Electrocardiogram in progressive systemic sclerosis. Analysis of 73 cases.

Electrocardiographic abnormalities were found in 53 out of 73 patients (72,6%) affected with progressive systemic sclerosis or scleroderma. Along with some modifications of little value, like heart rate increase or decrease, sporadic premature beats, slight ST depression or T-wave flattening, other more important abnormalities were recorded. One patient had atrial fibrillation and one supraventricular tachycardia. In 21 cases (28.8%) conduction disorders were found, including A-V block, right bundle branch block, left anterior hemiblock and bifascicular block. Low QRS voltages were present in 15 cases (20,6%), confined in all but one to the peripheral leads. In 13 patients (17,8%) Q or QS aspects suggesting myocardial necrosis were observed, but a clinical history or clinical picture of myocardial infarction syndrome was lacking in all cases but one. Electrocardiographic patterns of myocardial necrosis in scleroderma may indicate not only myocardial infarction, which seems to be a rather rare occurrence in such disease, but also intraventricular conduction system defects or progressive replacement of myocardium by fibrous tissue.

[Tissue autoantibodies during therapy with levamisole for rheumatoid arthritis].

In 20 patients affected with rheumatoid arthritis, tissue autoantibodies were studied during a year of treatment with levamisole. Before therapy, antinuclear antibodies were present in 11 cases, anti-thyroid microsomes in 1 and anti-smooth muscle in 6. During therapy, autoantibodies remained almost unchanged in all patients, thus indicating that levamisole does not influence this aspect of humoral immunity in rheumatoid arthritis. Furthermore, no correlation was found between autoantibody pattern before treatment and effects of levamisole on clinical course of disease.

Idiopathic esophagopleural fistula in the newborn. A review, with a case report.

Idiopathic rupture of the esophagus in the neonate is a rare event, probably related to the same mechanism of ischemic necrosis responsible for other "spontaneous" g.i. tract perforations in the newborn. The laceration is usually located on the right aspect of the distal esophagus and is complicated by esophagopleural fistula and hydropneumothorax. Plain chest film and esophagography are diagnostic. The condition is an emergency one and usually carries a bad prognosis without prompt surgical repair. A typical case is reported in a baby who survived without early surgery; a residual tiny blind pouch and a small hiatal hernia required surgery at 1 year of age.

CT for assessing feasibility of separation of thoracopagus twins.

The value of the angio-computed tomography technique for evaluating communication between cardiac cavities of the two hearts in conjoined twins is reported. After injecting contrast medium into the peripheral vein of one of the twins, the increase in density values of the heart in the other twin suggested shunting between the two hearts.

[The phenomenon of accelerated A-V conduction. Electrophysiological study with a "selective" calcium antagonist (Verapamil)].

In order to discover slow response calcium dependent fibers within the structure with enhanced A-V conduction, an acute i.v. test with Verapamil (a highly selective calcium antagonist) was done in 13 patients with enhanced A-V conduction syndrome (4 had A-V by-pass, Kent type, associated). The action of Verapamil on the following parameters was studied: 1) A-H interval in sinus rhythms; 2) delta A-H interval (i.e. the maximum increment of A-H interval after incremental atrial pacing); 3) antegrade and retrograde function curves of the structure with enhanced A-V conduction; 4) R-R cycle of the reciprocating tachycardia, antegrade and retrograde conduction time (after identification of the reentry circuit) and pattern of induction and interruption of the tachycardia. Only in 1 patient a total atrio-hisian by-pass was identified. In all the other patients the electrophysiologic response to i.v. Verapamil suggested the presence of slow response calcium dependent fibers within the structure with enhanced A-V conduction which was always depressed by the drug even if to a lesser degree than in a normal A-V node. From these data and from the analysis of the epidemiology of enhanced A-V conduction, the Authors hypothesize that an infantile type A-V node could be in most cases the anatomical basis of enhanced A-V conduction while only rarely a total or partial A-V node by-pass.

[Early effect of levamisole on plasma immunoglobulins in rheumatoid arthritis].

