[Vaginal sonographic follow-up of embryo development].

The clinical study reports on facilities of transvaginal sonography to demonstrate early embryological structures. A collective of 659 sonographical examinations in 593 women between 4. and 13. week of gestation (wog) is described. The earliest imagery of an intrauterine pregnancy could be performed at 28. day post menstruation (p.m.) and first heard beats could be detected at 36./37. day p.m.. The secondary yolk sac could be discovered at the end of 5. wog and first motions of the embryo in 8. wog. Certain structures as parts of the skull, brain and spine were clearly to be differentiated in 10.-12. wog. Transvaginal sonography renders an very early and comprehensive control of regular embryological growth and development. Striking malformations are early detected.

[Control of postoperative vaginal sonographic findings following Shirodkar cerclage].

Up to now postoperative control of the uterine cervix following cerclage was performed by bimanual palpation. Since clinical use of transvaginal sonography it is possible to get objective results about preoperative morphology of the cervix (exact cervical length, dilatation of the cervical canal and opening of internal and external os). Besides postoperative vaginal sonography following cerclage can ascertain lengthening and stabilization of the incompetent cervix and localize the ligature's position. 75 pregnant women between 15th and 30th week of gestation were examined using a 5-MHz vaginal sector scanner probe following Shirodkar cerclage to gain information about the residual cervical length and the distance from the surface of the ectocervix to the ligature's position within the anterior and posterior lip of the cervix. The mean postoperative cervical length was 3.75 cm (1.5 cm-6.5 cm). Compared to the preoperative average length of 3.0 cm (0.5 cm-5.0 cm) the cerclage resulted in an increase of about 25%. The average distance from the surface of the ectocervix to the ligature at anterior lip of the cervix was 1.85 cm, ranging from 0.9 cm to 2.6 cm. The mean value at the posterior lip was 1.56 cm, ranging from 0.8 cm to 2.0 cm. This study showed that the actual positions of ligatures after cerclage operations varied very much. Unsatisfying position can be an explanation why some preterm deliveries can not be prevented. Therefore it is recommended to control the position of the cerclage ligation postoperatively using transvaginal sonography.

[Indication for and benefit of 574 cerclages performed at the Medical University of Hannover (1987-1985)].

574 cervical ligations according to the method of Shirodkar had been performed at the Department of Obstetrics and Gynecology during the years 1978 to 1985. In all cases the "Cervix Set" of Braun-Dexon was used. Compared to the number of 11,397 deliveries in this period, there is a frequency of cerclage operations of about 5%. The average age of the patients had been 29.2 years. Most of the operations had been carried out between the 17th and the 20th and between the 25th and the 30th week of gestation, 20.6% and 34.9%, respectively. 512 women delivered in our hospital, the records of these patients could be evaluated further. 185 women had been primiparae, 225 secundiparae and 102 pluriparae. In a retrospective study the singleton pregnancies (n = 481) had been grouped due to the indication for cerclage: group "obstetric history", group "dilated internal os", group "dilated external cervical os plus decreased cervical length", group "visible fetal membranes" and group "premature contractions plus threatened preterm delivery". Twin and triplet pregnancies (n = 31) had been evaluated separately. Only 48.8% of the multiparae patients had already delivered a viable child, while after cervical ligation 93.9% of the children survived beyond the perinatal phase. Before the 37th week of gestation delivery occurred in only 19.3% of the cases. According to maturity stage of the newborn and to the time period between removal of the cervical ligature and delivery we judged the effectiveness and the necessity of Shirodkar operation in the different patient groups. The results could demonstrate the high importance of this treatment especially in patients with "visible fetal membranes".(ABSTRACT TRUNCATED AT 250 WORDS)

[Control of early pregnancies by vaginal sonography].

151 patients with suspected early pregnancies between 5 to 10 weeks were examined by transvaginal section scanning. Pregnancies could be verified as early as 16 days after conception while the first heart beat actions were seen after 21 to 22 days. In early pregnancies where heart actions could not be shown at first examination a second look was taken after the 6. week dated from conception. beta-HCG-units were found in a range of 59 to 55,000 mU/ml. 3 pregnancies in the 5th, 6th, respectively 7th week escaped our detection by the first examination. In 44 cases screened after the 6th week we did not prove heart actions. In all of these patients controlled measurements showed either values below or borderline low compared to normal scales.

[Tubal pregnancies: early pregnancy factor, progesterone, beta-HCG and vaginal sonography as differential diagnostic parameters].

