluation of chromium body burden and is related to the duration as well as to the severity of exposure. This interpretation is supported by the relation between the exposure biological indexes and the clinical and instrumental investigations which make possible the evaluation of lesions caused by chromium exposure, mostly concerning the respiratory system.
