A case of Goodpasture's syndrome with normal renal function is described. Clinical, morphological and prognostic features of the diagnosis are compared with the findings in Goodpasture's syndrome with progressive renal failure and with the findings in idiopathic pulmonary hemosiderosis.
The phenotypes of the protease inhibitor (PI) alpha-1-antitrypsin have been analyzed by isoelectric focusing on polyacrylamide gels. With improved resolution by a modified procedure it was possible to demonstrate a fifth PI*M suballele. The bands of PI M5 are located between PI M1 and PI M3. In addition, a further deficiency allele similar to PI*Z was found in a female patient with obstructive pulmonary disease. This variant was provisionally named PI Z augsburg (PI Z aug). Family data confirm a simple codominant mode of inheritance for PI Z aug.

Eight cases of endothoracal carcinoma associated with pulmonary fibrosis or with interstitial pneumonia are reported. The interstitial pneumonia and the fibrosis and carcinoma are confirmed in all cases. Statements on incidence cannot be made: they are more or less random observations in recent years. There are, however, grounds for supposing that this coincidence is by no means rare. Problems arise in differential diagnosis and the decision to operate, for irradiation therapy and for chemotherapy. The question of a possible causal connection, whether in the sense of a paraneoplasia or of cancer development due to previous damage to the lung is left open.

The term "exogenic pneumopathology" means a cluster of problems resulting from the still unknown consequences of the exposure of workmen to various industrial noxious agents. Progress is to be expected from advanced methods of examination: Perbronchial lung biopsy is a new chance to solve the problems. With perbronchial biopsy we get lung tissue in about 90% of examined cases. The complication rates are low: pneumothorax developed in about 4% controllable bleedings occurred in 1.2%. Relevant diagnostic results were possible in about 60%. From the tabulated global results after more than 1400 perbronchial lung biopsies, the occupational diseases and the lung fibrosis are especially pointed out and discussed in detail.
