Efficacy of two temephos formulations against Chironomus salinarius (Diptera: Chironomidae) in the saltwater lagoon of Venice, Italy.

Two formulations of the organophosphorus insecticide, temephos (Abathion Granulare, 1% AI granular and Tambro Compresse, 2% AI tablet) were evaluated against Chironomus salinarius midge larvae in 50 x 50 m experimental plots in the saltwater lagoon of Venice, Italy. Each formulation was applied at 0.2 and 0.4 kg AI/ha. Abathion Granulare produced 56 to 73% larval reduction at 0.2 kg AI/ha and 69 to 83% reduction at 0.4 kg AI/ha during 3 wk after treatment. Abathion Granulare lost effectiveness at 4 wk after application at both rates. Posttreatment larval reductions resulting from Tambro Compresse applications ranged from 77 to 86% for 3 wk, and 82 to 92% for 4 wk at rates of 0.2 and 0.4 kg AI/ha, respectively. The tablet formulation (Tambro Compresse) gave better control of C. salinarius (magnitude and duration) than the granular formulation (Abathion Granulare) in these evaluations.

Atypical cilia in the tracheal epithelium of healthy horses.

Specimens of the tracheal epithelium were obtained from two sites in nine healthy horses by a fibre optic endoscope. Electron microscopic examination of 53,550 cilia revealed that 5.2 per cent of cilia were atypical. Nine hundred and seventeen (1.7 per cent) were compound, 78 (0.15 per cent) were swollen, 27 (0.05 per cent) were intracytoplasmic and 171 (3.35 per cent) had microtubular defects. These microtubular defects (159 peripheral and 12 central) were found in 5103 cross-sectioned cilia.

[Spontaneous remission of visceral metastases of hypernephroma after nephrectomy. Description of a case and review of pathogenetic hypotheses].

We describe an absolutely spontaneous regression of visceral metastases from a renal carcinoma, following nephrectomy, in a 46 years old patient, who is at present in a complete uninterrupted remission for 39 months. After a literature review we point out the rarity of this event and emphasize its absolute spontaneity. Moreover, we suggest some considerations about the possible pathogenetic mechanisms of this unexpectedly positive outcome. Between these, because of the strongly positive psychologic attitude of the patient, the psycho-neuro-immunologic one is suggestive.

[Current role of hysterosalpingography in the assessment of female infertility: review of a series of cases].

The authors reviewed 463 hysterosalpingographies from a series of patients examined for primary infertility. 25.5% of them had been studied with echotomography and 4.1% with therapeutic and explorative laparoscopy. Hysterosalpingographic results were compared with positive findings at clinical examination and with various pathologic associations: the method proved to be extremely valuable in the diagnosis of the primary causes of female infertility, and even showed secondary, sometimes unsuspected, causes--e.g., iatrogenic and inflammatory. Moreover, high agreement (50%) was observed with echotomographic findings. The examination was carried out on an outpatient basis, and proved less dangerous and cheaper than other invasive techniques, its cost being more or less the same as that of US, which however is still the least dangerous and expensive method for the diagnosis of female infertility. Therefore, the authors suggest that diagnostic laparotomy be a second choice and be employed only in the cases where it can have therapeutic purposes. In all the extant cases, combined hysterosalpingography and US are still recommended.

Nonrandom chromosome changes in multiple sclerosis.

In order to study the role of genetic factors in multiple sclerosis, cytogenetic analysis was performed on 48 patients with the clinically defined disease. We found a high incidence of subjects (50%) with abnormal chromosomes, showing premature centromere division of the X chromosome and structural aberrations, translocations, or deletions that could suggest preferential breakpoints. Correlation between clinical and cytogenetic data showed that cytogenetic abnormalities were more common in patients with high frequency of relapse or with a progressive form of the disease.

Cyclophosphamide in relapsing remitting multiple sclerosis.

7 patients with relapsing-remitting multiple sclerosis (MS) were subjected to an intensive course of intravenous (I.V.) cyclophosphamide (CY) therapy. All patients received induction therapy with 11 daily doses of 300 mg/m2 and then a single dose every six months for three years. After one year of follow-up all patients showed a decrease in relapse rate (0.57.57); in the two subsequent years of follow-up 2 patients showed a mild worsening while the others were clinically stable. As suggested by others, our results indicate that I.V. CY therapy may influence the clinical course of relapsing-remitting MS.

Reconstruction of hypospadias with a vertical preputial island flap. A follow-up study of 127 patients.

The preputial island flap technique presented here was described by Scuderi for a one-stage repair of hypospadias. The operation utilizes a subcutaneous pedunculated preputial flap, designed dorsally along the vertical axis of the penis and transposed ventrally to reconstruct the new urethra. This surgical procedure is useful in distal and proximal hypospadias or in cases with short or absent prepuce, when the dorsal coating of the penis can be used. The technique permits a complete release of the chordee and a meatus placed at the apex of the glans. One hundred and twenty-seven patients treated during the period 1982 to 1987 were analysed. All patients had a urodynamic check and some also had a urethroscopic evaluation of the new urethra. The incidence of complications (8.65%) was very low compared with other techniques.

[Surgical treatment of blepharoptosis in patients with mitochondrial myopathies. Apropos of 4 cases].

