Spinal surgery for spasticity (46 cases).

The authors report their experiences in the surgical treatment of spastic syndromes by operations on the spinal cord and nerve roots. In the authors' neurosurgical department 20 lumbar myelotomies, 19 posterior selective lumbar rhizotomies, 2 complete posterior lumbar rhizotomies, 4 anterior lumbar rhizotomies and five posterior cervical rhizotomies were performed. Clinical results can be considered good on the whole. There was no mortality and no major complication in the entire series. The effect of selective posterior lumbar rhizotomy on the evolution of cerebropathic spastic paraparesis was particularly impressive. Six of 46 patients experienced a late recurrence of spasticity, but they all had good results after a further operation. The particular criteria for selection of each surgical procedure are discussed and related to the aetiology and clinical picture of the spastic syndrome.

[Juvenile hypertension. Results of a study in high school in Milan (author's transl)].

Blood pressure has been measured in a cohort of 2300 high school students of Milan (14-15 years old) for two years on end. The prevalence of "borderline hypertension" was 11.5% at the first examination and 8.5% at the second. The values of blood pressure were decreased, in every measurement, after a year from the initial screening especially in "borderline hypertensives". This study has confirmed that the "borderline hypertension" in subjects 14-15 aged is a transient phenomenon, but the incidence of "stable hypertension" as been found higher significantly in "borderline hypertensives" than normotensives. The results are discussed.

The broken clip.

A case of a broken clip demonstrated by radiography is presented. As angiography showed no filling of the aneurysm, further operation was not considered necessary. The cause of the fracturing of the clip remains unknown.

[Arterial hypertension caused by ingestion of licorice].

The Authors present a case of licorice-sustained hypertension in a young man. blood and urine tests revealed: hypokaliemia with hyperkaliuria, suppressed plasmatic renin activity, absent urinary aldosterone. A symmetric left ventricular hypertrophy was detectable at the echocardiogram. Five months after the interruption of licorice ingestion the valves of systemic blood pressure were normal and the haematochemical investigations were also within normal limits. An echocardiogram revealed the disappearance of the signs of left ventricular hypertrophy and myocardial mass was reduced of about 40%. The usefulness of an echocardiographic examination, to evaluate hypertensive cardiomyopathy, is stressed.

Argentaffin pulmonary tumorlets.

The left lung, surgically removed from a patient, was found to be bronchiectasic and polycystic. Using light microscopy it was possible to locate multiple tumorlets originating from the Kulchitsky cells of the bronchial and bronchiolar mucosae. With the traditional histochemical staining, argyrophilia and argentaffinity were demonstrated. The latter, not previously reported in the literature, suggests the presence of different monoamines in the neurosecretory cytoplasmic granules of the tumorlet cells.

Possible aetiopathogenetic correlation between primary empty sella and arachnoid cyst.

Three cases of surgically treated non-neoplastic diseases of the sella (two arachnoid cysts and one empty sella) are reported. All three patients had visual deficits but no gross endocrine disturbances. The study of these three clinical cases and an analysis of the relevant literature suggest a possible unitary hypothesis regarding the pathogenesis of sellar arachnoid cysts and empty sella.

[Studies on the risk factors of arteriosclerosis in a health district region of Lombardy].

Atherosclerosis risk factor, 8,5% two, 2,7% three and 0,6% four.rs have been investigated in 194 males and 135 female, 25 years old. Elevated lipids were found in 7,3% of the sample; "labile" and stable hypertension in 2,7%; diabetes mellitus in 2,7% and overweight in 19%. 19% of the subjects smoked more than 20 cigarettes daily. Prevalence of hyperlipemia and electrocardiographic changes was higher among people in the lower socioeconomic classes. Taking into consideration hyperlipidemia, hypertension, cigarette smoking, overweight and diabetes mellitus as risk factors, 25% exhibited one risk factor, 8.5% two, 2.7% three and 0.6% four.
