[Echographic screening for autosomal dominant polycystic kidney disease from intrauterine life to adult life].

ADPKD is the most widespread genetic nephropathy. Only twenty-five per cent of patients are symptomatic. In order to prevent the disease it is important early screening by ultrasonography that shows a typical picture only in adults. Familiar history is determinant for diagnosis in intrauterine age, in newborns and in infants. Ultrasonographic diagnosis is certain only after twenty-five weeks of pregnancy.

IgA mesangial nephropathy associated with renal cell carcinoma.

The authors report the occurrence of proliferative glomerulonephritis with mesangial IgA deposits in a 47-year-old man, heavy smoker, affected with renal cell carcinoma at stage I. The relationship between neoplastic diseases and IgA glomerulonephritis is unresolved. It is possible that IgA mesangial nephropathy may occur as a paraneoplastic disease, but in this patient its association with renal cell carcinoma may be merely fortuitous.

Acetic acids bearing the 1-phenyl-1H-indazole nucleus with analgesic and antiinflammatory activity.

The synthesis of [(1-phenyl-1H-indazol-4-yl)oxy]acetic acid (IV), (1-phenyl-1H-indazol-4-yl)acetic acid (IX) and (1,5,6,7)tetrahydro-1-phenyl-4H-indazol-4-iminooxy)acetic acid (XIII) starting from 1,5,6,7-tetrahydro-1-phenyl-4H-indazol-4-one (I) is described. These compounds showed a good antiinflammatory activity in rats, and an analgesic activity in mice similar or comparable to that of indomethacin.

Effect of naloxone on plasma concentrations of prolactin and LH in lactating sows.

Six lactating sows were injected through an indwelling vena cava cannula with naloxone (2.5 mg/kg body weight) on Day 15 post partum. Blood samples were collected through the cannulas at 10-min intervals for 8 h before and 10 h after naloxone administration. Plasma prolactin and LH concentrations were measured by radioimmunoassay. Naloxone caused a marked suppression of plasma prolactin concentrations lasting 4-6 h. LH concentrations were also affected by naloxone: LH rose to reach maximum values 20-50 min after naloxone treatment. Pretreatment values were recorded 200-300 min after the treatment. These results indicate that endogenous opioids are involved in causing the endocrine patterns occurring during lactation, i.e. high prolactin and low LH concentrations.

Planned release dihydroergotamine in common migraine and "tension-vascular headache": multicentre clinical trial.

The antimigraine efficacy of dihydroergotamine methanesulphonate in the pharmaceutical form of planned release capsules has been evaluated. Forty-nine subjects were treated, 39 suffering from common migraine 10 from tension vascular headache, for a period of 90 days, with an oral dose of 15 mg twice daily. The product was found to be efficacious in 71% of the common migraine cases, while unsatisfactory or nil results were obtained in the remaining 29%. In the tension-vascular headache cases, efficacy was 40% compared to 60% unsatisfactory or nil results. Tolerance was good in 43 cases, while transient digestive disorders (nausea and vomiting) occurred in 6; in 2 cases the treatment had to be withdrawn for allergic erythema. The results demonstrate that the drug is indicated in cases of common migraine, while it does not appear to significantly influence the course of attacks of tension-vascular headache.

Monitoring of a long survival myeloma patient.

Monitoring with anti-idiotypic sera has been applied to identify tumoral cells in a myeloma patient still alive in complete remission 9 years after diagnosis. Monoclonal plasma cells displayed a labeling index that decreased in complete remission below 1%. The great majority of B lymphocytes belonged to the tumoral clone even in complete remission and were therefore not affected by conventional chemotherapy. Some aspects of this myeloma patient are discussed in the light of these immunologic and kinetic findings. In addition, it is suggested that the therapeutic management of the complete remission should be re-examined, by considering a lymphocytolytic therapy.

Pseudohypoparathyroidism: inheritance of deficient receptor-cyclase coupling activity.

Pseudohypoparathyroidism, type I (PHP-I) is an inherited disorder of primary resistance to multiple hormones that work by stimulating adenylate cyclase. In an attempt to clarify the mode of inheritance of PHP-I, we measured the activity of the N protein, a receptor-cyclase coupling component, in erythrocyte membranes. Erythrocyte N-protein activity was reduced by approximately 50% in erythrocytes of 15 PHP-I patients and was normal in 19 of their clinically normal first degree relatives. Reduced N-protein activity and the PHP-I phenotype in these families exhibited both dominant and recessive patterns of inheritance. This suggests that at least two distinct genetic loci are involved in inheritance of N-protein deficiency. In two additional families, dominant inheritance of the PHP-I phenotype was associated with normal activities of erythrocyte N protein. Thus, it appears that mutation of at least one additional genetic locus, not involving the N protein, can produce PHP-I.
