[Differential diagnostic problems in psychiatric diseases of involution illustrated with a case of abortive Osler disease].

Differentialdiagnostic problems of psychic diseases in the lifetime of involution commented on a case of abortive Osler's disease. From a differentialdiagnostic view, common problems of ranging in forms of psychosis in the age of involution are discussed by demonstration of a several years persisting and interesting course of psychosis in involution in a case of abortive Osler's disease, sustained by affectiveness. We underline, that in reflections of differentialdiagnostic value about psychosis of the involutional age, it is of priority, to identify a clear contoured psychic disorder as an accessory one in a primary somatic disease, or as a representative syndrome of a primary psychic disease. Somatic findings, in particular those of vascular origin (neuroradiological findings in the present case too) may complicate the problematic diagnostic efforts anyhow in the group of psychosis in the age of involution too.

[Anesthesia dolorosa in the higher ages - example of problems in therapy (author's transl)].

Chronic forms of facial pain in higher decades of age demonstrate problems of therapy in this period of life. Following herpetic infections and operations in the course of the trigeminal nerve the anesthesia dolorosa is a complication often resistant to therapy. Observations in two patients with typical pain syndrome made clear, that besides common problems of therapy in the higher ages, disorders of communication influence especially every cooperation and every step of therapy, which the patient does. Independent os success in therapy, longtime-care and permanent allocation in the older patient must compensate the deficit of communication.

[Methylphenidate addiction--Reversal of effect on withdrawal].

A case history of a prolonged ritalin addiction (18 years) is given, describing the symptoms and the diagnostic categorisation. Because of the remarkable, somewhat atypical withdrawal symptoms, it is suggested to conceive these withdrawal symptoms as an expression of a 'reversal of effect on withdrawal' in the sense of a continuing, independent manifestation of counter regulatory, protective and specific mechanisms.