By anamnestic, clinical, biochemical (beta-HCG), and ultrasonographical data, only a portion of ectopic pregnancies can be diagnosed early enough to allow conservative surgery. In the present study, the diagnostic value of the Early Pregnancy Factor (EPF), progesterone, and endosonography was investigated in 142 patients with suspected tubal pregnancy. beta-HCG and Progesterone serum levels had been in the lover range of or below normal values in most cases of tubal pregnancy compared to intrauterine pregnancies. The EPF was found to be negative in 94% of all cases of ectopic or disturbed intrauterine pregnancies, while all these patients showed a positive pregnancy test (HCG). By abdominal ultrasonography, only 73.7% of all tubal pregnancies could be detected, while transvaginal scanning increased the rate of verified diagnoses up to 90.6% of these cases. Our results suggest that EPF measurement and endosonography are valuable supplements for precise and rapid diagnosis of ectopic pregnancies.

[Vaginal sonographic assessment of a cross section of the amniotic sac in early pregnancy].

The present study describes the results of gestation sac measurement in 237 cases in early pregnant women by the technique of transvaginal sonography. Mean distance between uterine cavity and vaginal scanner probe is the reason for early detection of the gestation ring. Comparing with the transabdominal method the gestation sac can be ascertained one week earlier when performing transvaginal sonography, so that a detection of an intrauterine ring-shaped structure is possible to a time when in case of an unknown pregnancy regular menstrual bleeding is expected. Up the 10th week of gestation the amnion sac diameters are constantly smaller than the diameter obtained by transabdominal sonography, from the 11th week on results in both techniques are approximately equal. The early measurement of gestation sac diameter is usefull to determine correct gestation age in case of unknown conception date. From our point of view the gestation ring diameters, obtained by transabdominal technique should not be used father more for pregnancy determination when choosing the transvaginal method.

[Vaginal sonography versus vaginal palpation: initial experiences in 120 pregnant patients with suspected cervix insufficiency].

In a clinical study a group of pregnant women with suspected cervical incompetence was examined by vaginal sonography. Aim of the investigation was to compare results of performed vaginal palpation with results of sonography. 120 pregnant women with cervical insufficiency between 16th and 33rd week of gestation were examined by a 5-MHz vaginal sectorscanner probe. After focussing sagittal projection of uterine cervix and lower uterine segment the cervical length and opening of the internal os were assessed prior to cerclage. Postoperative vaginal sonography was performed to ascertain lengthening and stabilization of the incompetent cervix. Another group of 50 pregnant women with unsuspicious obstetrical findings were also examined to gain information about normal sonographical morphology and length of the competent uterine cervix. Comparing results of vaginal palpation and vaginal sonography showed, that the cervical length obtained by sonography was constantly higher in all patients than the results obtained by palpation. This difference became more distinct in the group of patients with extreme cervical incompetence. We are of the opinion that vaginal sonography is an objective method revealing the extent of cervical incompetence. Exact measurement of the cervical length and assessment of the internal os are efficient diagnostic criteria. They complete results of cervical palpation and offer precise information concerning an intended cerclage. In case of suspected cervical incompetence continuous sonographical examination can supervise the development of the uterine cervix during pregnancy. In future the number of prophylactic cerclage-operations perhaps decreases by using the technique of transvaginal sonography.

[Acute polyhydramnios in a twin pregnancy--case history of a successful therapy attempt].

Acute polyhydramnios is a rare complication in twin pregnancies. The perinatal mortality is about 100% without treatment. We present a case of twin pregnancy with an early acute polyhydramnios which was treated with serial transabdominal amniocentesis. 11 litres of amniotic fluid were removed and we succeeded to prolong the pregnancy by 13 weeks. In the 35th week of gestation, two healthy newborns were delivered by cesarean section. After exclusion of fetal malformations, we recommend the early beginning of serial amniocentesis to normalize the intrauterine pressure and to prevent premature labor or cervical incompetence. Compared to the poor prognosis of an untreated pregnancy, we consider the punction even of large volumes of amniotic fluid as a procedure of an acceptable risk.

[Delayed bone growth as a sonographic sign in the early detection of Smith-Lemli-Opitz syndrome].

The Smith-Lemli-Opitz syndrome was first described in 1964. It is inherited as an autosomal recessive trait. Characteristic manifestations are abnormalities of the facial skull, a delayed growth, lazy feeder, zygodatly and a cryptorchism in males. The case of a girl is presented born with this syndrome likewise her sister. Diagnosis was confirmed in the 30th gestational week by means of ultrasound. A retardation of bony growth and telltale signs of this syndrome were observed. At birth length of diaphysis differed 5 weeks compared with standard.

[Transvaginal ultrasound biometry of crown-rump length in the 1st trimester].

The embryonary crown-rump-length (CRL) in 225 pregnant women between the 6th and 13th week of gestation was measured by the technique of transvaginal sonography. A curve and a rate of growth was determined. The investigation showed that the CRL can already be measured in the 6th week of gestation when using transvaginal sonography, whereas transabdominal sonography makes a CRL measurement in the 7th week possible. A comparison of results and growth rates revealed a deviation up to 12th week of gestation age in both ultrasound methods. The known tabels with embryonary data, obtained by transabdominal technique, should not be used any longer for CRL determination, when prefering the technique of transvaginal sonography.