In 8 patients affected with rheumatoid arthritis the effects of levamisole administration (150 mg/day once a week) on plasma immunoglobulins were studied. The concentration of IgG, IgA and IgM was measured before and after 15,30,60 and 90 days of treatment. We observed a significant increase of IgG after 30, 60 and 90 days, of IgA after 30 and 90 days, and of IgM after 15 and 30 dyas. Such response is in contrast with previously reported decreased of immunoglobulins most frequently observed after 6 months of drug administration. Therefore, it seems that levamisole at the beginning of its activity on immunocompetent cells enhances rather than decreases the antibody production.

Stereochemical consequences of dynamic gearing in substituted bis(9-triptycyl) methanes and related molecules.

Empirical force-field calculations show that bis(9-triptycyl)methane has a C(s) ground state and that the securely meshed triptycyl groups undergo nearly unhindered cogwheel rotation through a C(2) transition state, with an activation energy of approximately 1.0 kcal mol(-1). These features lead to note-worthy stereochemical attributes in appropriately substituted derivatives and analogs. Bis(9-triptycyl)carbinol has been prepared as an example of a compound that is chemically achiral, even though all conformations are chiral under the constraint of gear meshing. It is shown that, under the same constraint, ring substitution of either compound (or analogous systems) may lead to residual stereoisomerism. A permutational analysis of chemical isomers and isomerizations for every possible type of substitution pattern gives an enumeration of residual stereoisomers under the operation of various gearing and nongearing modes and provides information essential for a choice among mechanistic alternatives for a given isomer count.

Angiographic diagnosis of jugular venous system dilatation in children. A report of five cases.

We report five children with a soft mass in the neck due to congenital jugular venous ectasia. Three had fusiform dilatation of the internal jugular vein, which in one case was associated with dilatation of the ipselateral external jugular vein. Two children had aneurysmal dilatation of the superficial cervical communicating vein. The first four cases required angiographic studies for final diagnosis. Venography via the femoral vein was most valuable for visualization of the dilated segments of internal jugular veins but failed to show the vascular mass communicating with the superficial vein of the neck. These were best visualized by direct injection of the contrast medium into the vessel. In the fifth case a correct diagnosis was obtained with xeroradiography alone.

A common pathophysiology for sudden cot death and sleep apnoea. "The vacuum-glossoptosis syndrome".

Several congenital and acquired conditions, characterized by upper airway narrowing, may result in respiratory, cardiac, and sleep disturbances. In all these conditions the leading clinical feature is the occurrence of cyclic obstructive apnoea, mainly during sleep. The common pathogenic mechanism for the airway occlusion seems to be a backward displacement of the tongue and mandible, favoured by the muscle relaxation which occurs during sleep and by gravity in the supine position. A constant factor determining the glossoptosis is the pharyngeal vacuum occurring in these conditions as a result of inspiratory efforts in face of a narrowed upper airway. The response to this type of obstruction is age-dependent, since only in early infancy may these apnoeic spells be life-threatening. A similar pathophysiology is suggested for sudden cot death, which could be considered as a peculiar presentation of this "vacuum-glossoptosis syndrome". This hypothesis could explain why sudden cot death is often associated with nasopharyngitis and occurs always in infancy, almost invariably during sleep.

[The herniotomy operation in the treatment of congenital inguinal hernia].

--203 cases of congenital inguinal hernia operated with Mitchell-Banks technique have been studied. Just over half the patients were less than 1-year old; 48% were males; right inguinal hernia (53%) predominated, while 13% presented bilateral hernia. Post-operative hospitalization was 1-3 days in 73% of cases. Fever was observed in 86% of the children but it never lasted more than 2 days. 81 patients presented scrotal haematoma. Mortality was nil. Long-term follow-up showed that only 1% had suffered a recurrence. As a result of all these advantages and the low hospitalization (one day surgery), the technique is recommended for the treatment of infant inguinal hernia.

Low birthweight babies with oesophageal atresia or tracheo-oesophageal fistula.

Of 142 patients of known gestation with oesophageal atresia or tracheo-oesophageal fistula, 21% were preterm with a mean weight of 2 kg, 19% were small-for-dates with a mean weight of 2.23 kg, and the remaining 60% were born at term and weighed more than 2.5 kg. There were almost five times as many preterm and almost eight times as many small-for-date infants as in a normal population. Respiratory distress syndrome and additional severe congenital anomalies were commoner in babies of short gestational age, and pneumonia was more common in small-for-dates babies.