The authors briefly describe the commonest forms of mitochondrial myopathies particularly those associated with disorders of ocular motility. They report their series of four patients suffering from ptosis of the eyelid caused by mitochondrial myopathies. The ptosis was corrected surgically in all cases by means of Müller's muscle adaptation technique. The authors discuss their results and the indication for this technique.

[Glaucoma as etiopathogenic hypothesis of amaurosis after blepharoplasty. Apropos of a clinical case].

Amaurosis after blepharoplasty is a complication which is considered to be rare, but which nevertheless deserves extreme attention. Although various hypotheses have been proposed (and sometimes proved) its etiopathogenesis is still uncertain. The observation of a case of temporary loss of vision after blepharoplasty, which promptly regressed after medical therapy, led us to a revision of the most common causes of amaurosis, and to formulate, for this case, the etiopathogenetic hypothesis of acute glaucoma, which has already been reported in scientific literature, but without any case report.

[Changes in headache with age: symptomology and clinical features].

The clinical features of 76 patients 5 to 14 years old suffering from primary headaches has been compared with those of 495 adults in the same diagnostic group. An association between migraine without aura and female sex was evident in both age groups, while an inversion of the usual M/F ratio was found in the younger group with other types of headache. Childhood headache attacks resulted to be less frequent, less severe and with a shorter duration than in adult patients. Also accompanying symptoms were less common in children than in adults. Furthermore pain was more often unilateral but less frequently pulsating in the younger group. These results point out the need for a different approach to childhood headaches.

Oestrogen and progesterone sensitivity in cultured meningioma cells.

Several studies have detected oestrogen and progesterone receptors in meningioma specimens; recently we have also confirmed the presence of steroid receptors in cultured cells from meningiomas. This paper describes the oestrogen and progesterone receptor assay in cultured cells from 6 meningiomas and the influence of steroid hormones on the cell growth and morphology. Four (66%) of the 6 specimens were positive for both receptors. Growth of cultured cells from tumours without receptors is not appreciably modified by the addition of hormones; the cultured cells from tumours with positive receptors are not essentially influenced by oestrogen, whereas progesterone produces a rapid and marked suppression of the cell growth and modifies their form and adhesivity; also the addition of an oestrogen and progesterone blend produces growth suppression. A similar effect of the progesterone on the cultured cells has also been obtained in a specimen of malignant meningioma. The results of this study suggest that the modulation of progesterone levels may be of therapeutic usefulness, particularly in patients with recurrent malignant meningiomas.

[Role of plastic surgery in male sterility].

The Authors discuss the problems related to male infertility, pointing out their different clinical and therapeutical features. They stress the importance of plastic surgery in the treatment of disorders such as penile chordee, hypospadias as well as nervous and vascular lesions.

Nonobstructive symptomatic plaques of the carotid bifurcation: echotomographic-angiographic correlations.

This study reports a selected group of 45 patients with symptomatic low-grade stenoses of the carotid bifurcations, detected by real-time B-mode echotomography, and correlates the results of this technique with those of the Doppler flow study and carotid angiography. The Doppler study with spectral analysis was negative in all cases, whereas a correlation between the echotomographic and angiographic results was observed in 20 among 27 patients explored by angiography (74%). The causes of failure of angiography in some cases are briefly discussed. The almost absolute sensitivity of high resolution B-mode echotomography in detecting these early, sometimes symptomatic atherosclerotic changes of the carotid artery wall is emphasized.

A prevalence study of multiple sclerosis in L'Aquila, central Italy.

We report the results of an epidemiologic study in the Municipality of L'Aquila, Abruzzo, Central Italy, undertaken to estimate the prevalence rate of Multiple Sclerosis (MS). On December 31, 1984, the prevalence was 33.2 per 100,000 (34.2 when age and sex is standardized to the Italian population). In agreement with recent intensive surveys from different regions in the North and South of Italy, our data indicate that the prevalence of MS in Italy is higher than 30 per 100,000, and emphasize the importance of small population studies in the epidemiology of MS. Our findings suggest that in Italy the risk for MS is higher than recorded before.

[Clinical evaluation of aztreonam in the prevention and therapy of infections in patients subjected to neurosurgical intervention].

Twenty-six neurosurgery patients were given prophylactic treatment with Aztreonam at a dose of 1 g every 8 hours (30 mg/kg every 12 hours in children) for 8-12 days after the operation. In 18 cases 300 mg Clindamycin was also given every 8 hours for 8 days. No local or general infections arose in the postoperative period. Three patients with infections caused by susceptible bacteria were given 1 g Aztreonam every 6 hours (30 mg/kg every 8 hours in children) for 8 days. In all cases all clinical and microbiological signs of the infection had disappeared by the end of treatment. Apart from one episode of mild nausea no side effects were noted. Aztreonam is therefore considered a safe and effective drug for the treatment and prophylaxis of postoperative infections in neurosurgery.

Acute normovolemic hemodilution in aesthetic plastic surgery.

Acute normovolemic hemodilution (ANH) is indicated and useful in all procedures of plastic surgery in which great blood loss is expected. Here we present the advantages of ANH such as avoidance of incompatibility reaction, no risk of disease transmission, better microperfusion, economic and psychological advantages, and saving blood.
