Quantitative cerebral blood flow and metabolism determination in the first 48 hours after severe head injury with a new dynamic SPECT device.

OBJECTIVE: To determine cerebral blood flow (CBF) and metabolism in the acute phase after severe head injury by a new dynamic SPECT device using 133Xenon and to evaluate a possible role of CBF and metabolism in the determination of prognosis. DESIGN: Prospective study. SETTING: General intensive care unit in a universitary teaching hospital. SUBJECTS: 23 severely head injured patients having CT scan and CBF determination, intracranial pressure (ICP) and jugular bulb oxygen saturation monitoring in the first 48 hours. MEASUREMENTS AND MAIN RESULTS: CBF varied from 18.0 to 60.0 ml/100 g/min. No correlation was found between early CBF and severity of trauma evaluated with the Glasgow Coma Score (GCS) (F = 2.151, p = 0.142) and between CBF and prognosis at 6 months evaluated with Glasgow outcome score (GOS) (F = 0.491, p = 0.622: rs = 0.251, p = 0.246). CMRO2 was depressed in relation to the severity of injury, specifically ranging from 0.9 +/- 0.5 ml/100 g/min in patients with GCS 3 to 1.7 +/- 0.8 ml/100 g/min in patients with GCS 6-7. In no patient with CMRO2 less than 0.8 ml/100 g/min was a good outcome observed. A significant correlation was found between GCS and GOS (rs = 0.699, p = 0.0002), between CMRO2 and GOS (F = 4.303, p = 0.031; rs = 0.525, p = 0.013) and between AJDO2 and GOS (F = 3.602, p = 0.046; rs = 0.491, p = 0.017). Fronto-occipital ratio (F/O) of CBF distribution was significantly lower than normal values (chi 2 = 18.658, p = 0.001) but did not correlate either with prognosis (chi 2 = 1.626, p = 0.443) or with severity (chi 2 = 1.913, p = 0.384). CONCLUSIONS: CBF in the first 48 hours after trauma varies within a large range of values and is not correlated with severity and prognosis. Clinical evaluation with GCS and CMRO2 are much more reliable indicators of severity of head trauma and have a significant role in the determination of prognosis. F/O ration is significantly altered from normal values confirming "post-traumatic hypofrontalism" but does not correlate with severity and prognosis.

Ischaemic myelopathy associated with cocaine: clinical, neurophysiological, and neuroradiological features.

Two patients with spinal infarction and one patient with the previously unreported complication of spinal transient ischaemic attack associated with cocaine misuse are reported. Spinal MRI documented an infarction in the territory of the anterior spinal artery in the first two patients and was completely normal in the patient with a transient ischaemic attack. Motor evoked potentials were abnormal in all three patients.

Human and recombinant erythropoietin stimulate erythropoiesis in the goldfish Carassius auratus.

Erythropoietin (EPO) stimulates differentiation of erythrocytes and biosynthesis of haemoglobin in mammals. It has not been confirmed that EPO is effective in other vertebrates, so we attempted to verify the effectiveness of human-urinary and recombinant EPO on erythropoiesis of the goldfish Carassius auratus. The results indicate that both types of EPO produce an increase in haemoglobin, erythrocytes and erythroblasts number in peripheral blood of the goldfish; moreover, they stimulate erythropoiesis in lymphomyeloid tissue of kidney which, in normal conditions we find to be prevalently myelopoietic.

[Urodynamic picture in high spinal cord ischemia, induced by addictive use of cocaine. Report of a case].

The authors present a case of spinal cord ischemia at level C2 related to cocaine use. The neurologic pattern was an anterior cord hemisection syndrome. Urodynamic assessment was performed at 6 and 14 months from the neurovascular event. The ice-water test (IWT) showed an uninhibited bladder cooling reflex, despite an areflexic detrusor (at routine filling cystometry). Occult bladder hyperreflexia became overt during the following month and was registered by the second urodynamic assessment. Anatomical bases of spinal cord blood supply are discussed. The authors emphasize the role of the IWT in predicting the development of overt bladder hyperreflexia and thus, spinal shock phase resolution.

The blood cells and haemopoiesis of Diplodus sargus L.: haematological values, cytochemistry and leukocytes' response to vaccine stimulation.

The treatment with antipiogenic and trivalent antiviral vaccines evokes in sargus' blood an increase of the neutrophilic granulocytes and an activation of the lymphocytes. By using cytochemical reactions for acid phosphatase and aliesterase, a population of lymphocytes, amounting to about 25%, which displays positive reactions has been identified. The immunocytochemical reactions used to identify the mammalian T and B-lymphocytes, such as rosetting test, immune localization of CD (Cluster of differentiation) and of sIg, were performed with the aim of better classifying the lymphocytes of sargus, even though the use of mammalian monoclonal CD antibodies cannot be considered a certain proof of such a presence. Moreover, cytochemical reactions have revealed the presence of lysosomal enzymes in neutrophils and the presence of peroxidase which characterize the eosinophils. Hemopoiesis takes place in the spleen and in the lymphomieloid tissue of the kidney.

Neurogenic pulmonary edema: a presenting symptom in multiple sclerosis.

Altered cardiovascular and respiratory function is uncommonly encountered in multiple sclerosis, though it may appear late in the course of the disease [4]. Episodes of acute ventilatory failure due to autonomic and/or voluntary respiratory function paralysis have already been described. These episodes are often accompanied by a focal neurological deficit which expresses lesion at the level of the medulla [6]. A demyelinating bulbar lesion leading to altered cardiovascular function is likewise infrequent but when it happens, bradycardia, postural hypotension [2], or acute pulmonary edema without heart failure may occur [1]. We present a case of non cardiogenic acute pulmonary edema which had neither a toxic insult nor an infective agent as etiology, but appeared as the initial manifestation of a multifocal demyelinating syndrome.

Serial multimodality-evoked potentials in severely head-injured patients: diagnostic and prognostic implications.

OBJECTIVES: To assess the prognostic reliability of multimodality-evoked potentials and to evaluate the diagnostic implications and define the limits of these evoked potentials. SETTING: An ICU in a university hospital. DESIGN: Prospective clinical study. PATIENTS: Seventy-three severely head-injured patients aged 10 to 75 yrs. METHODS: Serial recording of brainstem auditory-evoked potentials and somatosensory-evoked potentials between days 1 and 21 after trauma. Comparison between evoked potential findings and prognosis, along with clinical data. RESULTS: Considering the single recordings of both brainstem auditory-evoked potentials and somatosensory-evoked potentials, the accuracy of prognostication in predicting a bad outcome was good only for severely abnormal brainstem auditory-evoked potentials. Serial brainstem auditory-evoked potential recordings and simultaneous recordings of brainstem auditory-evoked potentials and somatosensory-evoked potentials proved to be good prognostic indices in predicting a favorable outcome. Brainstem auditory-evoked potentials correlated well with brainstem reflexes and with pupil asymmetries but did not correlate with Glasgow Coma Scale scores. CONCLUSIONS: Serial recording and the use of a multimodality approach provided the best prognostic capabilities. The main diagnostic implications were: a) the possibility of detecting brainstem compression by means of brainstem auditory-evoked potentials before the appearance of pupil abnormalities; b) the usefulness of brainstem auditory-evoked potentials in monitoring brainstem function in patients undergoing high-dose barbiturate therapy. The main limitations of evoked potentials were the occurrence of peripheral acoustic damage, the electromagnetic sources of artifacts in the ICU, and the administration of ototoxic drugs.

Postoperative toxic shock syndrome following adrenalectomy: a case report.

This report describes a fatal case of toxic shock syndrome following surgical procedure of adrenalectomy. Toxic shock syndrome is a severe multisystemic illness associated with Staphylococcus Aureus infection. The disease is usually associated with menstruation and tampon usage. However, it has recently been reported in the postoperative period following simple surgical procedures. The surgical wound does not usually appear infected. The syndrome is associated with specific strains of Staphylococci producing the toxic shock syndrome toxin 1 (TSST-1), that mainly contributes to the illness. The major clinical signs are: fever, diarrhea, cutaneous rash and hypotension. Toxic shock syndrome requires early recognition and prompt aggressive symptomatic treatment based essentially on fluids administration, appropriate intravenous antibiotics and corticosteroids.
